Concomitant Impact of High-Sensitivity C-Reactive Protein and Renal Dysfunction in Patients with Acute Myocardial Infarction

2014 Yonsei medical journal Vol.55 No.1

Purpose: The present study aimed to investigate the impact of high-sensitivity C-reactive protein (hs-CRP) and renal dysfunction on clinical outcomes in acute myocardial infarction (AMI) patients. Materials and Methods: The study involved a retrospective cohort of 8332 patients admitted with AMI. The participants were divided into 4 groups according to the levels of estimated glomerular filtration rate (eGFR) and hs-CRP: group I, no renal dysfunction (eGFR ≥60 mL·min-1·1.73 m-2) with low hs-CRP (≤2.0 mg/dL); group II, no renal dysfunction with high hs-CRP; group III, renal dysfunction with low hs-CRP; and group IV, renal dysfunction with high hs-CRP. We compared major adverse cardiac events (MACE) over a 1-year follow-up period. Results: The 4 groups demonstrated a graded association with increased MACE rates (group I, 8.8%; group II, 13.8%; group III, 18.6%; group IV, 30.1%; p<0.001). In a Cox proportional hazards model, mortality at 12 months increased in groups II, III, and IV compared with group I [hazard ratio (HR) 2.038, 95% confidence interval (CI) 1.450-2.863, p<0.001; HR 3.003, 95% CI 2.269-3.974, p<0.001; HR 5.087, 95% CI 3.755-6.891, p<0.001]. Conclusion: High hs-CRP, especially in association with renal dysfunction, is related to the occurrence of composite MACE, and indicates poor prognosis in AMI patients.

Vol.15 No.-

This study shows "the consciousness of the old home" that we think it the most important distinction of Lee Ho-Cheol's Literature. Because his subject is so much important in prescribing the original form of creating way and view of the world of Lee Ho-Cheol's Literature. The Korean War in 1950 had made a lot of change to the society and every indivisual of Korea. Refugees from North Korea had experienced the shock of war more seriously than any other person. We're sure that all the people came down to South Korea with lost everything felt so much troubles of life than any other at that time and they were absolutely frustrated since they could not be back to their old home although the Korean War was over. The very point of Lee Ho - Cheol's Literature is showing the way they were. 「Talhwyang」is telling the story all about the uneasy psychology of the four young guys in a strange refuge place. It also shows an aspect of new start to an unfamiliar place. 「New start」here in the story means rebuilding the foundation of life after adapting to the way of South Korea. Each of four young guys in 「Talhwang」 made different choice of an aspect of start, but we can guess the course of "Get settling" of a crowd of people lost their whole old home. 「Talgak」shows the process of "Get settling" itself is. We looked into that "the consciousness of the old home" of Lee Ho-Cheol's refers to the origin floating through the whole his literature by the analysis of work like this. And one thing to be designed we've pointed out is that all the impressions accepting the hurts and pain of division from his works with the theme of division of Korea. We've pointed out that fat-enough customs and too weak ideology overflowing in his works oversimplify so many matters given rise to by the historical affair "Korea War"

Vol.1 No.1

This paper was intended to examine the meaning of the Korean War that Park Wan-suh experienced in her life and her works. The life and literature of Park, Wan-suh are closely associated with the Korean War. Her personal memory of the Korean War that she experienced and ruminated was a passageway for representing her being as a human being to the world and the starting point of her literature. Her life was threatened in the violence of brutal war and she had to endure all kinds of shame for survival. Later, she signified the shame and cruelty that she experienced at that time as “the time or worms”. She had lost her beloved family during the Korean War. And because of consequential economic deprivation, she, as a young woman, had to support her family that survived the war. In her 20s, the beginning of her adulthood, the Korean War that she witnessed as a college fresh woman has destroyed her passion that she had in her adolescence, which made her disillusioned about humans and the world. She felt resentment, so she wrote to alleviate the resentment. Through writing, she summoned the memories of her youth and gave meanings to them. Therefore, the world of her works can be evaluated as showing the absence and lack of the desire against the violence and the dehumanization of women due to war, division and ideological oppression. In other words, the works of Park, Wan-suh, who has undergone the process of the modernization - the times of Japanese Imperialism, liberation, war and division - literally embodies our history. Eventually, the world of her works is a record of a personal memory and a historical record of the sufferings of a Korean woman. Her works is a record of the self-conscious formation (identity) of a person and a record of the identity formation in a modern woman. However, the memory of the wound and the process of healing that Park, Wan-suh put forwards has a very large historical weight that a person can bear. The pain and aftereffects left by the Korean War are still operating and they are the ongoing problem that must be solved. It is because the memories and wound of the Korean War that Park Won-suh constantly recalls in her novels are not the personal wound but the wound of Korean nation.
