        수동적 어깨뼈 자세 교정 전략과 능동적 어깨뼈 뒤쪽 기울임 전략이 어깨뼈 주변근육 활성도에 미치는 영향

        Purpose: The purpose of this study was to compare the effects of passive scapular upward rotation and posterior tilt and active scapular posterior tilt on the muscle activity of the upper trapezius (UT), lower trapezius (LT), and serratus anterior (SA). Methods: Fifteen healthy subjects performed general arm elevation, arm elevation with passive scapular upward rotation and posterior tilt, and arm elevation with active scapular posterior tilt. For active scapular posterior tilt, the subjects were trained in this movement using visual biofeedback and a motion sensor. During each arm elevation condition, electromyography was used to measure the muscle activity of the UT, LT, and SA. The measured data were analyzed using a one-way repeated ANOVA. Results: LT muscle activity was significantly increased during arm elevation with active scapular posterior tilt compared to both general arm elevation and arm elevation with passive scapular upward rotation and posterior tilt (p < 0.05). SA muscle activity was greater during arm elevation with passive scapular upward rotation and posterior tilt than during general arm elevation (p < 0.05). There was no significant change in UT muscle activity among the tested arm elevation conditions (p > 0.05). Conclusion: Performing arm elevation with active scapular posterior tilt and performing arm elevation with passive scapular upward rotation and posterior tilt may be useful strategies for increasing muscle activation of the LT and SA, respectively.

        Influences of Doorway Stretching with Scapular Posterior Tilt on Glenohumeral Internal Rotation Range of Motion

        Background: Pectoralis minor tightness is associated with reduced glenohumeral internal rotation range of motion (ROM) and scapular anterior tilt alignment. However, the synergistic effects of the pectoralis minor stretching exercise and scapular posterior tilt strategy on glenohumeral internal rotation ROM and pectoralis minor length remain unexplored. Objectives: This study examined the effects of doorway stretching with scapular posterior tilt on pectoralis minor length, scapular alignment, and glenohumeral internal rotation ROM. Design: A case series study. Methods: Fifteen adults with pectoralis minor tightness performed doorway stretching with scapular posterior tilt. Pectoralis minor length, acromion–table distance, and glenohumeral internal rotation ROM were measured before and immediately after stretching. The pectoralis minor length was calculated using the pectoralis minor index. The measured variables were analyzed using a paired t-test. Results: While the pectoralis minor index and glenohumeral internal rotation ROM significantly increased, the acromion–table distance markedly decreased after doorway stretching with scapular posterior tilt (P<.05). Conclusion: Based on the present findings, doorway stretching with scapular posterior tilt could be an effective exercise for increasing glenohumeral internal rotation ROM and pectoralis minor length.

        Effects of Ultrasound Biofeedback on EMG Activity of Trunk Stabilizer Muscles during Lifting Tasks

        The purpose of this study was to examine the effects of abdominal hollowing with ultrasound biofeedback on multifidus and transversus abdominis/internal oblique muscle activities during lifting tasks. This study recruited 12 healthy men. All subjects received training on expiration without and with abdominal hollowing with ultrasound biofeedback. After receiving training for each condition, the thickness of transversus abdominis was measured using ultrasound, and then, electromyography activities of transversus abdominis/internal oblique and multifidus during lifting tasks were measured using a surface electromyography system. The differences in thickness of transversus abdominis and electromyography activities of transversus abdominis/ internal oblique and multifidus between the two conditions were analyzed using paired t-test. Thickness of transversus abdominis and transversus abdominis/internal oblique and multifidus activities were significantly increased during expiration with abdominal hollowing with ultrasound biofeedback compared to during expiration without abdominal hollowing (p < 0.05). These findings suggest that abdominal hollowing with ultrasound biofeedback is effective in increasing trunk stabilizer muscle activities during both static and dynamic tasks such as lifting.

        구순 구개열 환자중 중안면이형성증의 증례보고

        The term median facial dysplasia was first reported by Noordhoff and Cheng to describe a subgroup in the cleft lip and palate patients exhibiting the following characteristic facial defects; lack of Cupid's bow, short prolabium, absence of the labial frenulum and columella, poorly developed premaxilla, absent upper central and lateral incisor of the cleft side, and deficient septal cartilage and nasal spine. But gross brain anomalies were usually absent. From May 1998 to February 2002, four cleft lip and palate patients at the Asan Medical Center were diagnosed as median facial dysplasia. Lip was repaired at 3 months of age and palate was repaired at 1 year of age. All patients showed significant midface disturbances during the follow-up period. We report 4 cases of median facial dysplasia who showed severe midface growth disturbances.

        시상분할 절골술후 강직고정을 이용한 하악전돌증의 수술적 교정시 안정성

        Sagittal split ramus osteotomy(SSRO) has become one of the most popular procedure for correction of mandibular prognathism. Rigid fixation is favored for its stability and patient comfort. But there were few data presented about skeletal stability and factors contributing to relapse for sagittal split ramus osteotomy with rigid fixation. From August 1997 to August 2002, eleven patients, who underwent sagittal split osteotomy with rigid fixation, were studied. Patients with genioplasty or any other orthognatic surgical procedures were excluded from sample. Lateral cephalograms were analyzed before surgery, 1 month after surgery, and 12 months after surgery. The mean amount of surgical setback was 6.29 mm at pogonion and the mean amount of skeletal relapse was 1.29mm at pogonion. The mean postoperative horizontal change of soft tissue pogonion was 5.66mm posteriorly, vertical change of menton was 1.83mm superiorly, and angular change of ramus inclination was 5.88 degree increased. The mean amount of postoperative movement was 1.9mm anteriorly at soft tissue pogonion, 2.13mm superiorly at menton, 0.8 degree was decreases at ramus inclination. The amount of skeletal relapse is related to the amount of setback. The results of this study present that the bilateral sagittal split osteotomy with rigid fixation has many advantages and stable procedure for the correction of mandibular prognathism.

      • Relationship of Scratch Test and Glenohumeral Internal Rotation and Horizontal Adduction Range of Motion

        Background To identify limitation of combined shoulder movements, the scratch test has been widely used in clinical settings. However, no study has examined the relationship between the scratch test and glenohumeral (GH) joint passive range of motion (PROM). Purpose To investigate the relationship between the scratch test and GH internal rotation (IR) and horizontal adduction (HA) PROM. Study design Cross-sectional study. Methods Twenty-four healthy subjects were recruited in this study. Bilateral GH IR and HA PROM were measured using a smartphone application in supine position with 90° of shoulder and elbow flexion. Value of the scratch test was measured using tape measures. Pearson product moment correlations were performed to identify correlation between the scratch test and GH IR and HA PROM. Results Significant correlation between the scratch test and GH IR PROM (p=0.004, r=−0.408 and GH HA PROM (p<0.001, r=−0.620) was revealed. Conclusions These findings suggest that the scratch test could be helpful as well as quick measurement to identify GH joint ROM.
