경피적 도관 배액술과 경화요법으로 치료한 후복막 낭성 림프관종 1예

후복막 낭성 림프관종은 양성종양으로 매우 드문 질환이다. 수술적 절제가 원칙이나, 경화요법을 우선적으로 고려할 수도 있다. 9세 남자가 병원 방문 4일 전부터점점 심해지는 복통을 주소로 입원하였다. 혈색소 농도가 입원 당일 12.8 g/dL에서, 입원 3병일째 10.6 g/dL로 감소한 소견이 있었다. 복부 전산화 단층촬영에서 크고 종양내부에 격막이 있는 분엽상의 후복막 낭종($10{\times}9.5{\times}5cm$) 한 개가 좌측 신장 주위 공간에서 관찰되었다. 종양 내부에는 출혈이 동반되어 있었으며, 하장간막정맥이 종양에 의해 둘러싸여져 있었다. 수술적 치료의 위험도가 매우 높을 것으로 판단되어 낭종조영술을 시행하였고, 경피적 도관 배액술과 에탄올 경화요법을 시행하였다. 추적 검사로 시행한 복부 전산화 단층촬영에서 종양의 크기가 현저히 감소된 소견을 보였다. 환자는 퇴원 후 13개월 째 현재까지 낭종의 재발없이 건강하게 지내고 있다. 결론적으로, 수술적 치료의 위험이 높은 후복막 낭성 림프관종이 있는 경우 경피적 도관 배액술과 경화요법을 우선적으로 고려해야 할 것이다. Retroperitoneal cystic lymphangiomas are benign, extremely rare tumors. Although surgical resection is the treatment of choice, sclerotherapy should be considered initially. A 9-year-old boy was admitted due do worsening abdominal pain of 4 days duration. Serial complete blood counts revealed a hemoglobin level of 12.8 g/dL on admission to 10.6 g/dL on hospital day 3. An abdominal computed tomography (CT) scan showed a large, lobulated, septated, retroperitoneal cystic mass ($10{\times}9.5{\times}5cm$) in the left anterior pararenal space with intracystic hemorrhage surrounding the inferior mesenteric vein (IMV). Because of the high operative risk, we performed a tubogram of the cystic mass, percutaneous catheter drainage (PCD), and ethanol sclerotherapy. The follow-up abdominal CT scan showed that the cystic mass had decreased in size. He is well without relapse of the retroperitoneal cystic mass for 13 months after discharge. Sclerotherapy with PCD should be considered as initial therapy for patients with retroperitoneal cystic lymphangiomas at high surgical risk.

It is difficult for the steel tubes of large width-thickness ratios to rely on the axial force aristing in the columns in the high-rise steel buildings. In this paper, as the basic step to solve the axial force problems we investigate the maximum column strength of concrete filled steel tubes (square and circular) subjected to concentrically axial load experimentally and analytically. And the results of numerical analysis based on the beam-column theory are discussed to be applied to the multiple column curves of SSRC and ECCS.

This paper aims to report the experimental study on the axial compressive behavior of tubular stub columns filled with variable concrete strength(fcm=500, 700, 1.000kgf/cm²). Thirty-two specimens were tested to investigate the relationship between concrete strength and the ultimate strength of stub column. The effects of inner concrete on the failure mode, the ultimate strength, initial stiffness and deformation capacity were discussed. Also this paper presents experimental results on the strength and deformation of concrete-filled steel stub columns. Because the strength of concrete filled into the hollow tube was limited less than 500kgf/cm² in Korean standard. From this experimental study, it was found that the axial strength of CFT column was not proportioned increased with the compressive strength of concrete. Both circular and square stub column specimens filled with the compressive strength of 1000kgf/cm² concrete belonged to the failure mode 6 having a steeply decline curve after reaching the maximum load. Therefore, this results can make a contribution to the development of design standard.

저자들은 평소 건강하던 9세 남아에게서 Salmonella enteritidis 위장관염에 합병된 급성 비결석성 담낭염을 임상증상, 혈액 검사 및 배양 검사, 복부 초음파 검사등을 통하여 진단 할 수 있었다. 또한 항생제 치료와 비수술적 방법인 경피경간 담낭배액술을 통하여 성공적으로 치료 하였기에 이에 보고하는 바이다. We report a rare case of acute acalculous cholecystitis (ACC) caused by Salmonella enteritidis infection in a previously healthy 9-year-old boy. Salmonella enteritidis was isolated from stool and bile culture. The diagnosis of ACC was established upon clinical, laboratory, and ultrasonographic findings. The patient was successfully treated using percutaneous transhepaticcholecystic drainage (PTCCD) in combination with antibiotics therapy.

The purpose of this study is to investigate mechanical properties of the polymer cement concrete for CFT. Latex dispersion on the basis of styrene butadiene rubber was used as a cement modifier. Investigation of the fresh and hardened modified concrete behavior was made on the basis of testing concretes modified with 10 and 20 percent of polymer admixture on the cement mass.<br/> The setting time of the polymer cement concrete was very retarded with the increase of the dosage of superplasticizers and polymer-cement ratio. The increase of flexural strength for concrete modified with 10 and 20 percent of polymer admixture was 55 and 64 percent, respectively. At the polymer-cement ratios of 10 and 20 percent, the increase of tensile strength was 30 and 47 percent, respectively. And modules of elasticity and shrinkage of the polymer cement concrete was decreased with the increase of polymer-cement ratio. At the maximum stress, strains increased with the polymer-cement ratio and CFT filled polymer cement concrete did not show decreasing behavior after reaching the ultimate strength.

This paper presents an experimental study on the behavior of square hollow section stub columns filled with high strength concrete (fc=l,000㎏/㎠). Several specimen were tested to investigate the effect of the infilled concrete and the thickness to width ratio on the ultimate strength of stub column. From the test results, the effect of inner concrete on the failure mode, the ultimate strength, initial stiffness and deformation capacity was discussed. As a result of this study, we found that the square section members filled with High strength concrete did not show the confinement effect. The purpose of this study is to evaluate the characteristics of CFT stub columns filled with high strength concrete. It would be the basic data for the design standard of Concrete Filled Tubular structures of our country.
콘크리트 强度變化가 CFT短柱의 軸壓縮擧動에 미치는 影響

From this experimental study, we found that the ratio of concrete strength was not proportion to the increasing rate of ultimate strength. This paper presents an experimental study on the strength and deformation of concrete-filled steel stub columns. In Korea, there is not the compressive design standard of CFT columns filled with high strength concrete(500㎏/㎠ and over). Therefore, this results can make a contribution to the development of our own design standard.

In the design of tall buildings, in addition to the requirements of strength, stiffness and stability, the lateral deflections due to wind or seismic loads should be controlled to prevent both structural and nonstructural damage. When the square hollow section members are mainly influenced by local buckling of plate, the load-carrying and the deformation capacity are introduced in this paper. The main parameter of this test is the width-thickness ratio of stub columns. From the test result, the ultimate strength, the deformation capacity and the failure mode of specimen are discussed. The purpose of this study is to estimate the load-carrying and plastic deformation capacity through the experiment of stub columns.
