부정유합된 수지골 경부골절의 수술적 치료

It is generally accepted that malunited phalangeal neck fracture in hands not only limits range of motion but also accelerates the onset of degenerative changes, with increasing pain and stiffness of the affected joint. When displaced or rotated phalangeal neck fracture presents within the first or second weeks, properly performed closed or open reduction with percutaneous pinning or internal fixation is excellent options with predictable results. Malaligned fracture that present later frequently cannot be readily reduced. Once fully united, treatment options have included corrective osteotomy if function is significantly impaired or if appearance is objectionable. We have followed 9 patients, who had operations for malunited phalangeal neck fractures. The average length of follow up was 27 months. The interval between injury and operation ranged from 4 weeks to 6 years, with a mean of 21 months. Sites of operation include thumbs(3 cases), 2nd fingers(1 case), 3rd fingers(2 cases), 4th fingers(2 cases) and 5th finger(1 case). For those cases with less than 8 weeks elapsed since the injury, osteoclasis of the fracture with fixation using K-wire or pull-out was carried out. For those cases with more than 8 weeks elapsed, realignment osteotomy followed by fixation with K- wire or miniscrew was used. Parameters for the evaluation of result include range of motion to within 10 degree of full range in each joint, deviation of the fingers during active maximum flexion and extension, the minimum distance between the tip of the finger pulp and the palm, full bony union, relief of pain and the subjective cosmetic result. Excellent and good results were noticed in 7 cases. The best results can be achieved only with near-anatomic restoration of the joint surface and early active motion exercise. In conclusion, with careful patient selection and close attention to operative detail, operative treatment of malunited phalangeal neck fracture can be effective.

지하철 진동이 주변지반 및 인접구조물에 미치는 영향

최근 지하철 운행의 중대에 따른 지하철 각 부의 진동이 주변지반 및 인접 구조물에 큰 영향을 주고 있다. 본 연구에서는 지하철의 주행으로 인해 발생된 인첩 구조물의 진동을 저감하기 위한 새로운 해석방법을 제안하였다. 즉, X, Y, Z 3 축 방향 성분별로 측정된 각 방향별 기여관계와 각 방향별 진동신호를 하나의 구면파 신호로 합친 진동의 기여관계를 비교 분석하여 효과적인 진동 저감대책의 자료를 제시하였다. 이러한 연구의 결과로 지하철 각 부의 진동이 인근 지역 구조물에 미치는 기여순위는 전동차의 진동(특히, 1~2 Hz), 지하철 박스의 진동, 지반의 진동 순으로 됨을 알 수 있었다. Recently, as subway traffic have been increased, vibration of each parts of subway influence on structures above subway tunnel. This paper presents a new method to reduce which was generated by subway traffic. By comparing directional coherence relationship of X, Y, Z-axis and vibrational coherence relationship of spherical signal which was unified by three directional signals, effective vibration diminishing method was proposed. It can be found that locomotive vibration(specially 1-2Hz) have the worst effect on the vibration of structures and the vibration of subway box ranked 2nd, the vibration of earth ranked 3rd.

새로운 Non-6-fluoroquinolone 모핵의 합성

New ring systems for non-6-fluoroquinolones were designed and synthesized as a part of the program to develop new quinolone antibiotics. Two types of quinolone core ring were successfully synthesized from 3-chloro-4-fluroaniline as a starting material. Methyl and fluorine were introduced at C 5 and C 6 positions of the quinolone ring, respectively, in one system. In the other case, C 7 position was also displaced by nitro group which can be further transformed into amino group. Key steps include Sandmeyer reaction, Friedel-Crafts acylation, directed lithiation, and nucleophilic aromatic substitution.

상완골 과 상부 골절에서의 이중 긴장 대 기법을 이용한 수술적 치료

목 적 : 이중 긴장 대 기법을 이용한 상완골 과 상부 골절의 임상적 결과를 분석하여 그 유용성을 보고하고자 한다. 대상 및 방법 : 1992년 10월부터 2001년 9월까지 상완골 원 위부 골절로 치료한 환자 중 이중 긴장 대 기법을 시행한 14예를 대상으로 하였다. 골절의 원인으로서는 실족에 의한 단순 손상이 11예, 교통사고에 의한 손상이 2예, 직접 타격에 의한 손상이 1예가 있었다. AO/ASIF 분류 방법에 따라 분류하면 A2가 2예, A3이 1예,. C1이 6예, C2가 3예, C3이 2예였으며, 성별로는 남자가 5예, 여자가 9예였고, 평균 나이는 51.6 세 (최소 15세, 최대 88세)였다. 분쇄 골절이 심한 4예에서 주두 절골 술을 시행하였다. 술 후 고정기간은 평균 20.6일 이었고, 경화를 보인 시기는 술 후 평균 10.1주 였다. 65세 이상의 5예에 있어서는 술 전에 골 밀도 검사를 시행하였으며, 대퇴경부의 평균 T-score가 -3.65였고 요추부의 평균 T-score가 -2.97이였다. 평균 추 시 기간은 평균 16.4개월(최소 12개월, 최대 29개월) 이였다. 평가는 최종 외래 내원시 주관절의 운동 범위, 통증 유무, 환자의 만족도를 조사하였다. 결 과 : 최종 추후 결과를 분석하였으며, 운동범위는 Excellent군이 8예, Good군이 4예, Fair군이 2예 였으며, Poor군은 없었다. 10예에서는 술 후 통증이 전혀 없었으며, 4예에서는 가끔씩 통증을 호소하였다. 합병증으로는 1예에서 주두에 K-강선 삽입부위의 피부자극에 의한 통증으로 술 후 6개월째 조기 제거를 시행하였다. 척골신경 전방 전위 술을 8예에서 시행 하였으며, 시행하지 않은 6예 중에 2예에서 척골신경 자극증상이 있었다. 그 중 1예에서는 술 후 8개월째 척골신경 전방 전위 술을 시행 받았으나, 다른 1예에서는 대중적 치료로 증세가 호전되었다. 1예에서 K-강선 삽입부위에 부분적 피부괴사 소견이 보였으나 핀 제거 술 및 창상 치료 후 치유되었다. 결 론 : 이중 긴장 대 기법을 이용한 상완골 과 상부 골절의 치료가 수술이 간단하고 수술 시간을 단축시킬 수 있으며, 정복 시 골막의 박리 손상을 줄일 수 있어 골편의 탈활(devitalization)이 적으며, 골절부의 압축력을 얻을 수 있다. 특히 젊은 환자의 단순 과 상부 골절에서 과도한 술기를 피하고 조기 관절 운동을 하기위한 경우나, 술 전 골 밀도 검사상 골 다공 증을 동반한 분쇄가 심한 노령 환자의 과상부 및 과간 골절의 치료에 있어 이중 긴장 대 기법은 상완골 상부 골절의 치료에 유용한 방법이 될 수 있으리라 사료된다. PURPOSE : This study evaluated the clinical result of supracondylar fractures of the humans treated with double tension band osteosynthesis. MATERIALS AND METHODS : From October 1992 to September 2001, 14 patients with supracondylar fractures of the humerus were treated by method of double tension band osteosynthesis. eleven fractures were the results of slipped down injury and two traffic accidents and one direct trauma. All fractures were classified according to the AO fracture classification (A2; 2. A3; 1. C1; 6, C2; 3. C3; 2). There were 6 males and 9 females with average age of 51.6 age of 51.6 years (range 15 to 88). Olecranon osteotomy was performed in 4 patients with severe comminuted fractures. Postoperative immobiliza-tions were required for an average of 20.6 days and consolidations were noted after an average of 10.1 weeks. Before operation, bone marrow density studies were performed in five fractures above 65 years. Average T-score of the patients were -3.65 on femur neck and -2.97 on lumbar spine. All patients were re-examined after an average of 16.4 months. RESULTS : Union and consolidation was achieved in all 14 patients judged their results as excellent and four as good. Ten patients had no pain and four had only occasional discomfort. Early removal of K-wires was performed in one patient because irritation of one side K-wires were develop. Sensory change of the ulnar nerve was noted in two patients. Anterior transposition of ulnar nerve was performed in one patient and the other was improved spontaneously. Skin necrosis was developed in one patient but was healed after pin removal and conservative care. CONCLUSION : The advantages of double tension band wiring are an easier and faster procedure, less periosteal and muscle damage, and more symmetrical compression than double plating. Therefore, in the commiuted osteoporotic fracture with poor tolerance for internal fixation with the plate or the simple supracondylar fractures in young age, double tension band osteosynthesis can provide sufficient and secure stability to allow early functional exercise.

키랄아민 보조기를 이용한 비대칭 할로락톤화 반응

Seveal chiral amines were prepared and coupled with 4-pentenoic acjd to afford various amides. These amides were halolactonized under several comditions to provideγ-lactone whose %ee was determined to be low. Structural modifications of chiral amines for higher %ee were not successful.

피부 감염환자에서 CRAVIT^�(Levofloxacin)의 임상적 효과

Background : Levofloxacin, an oral fluroquinolone antibacterial agent, is the optical S-(-) isomer of ofloxacin. In vitro it is generally twice as potent as ofloxacin and active against most aerobic gram positive and gram negative organisms, but only moderate activities against anaerobics. Objectives : Our purpose was to investigate the therapeutic efficacy and tolerability of levofloxacin in patients with several kinds of cutaneous infections. Method : Levofloxacin(CRAVIT ; Korea Daiich Parm. Co.) 300-600mg/day for 5-14days was administrated to eighteen patients with cutaneous and subcutaneous infections. The Bacterial culture and laboratory tests were performed prior to and after treatments. Clinical effects and adverse events were evaluated on laboratory indics. Results : 1) Nine of thrteen cases(70%) were useful, four cases(30%) were slightly useful. 2) Clinically twelve of eighteen cases were cured and eight cases were partial response to the drug. 3) There were no laboratory abnormalities after levofloxacin treatment 4) Side effects were not significant except dizziness and sleep disturbance. Conclusion : Levofloxacin was evaluated as a highly effective antibacterial agent against dermatologic infections.

새로운 quinolone 항생제의 개발

New derivatives for non-6-fluoroquinolone antibiotics were designed and synthesized as a part of the program to develop more effective and less toxic quinolone antibiotics. Three types of quinolone core ring were successfully coupled with various cyclic secondary amines to produce nine non-6-fluoroquinolone derivatives. Biological essay is currently under way.

Sport industry is one of the promising industries in the 21st century. The market size of sport industry occupied 2.05% of Gross Domestic Product in 2002. To promote sport industry, it is necessary to educate new theories and practices about sport marketing for potential personnel related to sport industry. The purpose of this study was to develope of educational program for professional sport marketer on the basis of job analysis and survey on undergraduate or graduate who were educated. The educational program for professional sport marketer consist of lecture in classroom(six months), internship(three months), visiting program on foreign countries(three months). Through this educational program, it is possible to secure excellent human resource which is a drive of growth for sport industry.
