        The most noticeable aspects of medical environment in the next century will be an increase of the aged population in consequence of prolongation of the average life span and frequent outbreak of chronic degenerative diseases, also called adult diseases, such as hypertension, cancer, and diabetes. Limitations and faults of Occidental medical treatments for them has made world nations give further study to their traditional medicine and open their eyes to the importance of quality of life over treatment. They are also changing medical tendencies; more emphasis on prevention than on treatment, more on health promotion than on prevention. Under these circumstances, Oriental medicine must demonstrate its superiority in treatment, prevention, and health promotion by dint of brand-newly developed evaluation measures to meet the needs of the times without impairing its essential qualities.

      • 敗令散 및 補中治濕湯이 Puromycin Aminonucleoside로 誘發된 白鼠의 腎症에 미치는 影響

        The effects of Paeryungsan and Bojungchiseubtang on rats with nephrosis induced by a single intravenous injection of puromycin aminonucleoside(PAN), 2.5mg/100g of body weight were evaluated in the present study. The effects of Paeryungsan and Bojungchiseubtang on PAN nephrosis were evaluated by measuring ①the concentrations of albumin, total protein, total lipid, cholesterol, triglyceride, creatinine, blood urea nitrogen and uric acid in the serum, ②the amount of volume, protein, glucose and creatinine of the 24hours urine ③the concentration of blood in the urine, intake water, and by observing the changes of microscopic findings of kidney. The results are summerized as follows; 1. In the control group, the concentrations of cholesterol, creatinine, blood urea nitrogen in the serum, the amount of protein and glucose of 24 hours urine, creatinine of 24 hours urine and 24 hours urine volume on the 22th day were significantly increased. On the other hand, the concentration of albumin in the serum, the amount of creatinine of 24 hours urine on the 8th day, the volume of 24 hours urine and the intake water on the 8th, 15th day were decreased significantly. Electron microscopic findings were the segmental obliteration of the foot processes, swelling and villous transformations of the visceral epithelial cells and partial increase of the mesangial matrix. 2. In Paeryungsan group, the decrease of albumin in the serum and intake water were inhibited significantly. But the increase of the concentrations of cholesterol and blood urea nitrogen in the serum were inhibited significantly. And the amount of creatinine of 24 hours urine was increased significantly. There was no significant difference between Paeryungsan group and the control group in electron microscopic findings. 3. In Bojungchiseubtang group, the decrease of albumin in the serum and intake water were inhibited significantly. But the increase of the concentrations of cholesterol, total lipid, creatinine, blood urea nitrogen in the serum and the amount of protein in the 24 hours urine were inhibited significantly. The concentration of protein in the serum was increased significantly. There was no significant difference between Bojungchiseubtang group and the control group in electron microscopic findings. To conclude, PAN nephrosis was induced by a single intravenous injection of PAN 2.5mg/100g of body weight on rats. It can be inferred that Paeryungsan group has the effects of improving hypoalbuminemia in nephrotic syndrome and relieving azotemia when nephrotic syndrome is accompanied by the acute renal failure. It can be inferred that Bojungchiseubtang may improve proteinuria, hypoalbuminenia, hyperlipidemia in nephrotic syndrome. But there was no significant difference between Paeryungsan and the control group, between Bojungchiseubtang group and the control group in electron microscopic findings. So I hope that there will be further studies on the effect on the alterations of glomerular polyanion sites, and on the functioning mechanism of Paeryungsan and Bojungchiseubtang in the future.

      • 스테로이드 依存性 頻回 再發型 腎 病候群의 治療에 있어서 韓方製濟인 柴영(?)湯과 洋方藥物인 스테로이드의 倂用效果에 對한 硏究

        29名의 스테로이드 依存型 頻回再發型 腎 症候群 患者에게 1年間 柴笭蕩과 스테로이드를 倂用 治療하여 본 結果 18名(62%)에서는 治療途中 再發을 하지 않았으며, 8例(28%)에서는 1回 再發하였고, 3例(10%)에서는 3回 以上 再發하여 治療에 反應이 없었다. 使用途中 特記할 만한 副作用은 없었다. 좀더 많은 數를 對象으로 對照群과의 精密追跡硏究가 必要하겠지만 頻回再發型의 腎 症候群 患者에게 柴笭蕩(SA-1)과 스테로이드의 倂用治療는 스테로이드로 因한 重症 副作用을 크게 輕減시킬 수 있는 長點이 있고 腎症候群의 治療를 빠르게 治療할 수 있는 것으로 나타났다. Twenty nine children with frequently relapsing steroid dependent nephrotic syndrome were administered Siryungtang(SA-1) with low dosage steroid for one year. After treatment relapse rate were markedly reduced, there was no relapse in eighteen patients among twenty nine patients(62%), one time relapsed in eight patients(28%) and remaining three patients showed no response(10%). Although further controlled long-term follow up studies are needed, SA-1 has a steroid sparing effect, SA-1 may be a satisfactory substitute for patients with steroid dependent frequently relapsing nephrotic syndrome.

        The effects of Saryungopisan and Jesaengsinkiwhan on rats with nephrosis induced by a single tail-intravenous injection of puromycin aminonucleoside(PAN), 2.5mg/100g of body weight were evaluated in the present study. The effects of Saryungopisan and Jesaengsinkiwhan on PAN nephrosis were evaluated by measuring ① the concentrations of albumin, total protein, total lipid, cholesterol, triglyceride, creatinine, blood urea nitrogen(BUN), uric acid, glutamic oxaloacetic transaminase(GOT), and glutamic pyruvic transaminase(GPT) in the serum, ② the amount of protein, creatinine and volume of the 24 hours urine, the concentrations of glucose and occult blood in the urine, ③ the volume of intake water, and by observing the changes of microscopic findings of glomeruli. The results are summerized as follows : 1. In the Control group as compared to the Normal, the amount of protein of 24 hours urine was significantly increased, the concentration of albumin and total protein were significantly decreased. The concentrations of total lipid, cholesterol, triglyceride, creatinine, BUN, uric acid, GOT, GPT in the serum, and the concentrations of glucose, occult blood in the urine were also increased significantly. Electron microscopic findings were the focal segmental distortion and obliteration of the foot processes. 2. In Saryungopisan group as compared to the Control, the increase of the amount of urinary protein during 24 hours induced by PAN was significantly inhibited, and the concentrations of albumin in the serum was increased significantly. The concentrations of total lipid, cholesterol, triglyceride, creatinine, BUN, uric acid and GPT in the serum were decreased significantly. The volume of the in take water was significantly increased. There was no significant difference between Saryungopisan group and the control group in electron microscopic findings. 3. In Jesaengsinkiwhan group as compared to the Control, the increase of the amount of urinary protein during 24 hours induced by PAN was significantly suppressed, and the concentrations of total protein and albumin in the serum were significantly increased. The concentrations of total lipid, triglyceride, creatinine and uric acid in the serum were significantly inhibited. There was no significant difference between Jesaengsinkiwhan group and the control group in electron microscopic findings. To conclude, PAN nephrosis was induced by a single tail-intravenous injection of PAN 2.5mg/100g of body weight on rats. It can be inferred that Saryungopisan and Jesaengsinkiwhan, both of them have the effects of improving proteinuria, hypoproteinemia, hyperlipidemia in nephrotic syndrome, and relieving azotemia when nephrotic syndrome is accompanied by the acute renal failure. But there was no significant difference between Saryungopisan and control group, between Jesaengsinkiwhan group and the control group in electron microscopic findings. So I hope that there will be further studies on the alterations of glomerular polyanion sites, and on the other functioning mechanism of Saryungopisan and Jesaengsinkiwhan in the future.
