Diabetes Mellitus and the Risk of Fracture

The bone mass is known to be decreased in patients with type 1 diabetes, and this is primarily due to osteoblastic dysfunction. A number of epidemiologic studies have consistently shown a significantly higher risk of fractures in type 1 diabetic patients as compared to that of healthy subjects. The data on bone mineral density (BMD) in patients with type 2 diabetes has been inconsistent. However, many recent epidemiologic studies have shown that the osteoporotic fracture risk, and especially at the hip, is also increased in patients with type 2 diabetes, and even in patients with normal or higher BMD. Meanwhile, several studies have shown higher bone mass and a lower fracture risk in subjects with recent onset type 2 diabetes or impaired glucose tolerance with hyperinsulinemia. Poor bone quality and an increased risk of falling are thought to be important contributing factors for the higher fracture risk in patients with type 2 diabetes. Collagen overglycosylation by accumulation of advanced glycation end products (AGE) is thought to be one of the possible mechanisms underlying poor bone quality. The risk of falling in patients with diabetes is increased due to hypoglycemia, visual impairment from retinopathy and cataracts, altered balance, gait problems due to neuropathy and foot ulcers, associated cardiovascular diseases and medication use. Insulin-sensitizing agents, such as thiazolidinediones, are also associated with a higher fracture risk through an increase in bone marrow adiposity and a decrease in osteoblastogenesis. Because diabetes and osteoporosis are highly prevalent chronic diseases in the aging population and the incidences are increasing worldwide, understanding the risk factors that predispose patients to such conditions is very important. On considering the potential mechanisms associated with bone metabolism, adequate glycemic control while minimizing the episodes of hypoglycemia is the most important first step in the medical management to prevent osteoporotic fractures in patients with diabetes.

N/A Increased platelet aggregation in diabetes mellitus is considered to be one of the etiologic factors of diabetic microangiopathy. The mechanism influencing platelet aggregation by physical exercise in non-insulin-dependent diabetes mellitus (NIDDM) is poorly understood. This study was performed to evaluate the effectiveness of short-term exercise on platelet aggregation in patients with non-obese, non-insulin-dependent diabetes mellitus compared with the normal group. Before and after submaximal exercise using ergometer, platelet aggregation responses to adenosine diphosphate (ADP), collagen, and epinephrine were measured, and the following results were obtained. 1) In platelet aggregation test, in NIDDM patients (n=45), the maximal platelet aggregations [to ADP; 78.7% (p<0.01), to epinephrine; 74.5% (p<0.05)] were significantly higher than those [to ADP; 65.1%, to epinephrine; 67.3%] of control subjects(n=15). 2) In control subjects (n =9), platelet aggregations after exercise was not significantly changed [before exercise; ADP 65.8±3.55%, collagen 70.2±2.35%, epinephrine 58.3±6.08%, after exercise; ADP 68.0±2.84%, collagen 68.9±2.46%, epinephrine 59.5±5.73%, respectively]. 3) In NIDDM patients (n =13), platelet aggregations to ADP and collagen after exercise were significantly decreased [before exercise; ADP 73.4±3.98%, collagen 79.3±4.24%, epinephrine 72.8±3.57%, after exercise; ADP 67.9±3.27% (p<0.05), collagen 69.3±3.49% (p<0.01), epinephrine 66.8±3.97%, respectively]. 4) There were no differences in the plasma thromboxane B2 levels between controls (4.6±1.44 pg/ml) and NIDDM patients (8.5+1.95pg/ml), and after exercise, thromboxane B2 was decreased, but not significantly, in NIDDM patients (before exercise; 18.1±5.36pg/ml, after exercise; 11.5±2.58 pg/ml). 5) Duration of diabetes, hemoglobin Ai, and fasting blood sugar did not affect the platelet aggregation. These results indicate that short-term exercise decreases platelet aggregation in NIDDM patients.

1999년 4월부터 1999년 10월까지 경북지역의 유아 및 초동학생 3,178명(남 : 1,662명, 여 : 1,516명)을 대상으로 말초 혈액을 채취하여 Sysmex K-1000를 이용하여 얻은 성적을 요약하면 다음과 같다. 1. 백혈구수(×10(3)/㎣)는 2-4세가 5.1-12.9(남 : 4.2-12.2, 여 : 5.1-12.9), 5-6세 4.5-12.1(남 : 4.5-12.1, 여 : 4.5-12.1), 7-12세 4.0-11.0(남 : 4.0-11.0, 여 : 4.2-11.0)이었다. 2. 적혈구수(×10(6)/㎣)는 2-4세가 3.81-4.93(남 : 3.79-4.99, 여 : 3.84-4.84), 5-6세 3.86- 4.98(남 : 3.92-4.96, 여 : 3.81-5.01), 7-12세 4.01- 5.09(남 : 4.04-5.12, 여 : 3.98-5.06)이었다. 3. 혈색소(gm/dl)는 2-4세가 10.7-13.9(남 : 10.7-13.9, 여 : 11.0-13.8), 5-6세 10.9-14.2(남 : 10.9-14.1, 여 : 10.9-14.5), 7-12세 11.6-14.6(남 : 11.6-14.5, 여 : 11.4-14.6)이었다. 4. 혈소판수(×10(3)/㎣)는 2-4세가 181-400(남 : 182-430, 여 : 177-412), 5-6세 190-400(남 : 190-400, 여 : 180-400), 7-12세 190-400(남 : 190-400, 여 : 187-400)이었다. 5. 적혈구용적비, 평균혈소판용적, 평균적혈구혈색소량 및 평균적혈구혈색소농도는 7-12세가 각각 34.0-42.0%, 76.0-86.0 fl, 26.0-31.0pg, 32.0-37.0%이었다. To hematologic findings of peripheral blood in healthy infants and children population, we measured the WBC, RBC, Hgb, Hct, MCV, MCH, MCHC and Platelet counts, determined by Sysmex K-1000 in 3,178(M : 1,662, F : 1,516) persons. The results were summarized as follows : 1. The reference values of WBC count(×10(3)/㎣) were 5.1-12.9(M : 4.2-12.2, F : 5.1-12.9) in 2-4 years, 4.5-12.1(M : 4.5-12.1, F : 4.5-12.1) in 5-6 years and 4.0-11.0(M : 4.0-11.0, F : 4.2-11.0) in 7-12 years, respectively. 2. The reference values of RBC count(×10(6)/㎣) were 3.81-4.93(M : 3.79-4.99, F : 3.84-4.84) in 2-4 years, 3.86-4.98(M : 3.92-4.96, F : 3.81-5.01) in 5-6 years and 4.01-5.09(M : 4.04-5.12, F : 3.98-5.06) in 7-12 years, respectively. 3. The reference values of Hgb(gm/dl) were 10.7-13.9(M : 10.7-13.9, F : 11.0-13.8) in 2-4 years, 10.9-14.2(M : 10.9-14.1, F : 10.9-14.5) in 5-6 years and 11.6-14.6(M : 11.6-14.5, F : 11.4-14.6) in 7-12 years, respectively. 4. The reference values of platelet count(×10(3)/㎣) were 181-400(M : 182-430, F : 177-412) in 2-4 years, 190-400(M : 190-400, F : 180-400) in 5-6 years and 190-400(M : 190-400, F : 187-400) in 7-12 years, respectively. 5. The reference values of Hct, MCV, MCH and MCHC in 7-12 years were 34.0-42.0%, 76.0-86.0fl, 26.0-31.0pg and 32.0-37.0%, respectively.

To know the relationship between gastroduodena1 diseases and Helicobacter pylori infection we investigated the prevelence of H. pν10ri infection in 3,248 residents of Kyungpook providence who have participated in a cancer screening program of IgG anti-H. pν10ri antibody test. Among 3,248 residents we compared the patients having gastroduodena1 diseases in X-ray films of upper gastrointestina1 series with the norma1 control group showing norma1 film. H. pν10ri infection was positive in 70.9% of 1,216 control group, and was positive in 87.5% of 1,315 gastroduodena1 disease patients. In gastroduodena1 diseases group, H. Pνlori infection was positive in 87.3% (p va1ue 0.000) of 1,240 gastritis patients, positive in 93.3% (p va1ue 0.056) of 15 gastric ulcer patients, positive in 86.2% (p va1ue 0.072) of 29 duodenitis, and positive in 93.5% (p va1ue 0.006) of 31 duodena1 ulcer patients. H. pylori infection was more frequent in gastroduodena1 disease patients than in the norma1 control group in genera1 population.

N/A Acrylic resins (HSCs : EA/EMA/2-HEMA/CLA) which contain 70% solid content were synthesized by the copolymerization of monomers (2-hydroxyethyl methacrylate, ethyl acrylate, and ethyl methacrylate) and functional monomer (caprolactone acrylate : CLA) which improves the crosslinking density and physical properties of films. The physical properties of the prepared acrylic resins (HSCs) containing CLA, are as follows : viscosity 245~515 cps ; M_(n) 2670~2840 ; and conversions, 83~91%, respectively. From the correlation of T_(g) values, viscosities, and M_(n) of the HSCs, it was found that viscosity and M_(n) increased with T_(g) value.

N/A Acidic degreasing agent(AADA) was prepared by blending sorbitol, Newpol PE-68, Na-dioctyl sulfosuccinate, Tetronix T-701, MJU-100A, n-octanoic acid, and phosphoric acid, The physical properties of AADA tested with aluminum specimen shdhowed the following results ; when 3wt% AADA-5 was performed at 70℃, the degreasing rate was 95% which is comparitively good, and the percentage of etching was 0.277% which was found to be less than that of commercialized product. when 20wt% of AADA-5 was added at 65℃, the percentage of derusting was 91% and the good defoaming effect proved by following low foaming power tests respectively : Ross and Miles, and Ross and Clark methods.
