        Grapefruit 종자추출물의 안전성 검사

        광범위한 천연항균제로 그 활용이 고려되고 있는 grapefruit 종자추출물의 안전성을 검토하기 위하여 급성 경구독성, 피부자극 및 급성 안점막 자극시험을 수행하였다. 실험 흰쥐에 대한 급성 LD_(50)값은 3.75g/㎏이었으며, 피부에 대한 독성은 약한 편으로 부종 등의 병변이 거의 생기지 않았으나 눈에 대한 자극성이 있는 것으로 나타났다. Effects of grapefruit seed extract on the lethality, the primary skin irritation, and the acute eye irritation potentials were evaluated in rats. The acute oral LD_(50) of GFSE for Sprague-Dawley derived rats was 3.75g/㎏ with 95% confidence limits of 2.55 and 5.52g/㎏. Patch application to the intact and abraded skin of rabbits led to mild to moderate erythema and no or mild edema. Application to the eyes of rabbits led to severe corneal opacity, iritis, conjunctival erythema, edema, and discharge.

        It was reported that the main cause of intraepithelial neoplasm and squamous cell carcinoma of the uterine cervix is human papilloma virus infection, and that the expression of p16 is increased in cells infected by human papilloma virus. We performed an immunocytochemical staining for protein p16 in 17 cases of cervocovaginal smears initially diagnosed as atypical squamous cells of undetermined significance, to know whether the staining could help the differentiation of neoplastic cells from reactive atypical cells. Of 17 smears, 6 were diagnosed finally as high grade intraepithelial neoplasm or invasive squamous cell carcinoma by follow-up biopsy and smear, and 5 of the 6 were positive for p16. Three were diagnosed as koilocytosis, and one of them was weakly positive for p16. Eight were diagnosed as reactive atypical cells, and all of them were negative for p16. We thought that immunocytochemical staining of p16 in cervocovaginal smears could help the differentiation of neoplastic cells from reactive atypical cells.

        신생아에서 급성 복증으로 발현된 Meckel 게실 1예

        출생 후 건강하게 지내던 중 생후 9일째 급성 복증 및 장 폐쇄의 증상으로 내원한 남아에서 시험적 개복술 시행하여, Meckel 게실 천공과 장 유착을 발견하고 수술적으로 제거한 후 회복된 증례이다. 병리 소견상 경한 게실 염과 정상 장 점막 소견을 보여 자발적 Meckel 게실 천공이라고 추정 되는 증례이다. 신생아시기에 자발적 Meckel 게실 천공은 매우 드물지만 심각한 질환이다. 따라서 신생아 시기에 급성 복증 증상을 보일 때 반드시 감별 진단 목록에 Meckel 게실을 포함시켜야만 한다. Meckel's diverticulum is generally acknowledged to be the most prevalent congenital anomaly of the gastrointestinal tract. The preoperative diagnosis of Meckel's diverticulum is difficult, especially in neonates, because of the lesions ability to masquerade as one of a variety of much more common abdominal pathologies. Recently we experienced a case of perforated Meckel's diverticulum with mild inflammatory reaction, intestinal adhesion, and small bowel obstruction in a previous healthy 9-day-old neonate. The spontaneous perforation of Meckel's diverticulum in neonate is very rare but serious entity. The course was rapid and progressed to sepsis. So despite its varied presentation, Meckel's diverticulum should be kept in mind as a cause of acute abdomen in neonates.

        상부 위장관 증세가 있는 소아의 위십이지장병변 및 Helicobacter pylori 감염

        목 적: 만성 반복성 복통, 식사 후 상복부 불쾌감, 잦은 구토나 구역질이 있는 소아를 대상으로 위내시경을 시행하여 위십이지장 병변을 확인하고, 생검체를 이용한 위십이지장 조직학적 검사와 H. pylori 검출 그리고 면역블롯팅을 통해 혈청 내에 H. pylori 특이 항체 존재를 확인하여 한국에서 관찰되는 소아의 위장관 증세와 위십이지장병변 및 H. pylori 감염과의 관계를 조사하고자 하였다. 방 법: 1990년 6월부터 1991년 4월까지 경상대학교병원 소아과에서 상부 위장관 증상으로 위내시경을 시행 받은 184명 중 위 전정부에서 생검이되었고, 요소분해효소 검사, Warthin-Starry 은염색 혹은 Hematoxylin-Eosin 염색으로 조직학적에서 H. pylori의 존재유무를 확인할 수 있었던 107명을 대상으로 위십이지장 조직학적 검사와 IgG 면역블롯팅에 의한 항-H. pylori 항체 보유 유무를 확인하였다. 결 과: 1) 대상 환아 107명 중 남아가 61명(57%), 여아가 46명(43%)이었으며, 연령은 2세부터 15세까지 분포하였고 평균연령은 10.7세로서 10세에서 15세 사이가 가장 많았다. 2) 내시경상 15%에서 위출혈 반점, 위궤양, 십이지장궤양, 십이지장 미란, 출혈성 십이지장염 등이 관찰되었고 대부분은 다양한 정도의 위점막 발적이 관찰되었다. 3) 107명 중 94명(88%)에서 경도 이상의 조직학적 만성위염이 있었으며, 십이지장 조직이 검사 가능하였던 99명 전원에서 만성십이지장염이 있었다. 4) 요소분해효소 검사는 위에서는 45%, 십이지장에서는 25.6%에서 양성으로 판정되었다. H&E 염색 검사에서 38.7%, Warthin-Starry 은염색 검사에서는 40%에서 HPLO 양성이었다. 이 세가지 검사 중 1개 검사에서 양성인 경우인 조직학적 H. pylori 양성은 57%이었다. 5) IgG 면역블롯팅 양성은 96%이었다. 6) 연령군별 조직학적 H. pylori 양성은 0∼4세 군에서는 29%, 5∼9세 군에서는 41%, 10∼15세 군에서는 68%로 연령이 증가할수록 양성률이 증가하였으나, 조직학적 만성위염 및 만성십이지장염 빈도와 면역블롯 양성 빈도는 연령군별 차이가 없이 높은 양성률을 유지하였다. 결 론: 상부 위장관 증세가 있는 소아의 대부분은 조직학적 만성위염 및 만성십이지장염과 동시에 H. pylori에 대한 특이 IgG 항체를 보유하고 있었다. Purpose: This study was undertaken to evaluate the gastroduodenal pathology and Helicobacter pylori infection in children with upper gastrointestinal symptoms. Methods: One hundred and seven pediatric patients with upper gastrointestinal symptoms were undergone endoscopy at the Gyeongsang National University Hospital from June 1990 to April 1991. Histopathologic examination was done by H & E staining of gastric antral biopsy specimen and gastritis was defined according to the Sydney System. Tissue H. pylori status was evaluated with the urease test using Christensen's urea broth and H & E or Warthin-Starry silver staining of gastric antral biopsy specimen. IgG Immunoblotting were also performed to detect specific anti-H. pylori antibody in these patients. Results: The reasons for endoscopy were recurrent abdominal pain, acute abdominal pain, sallow face, hunger pain, and frequent nausea. Variable degrees of gastric mucosal hyperemia were found in most of the patients. Gastric hemorrhagic spots, gastric ulcer, duodenal ulcer, duodenal erosion, and hemorrhagic duodenitis were rare endoscopic findings. Histologic chronic gastritis was found in 88% of 107 patients. Histologic chronic duodenitis was observed in all 99 patients whose tissue were available. Gastric tissue H. pylori was positive in 57% of 107 patients by one of the ureasetest, H & E staining and Warthin-Starry silver staining. However, gastric tissue H. pylori detection rate was lower in the younger age groups. Anti-H. pylori IgG antibodies were detectable in 96% of 107 patients. Conclusion: Chronic gastroduodenitis and anti-H. pylori IgG antibody were ubiquitous in children with upper gastrointestinal symptoms.

        위점막에서의 장형 화생 아형과 위암종과의 관계

        N/A Background/Aims: Gastric carcinoma is the mnost common tumor in Korea and is known to be associated with intestinal metaplasia. By the way, the intestinal metaplasia is classified into subtypes in recent years. This study is to see the relationship between the subtypes of intestinal metaplasia and gastric carcinoma. Methods: This study involved 850 patients with various gastric disorders. After diagnosis with hematoxlin-eosin stain, alcian blue pH 2.5/Periodic acid-Schiff and high iron diamine/alcian blue pH2.5 stains were performed to classify the intestinal metaplasia. Results: The, intestinal metaplasia was found in 465 patients(54.7 %): the prevalence of type I, II, and III intestinal metaplasia were 88.2%, 66.2%, and 22.4%, respectively. There are no difference in the prevalence of type I and II intestinal metaplasia among the various gastric disorders(p 0.05). But, type III was strongly associated with intestinal-type gastric carcinoma as compared to either benign gastric disorders(p(0.01) or diffuse-type gastric carcinoma(p(0.01). Conclusions: This results suggest that the type III intestinal metaplasia may play a special role in the histogenesis of intestinal-type carcinoma. (Korean J Gastroenterol 1997; 30:1 - 8)

        Kimura disease is rare chronic inflammatory disorder of an unknown etiology and it has a predilection for males of Asian descent. It typically presents with non-tender subcutaneous swellings in the head and neck region, peripheral eosinophilia and an elevated immunoglobulin E level. We present here the case of 8-year-old boy with a past history of food allergy, and he developed swelling of the left neck and incidental eosinophilia. Kimura disease was diagnosed on the basis of the clinical and histological findings. The serum IgG titer of cysticercosis was twice the normal level. After surgical excision of the subcutaneous lesion, the patients eosinophilia was dramatically decreased. We suggest that allergy and remote parasite infection may be the causality or a triggering factor for Kimura disease. [Pediatr Allergy Respir Dis(Korea) 2006;16:349-354]

        Leukocytoclastic vasculitis (LV) is a systemic inflammatory disorder involving the small vessels. The exact cause of LV remains unknown, yet malignancy has been considered as a causative factor for LV. We experienced a 60-year-old male with purpura on his extremities. Computed tomography of the chest showed a mass lesion that was suspected to be a non-small-cell lung cancer and the biopsy revealed an adenosquamous carcinoma. There has been no report of LV associated with adenosquamous carcinoma of the lung in Korea. Thus, we report here on this case along with a review of the relevant articles.

        The Peutz-Jeghers syndrome is an autosomal dominant hereditary disease characterized by hamar- tomatous polyps of the gastrointestinal tract and by mucocutaneous melanin deposits. This syndrome is clinically irnportant because of the complication caused by the gastrointestinal polyp, leading to abdominal pain, gastrointestinal bleeding and intussusception. We experienced a case of Peutz-Jeghers syndrome who was complained of dizziness, abdominal pain, melanin pigmentations of the lips, oral mucosa and digits. Pathologic findings on gastrofiherscopic polypectomy showed hamartomatous polyp.

        N/A Objective: Advanced atherosclerotic lesion consists mainly of intimal smooth muscle cell proliferation. Many factors are considered to be implicated in this process. The role of locally produced angiotensin II as a potent growth stimulus had been investigated. To evaluate the effect of ramipril, a new converting enzyme inhibitor, on the smooth muscle proliferative process this animal study was performed, Ramirpil was selected because of its unique physicochemical properties. Methods: Forty nine male New Zealand White or Chinchilla rabbits were randomly assigned to four groups: Group I (N=5), normal control; Group II (N=12), 2% cholesterol diet feeding without vascular injury; Group III (N =16), 2% cholesterol diet feeding after vascular injury; Group IV (N=16), ramipril treated group. Bilateral femoral atherosclerosis was induced by endothelial damage using desiccated nitrogen gas followed by a 4-week high cholesterol diet (2%-cholesterol & 6%-peanut oil). Ramipril, dose of 1 mg/kg/day, was given from 7 days before injury. Results: No definite atherosclerotic lesion was found in group II rabbits, But atherosclerotic lesion with intimal enlargement was induced in all group III & IV rabbits. The lesions were made of abundant cellular components, collagen fibers, & elastic fibers. The presence of intimal smooth muscle cells was confirmed by the immunohistochemical staining with muscle-specific monoclonal antibody HHF-35. Occasionally the foam cell lesions were scattered in the media as well as in the intima. There was frequent disruption of internal elastic lamina. Luminal area, neointimal area, media area, & ratio of intima to media were examined from the histologic sections of the femoral artery. There was no significant difference in all parameters between group I & group II rabbits. But between group I & group III rabbits was there significant difference in all 4 parameters. The same relationship was also found between group II & group III. So it was apparent that the hypercholesterolemic diet for 4 weeks alone could not induce atherosclerotic lesion in this animal model. The luminal area was not significantly different between group III & group IV. (0.10±0.08mm2 vs. 0.11±0.08mm2; p>0.05). The neointimal area of ramipril treated group was significantly smaller than that of group III rabbits (0.56±0.36mm2 vs. 0.17±0.12mm2; p<0.01). The media area was also smaller in ramipril treated group (0.34±0.17 mm2 vs. 0.22±0.06 mm2, p<0.05). The radio of neointima area to media area was higher in group III (1.67±0.80 vs. 0.76±0.44; p<0.01). Conclusion: This rabbit-desiccation injury-cholesterol diet model proved to be useful for the study of atherogenesis. Ramipril was considered to inhibit the intimal smooth muscle cell proliferation in this animal model.
