          제 2형 당뇨병 환자에서 Metformin과 병용 시 Dapagliflozin과 Sitagliptin의 효능 비교

          Objectives: This study aimed to compare effects on glycemic control and weight loss between the metformin/dapagliflozin combination and the metformin/sitagliptin combination in type 2 diabetic patients. Methods: This study retrospectively reviewed the medical records, from January 1st 2015 to March 31st 2016, of type 2 diabetic patients who were older than 18 and were prescribed with dapagliflozin or sitagliptin in combination with metformin. Hemoglobin A1c (HbA1c) levels and weights were measured every 3 months. Results: The dapagliflozin group showed a greater decrease in HbA1c levels after 3 months (-0.75% vs. 0.01%, P<0.001), 6 months (-0.36% vs. 0.08%, P=0.029), and 9 months (-0.53% vs. 0.08%, P=0.046) compared to the sitagliptin group. Also, the dapagliflozin group showed a greater significant decrease in the rate of change in HbA1c levels after 3 months (-0.09 vs. 0.01, P<0.001), 6 months (-0.04 vs. 0.01, P=0.031), 9 months (-0.07 vs. 0.02, P=0.029), and 12 months (- 0.05 vs. 0.05, P=0.047). Furthermore, the dapagliflozin group showed a greater decrease in amount of weight change after 3 months (-2.46 kg vs. 0.37 kg, P<0.001), 6 months (-3.02 kg vs. 0.13 kg, P<0.001), and 9 months (-2.27 kg vs. 0.50 kg, P=0.002). Finally, the dapagliflozin group showed a greater decrease in the rate of change in weight after 3 months (-3.10% vs. 0.52%, P<0.001), 6 months (-3.83% vs. 0.21%, P<0.001), 9 months (-2.84% vs. 0.79%, P=0.002), and 12 months (-4.91% vs. 0.44%, P<0.001). Conclusions: It was concluded that dapagliflozin is more effective than sitagliptin for type 2 diabetic patients.

          은행신용과 산출의 인과관계 분석

          This paper examines whether the bank loan supply has an independent role in explaining the business cycle using the Structural VAR methology. As emphasized by the recent development of the research related to the credit view, 'the risk taking channel in which the deteriorations of the bank's balance sheet constrain the capacity of bank in providing the credit and thus lead to the reduction of the supply of the credit. This implies that the shocs to the supply of the bank credit has a causal effect on the economic fluctuation, while in the context of the RBC framework, the bank credit has no role in the business cycles. The estimation results show that the evidence showed in the estimated impulse responses seems to support the RBC theory, that is, the variation of the aggregate output is followed by the fluctuations of the bank credit. 본 연구는 이 논문은 은행대출공급이 경기변동에 독자적인 역할을 수행하는지의 여부를 구조적 VAR(SVAR)모형을 이용하여 살펴보고 있다. 최근의 논의에서 강조되고 있는 통화정책의 파급경로중의 하나인 은행의 ‘위험추구경로’는 신용을 공급하는 은행신용(대출)의 공급충격이 총수요를 억제하여 경기변동에 부정적인 영향을 미친다는 것을 시사하고 있다. 하지만 실물적 경기변동이론(RBC이론) 등에서는 대출(신용)과 화폐는 실질생산활동에 의해 수동적으로 반응할 뿐이라고 주장한다. 이 논문은 은행의 신용공급행위를 민간의 신용수요행위와 구분함으로써 신용공급충격을 식별하고 있다. 국내경제를 대상으로 추정한 결과, 신용시장의 충격이 산출에 영향을 미치기 보다는 산출이 은행신용의 크기와 대출이자율에 영향을 미치는 것으로 나타나 신용공급충격이 경기변동에 독자적인 역할을 하고 있다는 증거는 미약한 것으로 판단된다.

          동아시아의 지역경제통합체 유형과 무역구조 분석

          오늘날 세계 경제에서 다자주의와 지역주의는 주류적 흐름이다. GATT/WTO에 통보된 지역무역협정은 2017년 10월을 기준으로 455건에 이르며 대단히 빠른 속도로 증가하고 있다. 그리고 경제통합을 통해 국가차원에서의 경제적 이익을 도모하고 협상권 및 정치・외교적 우위를 선점하고자 한다. 이러한 상황에서 세계 경제의 3대 축인 동아시아 지역 내에서도 다양한 형태의 지역경제통합의 시도가 이루어지고 있다. 동아시아 국가들은 장시간에 걸쳐 시장중심적 통합을 이루고 있고 산업내 무역을 바탕으로 높은 수준의 역내교역 및 투자 비중과 생산네트워크체계를 구축하고 있다. 국가 간 산업내 무역의 정도는 경제통합의 효과를 견인하는 역할을 하고 구조조정비용 절감을 가져온다. 높은 산업내 무역 형태를 보이고 있는 동아시아 경제통합모형 중 어떤 모형이 보다 적합한모형인지를 분석하고자 한다. Purpose : There are two main waves which are multilateralism and regionalism on the world economy. Nowadays, 455 Regional Trade Agreements (RTAs) are notified to the GATT/WTO as well as the number of RTAs are increasing more and more speedily. All of countries have attempted to establish FTA in o rder to g et a e conomic benefit, bargaining rights, and t he p oliticaldiplomatic superiority. At this time, East Asia that is called the third pillar of world economy is focused on which pattern of FTA is going to establish as a regional trade agreement. Research design, data, methodology : This paper analyzes the intra-industry trade quotient of main regional economic integration in East Asia and suggest the optimal scenario for East Asia. Results : According to the analysis of this paper, intra-industry trade quotient of ASEAN+3 FTA is much higher than CJK FTA. So the members of ASEAN+3 have higher industry connectivity as well as induce more economic integration effect. Conclusions : countries in East Asia have built a great market-driven integration up such as intra-regional trade, inward-investment, and production network chain that is based on intra-industry trade. A degree of intra-industry trade between countries have something to do with the effect of regional economic integration. Both countries which have a high degree of intra-industry trade can get more economic benefit than do not others. It is related to the reduction of restructuring cost on regional economic integration.

          고등학생의 메르스 관련 건강 소양 수준에 대한 연구

          The purpose of this study is to evaluate the level of MERS (Middle East Respiratory Syndrome)-related health literacy in high school students. For this purpose, a survey questionnaire (11 questions) was developed on basic knowledge, prevention of infection and how to deal with symptoms of infection related to MERS. The survey was conducted on 491 high school students in G-province. The average score of high school students who responded to the questionnaire was 6.6 (60 points on a scale of 100 points), which is includeded in the middle level point in health literacy related to MERS. In the area of basic knowledge related to MERS, the high school students had a very high correct answer rate of 83.8%, but the correct answer ratio of knowledge on the prevention of MERS infection was 57.5% and that of the proper behavior to suspected MERS infection was 32.3%. The 35.8% of high school students who responded were distributed at the upper level in health literacy, 35.6% at the middle level, and 28.5% at the lower level. In addition, female students showed more distribution in the upper level of health literacy and less distribution in the lower level than male students. The important preventive actions against the epidemic of new respiratory diseases are practice of proper hand washing, coughing etiquette, and wearing a properly functioning mask. Therefore, this study suggests that there is a need to emphasize practical health education in the class of science (life science) and health subjects.

          LPS로 유도된 RAW 264.7 cell로부터 NF-κB 조절 억제와 마우스 모델을 통한 큰잎모자반 에탄올 추출물의 항염증 효과

          본 연구에서는 큰잎모자반의 항염증 효과를 알아보기 위해 LPS에 의해 염증반응이 유도된 RAW 264.7 세포에 대한 큰잎모자반 에탄올 추출물의 항염증 효과를 살펴보았다. 세포내 염증매개성 cytokine (IL-6, TNF-α 및 IL-1β) 분비량의 측정결과, 농도 의존적으로 유의적 감소를 보였으며, 특히, 100 μg/ml로 처리하였을 경우 LPS 단독 처리와 비교 시 약 48% 이상의 높은 분비량 감소효과를 보였다. 추출물이 iNOS, COX-2 및 NF-kB 발현 억제에 미치는 효과를 알아본 결과, LPS 단독처리구에 의해 각 단백질의 발현량이 현저히 증가하였으나, 추출물을 처리하였을 때 그 발현량이 농도 의존적으로 감소하는 것을 확인할 수가 있었다. 특히 50 및 100 μg/ml 농도에서 PBS 처리구와 유사한 발현량을 보여 우수한 항염증 효과를 가짐을 확인하였다. 귀 부종 억제 효과 및 조직 관찰을 수행한 결과, 추출물 250 mg/kg 농도에서 prednisolone 10 mg/kg 처리와 유사하게 감소함을 보였다. 또한, croton oil로 부종을 유발한 마우스 귀 조직에서 croton oil만을 처리한 경우에 비해 추출물을 100 mg/ml 농도로 처리한 경우 prednisolone 처리구와 유사한 정도로 경피 및 진피 두께가 얇아진 것을 확인하였으며, 조직 내 mast cell 침윤을 현저히 억제함을 보였다. 이 결과를 종합해 볼 때, 큰잎모자반 에탄올 추출물이 염증 치료제로써의 소재로 이용될 가치가 충분할 것으로 사료된다. The anti-inflammatory effect of Sargassum coreanum ethanolic extract (SCEE) was investigated using lipopolysaccharide (LPS)-induced inflammatory responses in this study. It was shown that there was no cytotoxicity in the viability of macrophages treated with SCEE when compared to the control. The production of NO was considerably suppressed by SCEE, approximately up to 50% at 100 μg/ml. This significantly decreased levels of IL-6, TNF-α, and IL-1β. In addition, the expression of iNOS, COX-2, NF-κB was suppressed by SCEE treatment. In in vivo testing, the croton oil-induced mouse ear edema was attenuated by SCEE and there were no mortalities in mice administered with 5000 mg/kg body weight of SCEE over a 2 week observation period. From these results, SCEE inhibits the release of LPS-induced pro-inflammatory cytokines and mediators, suggesting that SCEE could be a potential agent for anti-inflammatory therapies.

          Salmonella Enteritidis와 Salmonella Gallinarum의 세균막스트레스를 인식하는 spy-gfp 오페론 융합체

          Emergence of drug resistant strains of Salmonella enterica threatens milk processing and related dairy industries, thereby increasing the need for development of new anti-bacterials. Developments of antibacterial drugs are largely aimed to target the bacterial envelope, but screening their efficacy on bacterial envelope is laborious. This study presents a potential biosensor for envelope-specific stress in which a gfp reporter gene fused to spy gene encoding a periplasmic chaperone protein Spy (spheroplast protein y) that can sense envelope stress signals transduced by two major two-component signal transduction systems BaeSR and CpxAR in Salmonella enterica serovars Enteritidis and S. Gallinarum. Using spy-gfp operon fusions in S. Enterititis and S. Gallinarum, we found that spy transcription in both serovars was greatly induced when Salmonella cells were forming the spheroplast and were treated with ethanol or a membrane-disrupting antibiotic polymyxin B. These envelope stress-specific inductions of spy transcription were abrogated in mutant Salmonella lacking either BaeR or CpxR. Results illustrate that induction of Spy expression can be efficiently triggered by two-component signal transduction systems sensing envelope stress conditions, and thereby suggest that monitoring the spy transcription by spy-gfp operon fusions would be helpful to determine if developing antimicrobials can damage envelopes of S. Enteritidis and S. Gallinarum.
