Background: N-containing bisphosphonates (BPs), such as pamidronate and risedronate, can inhibit osteoclastic function and reduce osteoclast number by inducing apoptotic cell death in osteoclasts. The aim of this study is to demonstrate the effect of pamidronate, second generation nitrogen-containing BPs and to elucidate matrix metalloproteinases (MMPs) mRNA expression under serum starvation and/or tumor necrosis factor alpha (TNF-α) stimulation on metabolism of intervertebral disc (IVD) cells in vitro. Methods: Firstly, to test the effect of pamidronate on IVD cells in vitro, various concentrations (10-12 , 10-10 , 10-8 , and 10-6 M) of pamidronate were administered to IVD cells. Then DNA and proteoglycan synthesis were measured and messenger RNA (mRNA) expressions of type I collagen, type II collagen, and aggrecan were analyzed. Secondly, to elucidate the expression of MMPs mRNA in human IVD cells under the lower serum status, IVD cells were cultivated in full serum or 1% serum. Thirdly, to elucidate the expression of MMPs mRNA in IVD cells under the stimulation of 1% serum and TNF-α (10 ng/mL) In this study, IVD cells were cultivated in three dimensional alginate bead. Results: Under the lower serum culture, IVD cells in alginate beads showed upregulation of MMP 2, 3, 9, 13 mRNA. The cells in lower serum and TNF-α also demonstrated upregulation of MMP2, 3, 9, and 13 mRNA. The cells with various doses of pamidronate and lower serum and TNF-α were reveled partial down-regulation of MMPs. Conclusions: Pamidronate, Ncontaining second generation BPs, was safe in metabolism of IVD in vitro maintaining chondrogenic phenotype and matrix synthesis, and down-regulated TNF-α induced MMPs expression.

Purpose: Purpose of this study was to analyze the nursing focuses for standardization of ICU nursing records. Methods: The data were collected from 1,000days'nursing records of 197 ICU patients at a tertiary hospital in Seoul. Nursing focuses were unified at the consulting group meeting and they were cross-mapped with the NANDA nursing diagnoses. Results: The 62 nursing focuses in 7 NANDA categories were extracted from nursing record. Among total nursing focuses 41 correspond to the NANDA nursing diagnoses and 21 were added to ICU nursing focuses. The 10 most frequently used nursing focuses are 'Ineffective airway clearance', 'Impaired gas exchange', 'Ineffective tissue perfusion: cardiopulmonary', 'Ineffective breathing pattern', 'Ineffective tissue perfusion: renal', 'Ineffective infant feeding pattern', 'Risk for impaired skin integrity', 'Hyperthermia', 'Impaired skin integrity', 'Decreased cardiac output', Conclusion: Nursing focuses list of ICU was extracted from the result of this study. These nursing focuses might form a framework for development of research-based assessment guideline and care plans for ICU patients through standardization of nursing records.

The purpose of this study is to investigate the vairous change in the toxicity of cadmium by the simultaneous administration of selenium and zinc, which have been reported to change -the toxicity of cadmium through the interaction with cadmium, to rat. For the experiment, 42 rats of Sprague-Dawley strain were used. The experimental groups were divided into 6 groups: a control group, a cadmium (100ppm) alone treatment group, a cadmium (100ppm) and zinc (100ppm) combined treatment group, and three cadmium (100ppm), zinc (100ppm) and selenium (1, 4, and 8ppm) combined treatment groups. The rats were allocated seven to each group and observed for seven weeks. The results of experiment are as follows: 1. The food consumptions of each group were reduced, compared with a control group, especially, in a cadmium and zinc combined treatment group and a cadmium $\cdot$ zinc and selenium(1ppm) combined treatment group to the significant level compared with a control group (p < 0.05). The water consumptions of each group were reduced to the very significant level compared with a control group (p < 0.01). The feed efficiencies of each group were lower than a control group, and among them the highest group was cadmium $\cdot$ zinc and selenium (8ppm) combined treatment group as 90% of a control group. 2. In all groups, the weight gains were highest in the second week and the total weight gains were reduced to the very significant level compared with a control group (p < 0.01). 3. In all groups, the relative weights of liver were reduced, compared with a control group, especially, a cadmium alone treatment group was reduced to the significant level (p < 0.05). The relative weight of kidney was high to the significant level in a cadmium alone treatment group (p < 0.05) compared with a control group. In all groups, the relative weights of testis were reduced, compared with a control group, but the levels were not significant. 4. The accumulation of cadmium was highest in the kidney and the order of height was in liver, testis and blood, respectively. In all groups, the amount of cadmium accumulation was high to the very significant level compared with a control group (p < 0.01). In liver, the amount of acdmium accumulation in. a cadmium alone treatment group was high to the significant level compared with a cadmium $\cdot$ zinc and selenium (8ppm) combined treatment group (p < 0.05), and in kidney, the amount of cadmium accumulation in a cadmium alone tretment group was high to the very significant level compared with the cadmium $\cdot$ zinc and selenium (4, 8ppm) combined treatment groups (p < 0.01). However, in testis, among the treatment groups the level was not significant and in blood, a cadmium alone treatment group was low to the significant level compared with the cadmium $\cdot$ zinc and selenium (4, 8ppm) combined treatment groups (p < 0.05). 5. According to the histopathological finding on the testis, some of the seminiferous tubules of a group treated with cadmium alone showed severe necrosis and atrophy. But the testis of cadmium $\cdot$ zinc and selenium (8ppm) combined treatment group was similar to that of a control group. From the results of this study, it can be concluded that the repeated simultaneous oral administration of large doses of selenium with the cadmium produces the partial amelioration of cadmium toxicity, whereas zinc does not.

Purpose: The present study aimed to identify the physiological characteristics of cells by investigating the change in gene expressionand protein levels during extracellular matrix (ECM) synthesis in the intervertebral disc (IVD) under hypoxic conditions. Materials and Methods: To test the effect of oxygen on cell growth and ECM synthesis of chondrocyte-like cells, the cells from IVDwere separated and cultured in two hypoxia-mimicking systems: chemical hypoxic conditions using deferoxamine (DFO), andphysiological hypoxic conditions using a hypoxic chamber for 7 days. Chondrocyte like cells cultured without DFO and under thenormal oxygen concentration (21% O2 and 5% CO2, 37°C) served as the controls. Results: Chondrocyte-like cells cultured in the presence of 6% oxygen demonstrated a 100% increase in cellular proliferation comparedto the control. The cells treated with chemical hypoxic conditions demonstrated a dose-dependent increase in the mRNAexpression of glucose transporter-1, GAPDH, aggrecan, and type II collagen on Day 1. When treated with 100 μM DFO, the cellsshowed a 50% increase in the levels of proteoglycan protein on Day 7. The cells treated with chemical hypoxic condition demonstratedincrease in sulfated glycosaminoglycan (GAG) protein levels on Day 7. Moreover, the cells cultured in the presence of 6%oxygen showed a 120% increase in sulfated GAG levels on Day 7. Conclusion: The oxygen concentration had an important role in the viability, proliferation, and maturation of chondrocyte-likecells in IVD. In addition, chondrocyte-like cells are sensitive to the concentration of oxygen.
