        Nitroglycerin Patch가 가토피판의 경피산소분압과 피판생존에 미치는 영향

        In order to increase the survival rate of skin flap, many clinical and experimental research have been carried out continuously up to date. Vasodilators have been used to enhance skin flap survial with variable success rate. Nitroglycerin is a clinically available, promptly acting vasodilator for angina pectoris. In this study, recently developed nitroglycerin patch was applied on the rabbit skin. The survival rate and transcutaneous oxygen tension of flaps were measured and compared to those of control group. Transcutaneous oxygen tension, original intention of which is an attempt to determine the arterial oxygen tension by noninvasive method, was measured by transcutaneous oxygen monitoring equipment. The results were as follows; 1. The mean transcutaneous oxygen tension of normal skin in rabbit was 51.05±6.16mmHg. 2. Nitroglycerin patch on skin flap enhanced the mean oxygen tension from 23.8 mmHg (45.9% of normal) to 30.9 mmHg (60.7% of normal) and the mean survival rate of skin flap from 43.7% to 53.8%(p<0.01). 3. We expect that nitroglycerin patch could be used to enhance flap survival rate and to salvage failing skin flap.

        Silcone Tube를 이용한 유리피판의 미세혈관 연결에 관한 실험적 연구

        Recent increasing of trauma and cancer surgery has resulted in the development of reconstruction of tissue defect for rehabilitation in the aspect of restoration of appearance as well as well as function. Skin flaps are now become popular in variable cases of such reconstruction. The use of distant flaps are increasingly needed for the coverage of extensive defect of skin and soft tissue because of the limited amount of coverage offered by the local flaps. The development of free flaps based on microvascular surgery has reduced the numbers of operation, days of hospitalization and total expenses in every case of reconstruction, leaving some problems in operative techniques and complications. In vascular island of free flaps, it is commonly accepted that initial survival of the flap is due to adequate blood flow in its vascular pedicle. During the healing period, the flap can become independent on this pedicle as neovascularization from the surrounding tissue proceeds. This study in therefore designed to investigate the temporary blood supply to the transferred free flaps by silicone tubes connecting between femoral vessels and the effect of temporary blood supply to the flaps on survival of those in rabbits. The survival rate of the flaps was observed in relation to the patency of the silicone tubes. The results as follows: 1. Because of thrombus formation, silicone tubes were gradually obstructed as time elapsed with patency rate of about 70% at third postoperative day and about 50% at the fifth postoperative day. No. difference was noted in patency patency rate between artery and vein. 2. In rabbits of artery-to-artery or vein anastomosis, survival rate of flap was about 90% when the tubes were patent for three days postoperatively.

        Scalping flap을 이용한 비재건 치험례

        As the central and prominent feature of the face, human`s nose has borne great brunt-having been mutilated for punishment; shot, blown off and burnt in battle; bitten,avulsed and sloughed in peacetime; shriveled from within by disease; eroded by cancer; crushed on the road; and punched in the ring. The art of nose-making was born in the backstreets of India centuries before Christ in the hands of the Koomas caste of potters. It was rewakened by the Brancas and Tagliacozzi. At the present time the most popular cover is still the forehead, because of color, texture and vascularity. It is fashioned and transported by various popular methods, among which are gillies “up and down flap,”New`s“sickle flap,”Converse`s “scalping flap”and Kazanjian`s revival of the Indian vertical midline flap. The degree of loss and deformity will usually dictate the type and quantity of tissue required for correction. For the most satisfactory reconstruction the surgeon must consider all of the alternatives to reach the end point with the fewest number of procedures in the most predictable manner and leave the least amount of donor deformity. Authors have experienced patient of subtotal loss of nose due to syphilis and satisfactory result using scalping flap.

        전신 피부이완증 치험 1예

        This is a rare, congenital or/and acquried skin disease with associated other manife-stations, and loose redundant skin mainly showed disorders of elastic fiber in skin. Recently we have experienced a case of 25 years ofd female with skin laxity and fold involved to the face, neck, ear and dorsum of hand. And also mild geneneralzed involvement of other part of skin. Characteristically, the exposed area was more old looking skin than the hidden part of the remaining skin but histologically the abnormal involvement of the skin and internal organs were proved the abnormal histological finding are as follows, elastic fibers are diminished markedly in face skin, its scar, and slightly in abdominal skin, but partially in appendix and some blood vessels. Authors operated on this case twice for only cosmetic purpose such as total rhytidectomy of the face after surgery, the result was satisfactory but still the skin problems remains.

        성형수술 환자들의 정신의학적 측면에 관한 연구(I)

        The psychological problem in patients of plastic and reconstructive surgery is important for the decision of operability. In order to evaluate the psychological aspect of a patient, we analyzed 102 patients who agreed to Co-operate in our study. They were devided into three groups as aesthetic group(GroupⅠn=27), reconstructive group(GroupⅡ n=49) and acute trauma group(GroupⅢ n=26;controls) with Korean standadized MMPI from June,1990 to November,1990 in our hospital. The results of this study are as follows: 1) Un-married females of third decade of life are numerically dominant. 2) The most prevalent operation site is exposed face area. 3) The analysis of profile in clinical scale reveals that most patients of abnormal finding had neurotic tendency and personality problems.

        글루타알데하이드로 처리한 동종연골이식의 실험적 연구

        Reconstruction of the ear is a difficult problem in plastic surgery and there is still no ideal material to reconstruct ear cartilage. Recently glutaraldehyde treatment have been shown to decrease the antigenecity and break-down of graft material. Herein we performed an experimental comparative study of glutaraldehyde-treated homogenous graft, and glutaraldehyde untreated homogenous graft and autogenous graft of ear cartilage in rabbits. Degree of foreign body inflammatory reaction, capsular formation, and weight reduction of ear cartilage grafts were analyzed in each group at 1st week, 4th week, and 6th week. Glutaraldehyde treated homogenous ear cartilage graft showed less foreign body reaction and weight reduction than glutaraldehyde untreated homogenous ear cartilage graft and were well preserved in shape. Glutaraldehyde treated homogenous ear cartilage grafts were comparable to autogenous grafts. We conclude that glutaraldehyde treated homogenous cartilage is a highly promising graft material for the reconstruction of ear. We might expect that glutaraldehyde treated cadeveric cartilage can be used in the reconstruction of ear.

        Improvement of crystal quality and optical property in [11-22] semipolar InGaN/GaN light emitting diodes grown on hemi-spherically patterned SiO2 mask

        We report the improved crystal quality and optical property in (11-22) semipolar InGaN/GaN light emitting diodes (LEDs) grown on hemi-spherically patterned SiO2 mask on m-plane sapphire substrate (HP-SiO2) compared with the m-plane sapphire substrate (m-planar), using metalorganic chemical vapor deposition (MOCVD). The photoluminescence (PL) results showed that the integrated intensity of the near band edge (NBE) emission of the GaN layer grown on HP-SiO2 was increased by 3 times as high as that of m-planar. The full width at half maximums (FWHMs) of X-ray rocking curves for the on- and off-axis planes of the GaN layers on HP-SiO2 were narrower down than on m-planar, which indicates that the crystal quality of the semipolar GaN layers on HP-SiO2 was considerably improved as compared with that on m-planar by reducing defects such as perfect/partial dislocations and basal stacking faults. Cross- sectional transmission electron microscopy (TEM) images also showed the reduction of dislocation density in GaN layers on HP-SiO2 than on m-planar. The optical power of InGaN/GaN LEDs with HP-SiO2 was increased by 1.7 and 7.3 times at injection current of 20 mA and 100 mA, respectively, in comparison with the m-planar LEDs.
