      • A case of nevus lipomatosus superficialis on the palm with fillet flap

        Nevus lipomatosus superficialis is a hamartoma characterized by ectopic mature adipose tissue in the papillary dermis. The condition was first described by Hoffman and Zurhelle in 1921. Two types of nevus lipomatosus superficialis are recognized, namely, the classical multiple type (also known as the Hoffman-Zurhelle type) and the solitary type. A 6-year-old girl presented with skin-colored, 1.7x1.3cm sized, well-defined, dome-shaped nodule on the left palm for several years. Histologic findings showed mature adipocytes between collagen fibers in both the papillary and deep dermis. These findings were consistent with the nevus lipomatosus superficialis. The large lesion was totally excised by fillet flap. At follow-up, the excision region healed with a cosmetically acceptable appearance. Although the solitary type of nevus can develop anywhere on the body, the occurrence on the hand has never been reported. Herein, we report the first case of a child with nevus lipomatosus superficialis on the left palm with fillet flap.

      • [P265] A case of cutaneous protothecosis masquerading as herpetic lesion

        Protothecosis is a rare infection caused byachlorophyllic algae of the genus Prototheca. They aremainly found in the environment, in the soil, freshand salted water, mud of trees, sewage, animal wasteand in some types of food.There are 3 clinical forms: cutaneous,olecranon bursitis and systemic. The cutaneous formis the most common, and most infections are causedby P. wickerhamii.An 82-year-old female patient presented with prickling multiple grouped vesicles and crustson face, neck, chest and back for 1month. There was not a history of contact with soil,contaminated water and any injury of animal bite. Based on prickling pain and multiplegrouped vesicles and crusts, we initially diagnosed as herpetic diseases such as herpes zoster, eczema herpeticum. Histopathological examination revealed an intraepithelial abscess formation, fungal spherules with morula-like appearance and chronic granulomatous inflammation in upper dermis. For species identification, a suspension of theculture was submitted to the carbohydrate assimilationtest, indicating P.wickerhamii as the causative agent.After the diagnosis of cutaneous protothecosis,treatment was started with itraconazole at a dose of 200mg daily,resulting inimproving the lesion. Herein, we report a case of cutaneous protothecosis masquerading as herpetic lesion.

      • Manifestation of axion electrodynamics through magnetic ordering on edges of a topological insulator

        <P><B>Significance</B></P><P>Interactions between two adjacent surfaces of different surface orientations in a single-crystal topological insulator are investigated. We show that the edge between two surfaces can host nontrivial axion electrodynamics with sizeable experimental signals owing to the unique interaction between the two topological surface states. We find that the large work function difference between facets in a topological insulator can generate strong electric fields around the edges and that, in turn, the electric fields give rise to effective magnetic fields for a given broken time-reversal symmetry. Our theoretical work highlights a route to reveal intriguing axion electrodynamics in a real solid and provides methods to exploit macroscopic topological states.</P><P>Because topological surface states of a single-crystal topological insulator can exist on all surfaces with different crystal orientations enclosing the crystal, mutual interactions among those states contiguous to each other through edges can lead to unique phenomena inconceivable in normal insulators. Here we show, based on a first-principles approach, that the difference in the work function between adjacent surfaces with different crystal-face orientations generates a built-in electric field around facet edges of a prototypical topological insulator such as Bi<SUB>2</SUB>Se<SUB>3</SUB>. Owing to the topological magnetoelectric coupling for a given broken time-reversal symmetry in the crystal, the electric field, in turn, forces effective magnetic dipoles to accumulate along the edges, realizing the facet-edge magnetic ordering. We demonstrate that the predicted magnetic ordering is in fact a manifestation of the axion electrodynamics in real solids.</P>

      • Controlling Half-Metallicity of Graphene Nanoribbons by Using a Ferroelectric Polymer

        <P>On the basis of first-principles computational approaches, we present a new method to drive zigzag graphene nanoribons (ZGNRs) into the half-metallic state using a ferroelectric material, poly(vinylidene fluoride) (PVDF). Owing to strong dipole moments of PVDFs, the ground state of the ZGNR becomes half-metallic when a critical coverage of PVDFs is achieved on the ZGNR. Since ferroelectric polymers are physisorbed, the direction of the dipole field in PVDFs can be rotated by relatively small external electric fields, and the switching between half-metallic and insulating states may be achieved.         Hypertrophic lupus erythematosus(LE)was first described by Behcet`s in 1940 as lupus erythematosushypertrophicus et profundus. This entity is characterized clinically by hypertrophic verrucous or vegetative plaques with indurated borders and minimal scaling on extensors of forearms, face, and upper part of trunk. Although treatment includes topical, surgical, and systemic therapy, achieving remission can be extremely difficult. A 49-year-old man presented with an 18-month history of mild pruritic multiple well-defined hypertrophic erythematous scaly papuloplaques on both post-auricles and anterior chest.Histopathologicalexamination showed acanthosis and hyperkeratosisof the epidermis with plugged follicles,liquefactive degeneration of basal cell layer with perivascular and periadnexal lymphocytic infiltration.The clinical and histological findings were consistent with the diagnosis of hypertrophic LE. Hydroxychloroquine and acitretin were firstly tried to treat him, but these were not effective. Later on,he was managed with intralesionaltriamcinolone and topical tacrolimus for 1 month, but these also were not fully effective. As the lesions were not controlled, he was treated with additional cryotherapy for hypertrophic lesions. The lesions resolved successfully within a few months leaving discrete scars. Herein, we report a case of hypertrophic LE treated with cryotherapy.

        Vulvar intraepithelial neoplasia(VIN) is a high-grade intraepithelialsquamous lesion and precursor of invasive squamouscell carcinoma. Theclassification distinguished twotypes of VIN: usual type (human papillomavirus(HPV)-related)and differentiated type (not HPV-related). There is a close relationship with other genital and extragenital premalignant and malignant condition, immunosuppression, and lichen sclerosus.A 44-year-old woman presented with a 1-month history of painful multiple various sized eyrthematous hyperkeratotic maculopapules and plaques with oozing on vulva.On past history, there was a kidney transplantation10 years ago and she had taken antihypertensive and immunosuppressive medications. Histopathological examination revealedbasal layer involvement, enlarged nuclei, hyperchromatic, pleomorphic, andincreased numbers of mitotic figures. The pap smear and ultrasonography were within normal limit. HPV typing was positive with 6 and 11. Topical 5% imiquimod cream was initiated. After 1 week, she refused to use the treatment due to burning sensation.Later on, treatment with cryotherapy was appliedevery 2 week, resulting in the resolution of most lesions. Herein, we report a case of vulvar intraepithelial neoplasia in a patient with kidney transplantation successfully treated with cryotherapy.

        Lupus miliarisdisseminatusfaciei(LMDF)is a rare granulomatous skin disease mainly affecting the central area of the face. The eruption is characterized by small, discrete, red-brown or yellow papules, which occur singly or in crops, may be follicular or nonfollicular, develop rapidly.A variety of treatments are reportedly of some benefits, but controlled studies to establish the best treatment are lacking.A 33-year-old manpresented with a 2-month history of asymptomatic multiple various sized erythematous maculopapules on face. And, a 19-year-old man manifested with a 2-month history of multiple reddish papuloplaques distributed on the central and lateral areas of face, most obviously on the lower eyelids.Histopathological examinations of two cases revealed a large clear-boundary epithelioid cellgranuloma with central necrosis surrounded by lymphocytic infiltration. The chest X-ray and the results of routine laboratory studies were within normal limits. Based onthe clinical and histological findings, adiagnosis of LMDF was made.As oral tetracycline and antimalarials were not fully effective in our cases, oral mini-pulse steroid therapy(Dexamethasone 5mg bid for 2 days weekly) was initiated. After several months, the eruption improved significantly and resulted in the resolution of most lesions.Here, we report two cases of LMDF successfully treated with oral mini-pulse steroid therapy.

      • A case of subcutaneous and intranasal phaeohyphomycosis caused by Microsphaeropsis arundinis

        Microsphaeropsis arundinis is a dematiaceous fungus and capable of causing soft tissue infections known as phaeohyphomycosis, mostly in immunocompromised patients. An 76-year-old man presented with a 5-month history of diffuse erythematous plaques and swelling on both forearms and dorsal hands, and complained of rhinalgia. He was being treated with hypertension, angina pectoris, and diabetes.On the histopathologic examinations of skin, dense infiltrates of inflammatory cells such as neutrophils, monocytes, histiocytes, and giant cells were seen. On methenamine silver stain and Periodic-acid-Schiff stain, scattered fungal spores and hyphae were observed. In the ENT examinations, necrotic changes in the nasal septum were observed and a biopsy was performed at that site. And the histopathologic findings of nasal septum lesions were similar to the results of skin biopsy.The cultures of the biopsy specimen and nasal septum tissue grew a dematiaceous fungus showing irregularly shaped septate hyphae. So, the molecular identification of the organism was performed by gene sequencing analysis. Based on these findings, the authors diagnosed as subcutaneous and intranasal haeohyphomycosis caused by M. arundinis. He was treated with oral itraconazole for over 5 months, and there has been no recurrence to date. To our knowledge, this is the rare case of subcutaneous and intranasal phaeohyphomycosis caused by M. arundinis.

        Effects of Tested Pack Containing Plant Extracts on Elasticity and Size of Women’s Breasts

        본 연구는 콩과식물과 허브식물 및 국내 자생식물체들로부터 estrogen 생성과 대사에 활성을 나타내는 물질을 발굴하기 위하여, 12종의 식물체로부터 식물 유래의 isoflavone류를 포함한 2차대사산물을 용매로 추출하여 지방세포의 지방합성과 축적에 활성을 조사하였다. 활성을 보이는 Q. acutissima, C. lanceolata, P. mirifica, P. bambusoides 및 S. repens 5종의 식물체 용매추출물을 전체중량의 3% 되게 첨가하여 천연 팩을 제조하였으며 임상실험을 통하여 가슴의 탄력과 크기변화에 미치는 영향을 조사하였다. C. lanceolata 추출물을 25 μg/ml 농도로 처리한 곳에서 3T3-L1의 세포성장을 control과 비교했을 때 130% 이상 증가시켰으며, 지방의 축적은 S. repens H2O 추출물을 처리한 곳에서 190% 정도 지방축적량이 많았으며, C. lanceolata와 P. bambusoides EtOH 추출물을 처리한 곳에서도 각각 145%와 154% 활성이 높게 나타났다. 지방합성을 촉진하는 식물체추출물들을 첨가하여 천연 팩을 제조하였으며, 제조한 팩으로 더마프로에 가슴크기 변화와 탄력에 미치는 영향 등의 임상실험을 의뢰하였다. 임상 실험 결과, 가슴둘레, 탄력, 볼륨이 모두 유의적으로 향상되는 것을 확인할 수 있었고. 효능에 관한 설문평가 결과, 사용효과 항목에서 80% 이상의 긍정적으로 답하였으며, 사용 후 2주간 효과에 대해 65%이상이 긍정적인 응답을 하였다. 사용성 설문평가 결과 또한 피험자의 90%이상이 긍정적으로 평가하였다. 피부 안전성 평가에서는 피험자 6명이 사용 중 가려움을 호소하였으나, 그 증상이 미약하여 사용에는 문제가 없었으며, 1명은 사용 3일 후 홍반이 관찰되었으나 5일 후 소멸되었고, 그 외의 다른 이상반응은 관찰되지 않아 인체에 대한 부작용의 문제는 없었다. 이상의 연구 결과, Q. acutissima, C. lanceolata, P. mirifica, P. bambusoides 및 S. repens의 추출물을 혼합하여 첨가한 겔타입의 팩 제조물은 여성의 가슴크기 향상과 탄력유지에 유효하다는 사실을 세포수준의 실험과 임상실험에서 그 효과를 확인 할 수 있었다. In this study, we purified the extracts from the seeds and the roots of various plant species, including Q. acutissima, C. lanceolata, P. mirifica, P. bambusoides, and S. repens, and then investigated the effects of these extracts on cell growth and fat accumulation in adipocytes. We found that the extracts purified from Q. acutissima, C. lanceolata, P. mirifica, P. bambusoides, and S. repens more effectively increased the cell growth, as well as promoting the fat accumulation in adipocytes to a greater extent, than other extracts in vitro. Therefore, we made breast packs containing these effective extracts, and then investigated whether they were effective in enhancing the elasticity and volume of women’s breasts. The measurements of breast elasticity and size revealed that the breast packs efficiently increased the elasticity and size of women’s breasts. Furthermore, evaluation of the questionnaires related to usage of the breast packs indicated great satisfaction in terms of the lift, firmness, and elasticity of breasts. In conclusion, extracts purified from Q. acutissima, C. lanceolata, P. mirifica, P. bambusoides, and S. repens leading to cell growth and fat accumulation in adipocytes can effectively contribute to improving the elasticity and size of women’s breasts.

        치아 과민증에 Hydroxyapatite와 Tricalcium phosphate을 함유한 치약의 치료효과

        치아과민증은 치아의 병적인 증세는 아니지만 치아뿌리가 잇몸밖으로 노출되어 외부의 자극에 대한 민감한 반응으로 환자가 통증을 느끼는 것을 말한다. 칫솔질이나 잇몸질환 등의 다양한 원인이 있으며 치료를 위해서는 자극을 줄여 주는 방법으로 치아과민치료활성이 있는 물질을 이용하거나 레진으로 들어난 치주를 도포하는 방법이 있다. 본 연구에서 수산화인회석과 인산삼칼슘을 혼합하여 만든 치약과 시중에 판매하고 있는 불소함유치약을 4주간 사용하면서 치주과민성 치료효과를 비교 조사하였다. 첨가한 수산화인회석과 인산삼칼슘의 순도는 XRD분석결과 99% 이상으로 나타났으며, 제조한 치약(Hap-TCP)에는 각각 10%와 19%(W/W) 되게 첨가하였다. 치아과민성 치료 효과를 VRS값으로 조사한 결과 사용직후에는 대조군과 차이가 없었으나, 1주후에는 대조군에 비해 8% 통증감소효과가 나타났고, 2주와 4주 후에는 각각 30%와 60%정도 통증감소효과가 나타났다. 그리고 냉자극에 대한 지각과민증을 VAS값으로 조사한 결과도 초기에는 차이가 없었으나, 1주, 2주, 4주 후에 측정한 결과 기간에 따라 유의적으로 감소하였다. 4주후에는 VAS값이 3배이상 차이를 보였다. 이 결과는 현미경관찰에서도 확인이 되어진 것과 같이 수산화인회석이 확장된 상아세관들을 메우고, 구강내의 칼슘, 인 등 이온평형을 인산삼칼슘이 조절하여 메워진 상아세관을 안정적으로 자리를 잡도록 도와주는 재석회화현상으로 치아과민증상을 완화하는 것으로 확인되었다. Although it is not a pathological symptom, Dentinal Hypersensitivity (DH) describes pain felt by patients whose tooth roots are exposed outside of the gums and are therefore sensitive to external stimuli. DH is caused by tooth brushing or gum diseases and treatment to reduce the sensitivity can include use of materials having stimulation activity for DH or a resin material applied periodontally. This study examined the hypersensitivity treatment effects of a four-week treatment with a toothpaste containing hydroxyapatite and tricalcium phosphate (Hap-TCP toothpaste). The Hap-TCP toothpaste was made by mixing a commercially available fluorine-containing toothpaste with 10% (W/W) hydroxyapatite and 19% (W/W) tricalcium phosphate (both 99% purity based on XRD analysis). The tooth hypersensitivity treatment effect was surveyed by scoring VRS values, and showed no significant initial difference compared with the control. However, after 1 week of use, the pain reduction value was 8% in the treatment group compared to the control group. This value increased to 30% and 60% after 2 and 4 weeks, respectively. Hypersensitivity to cold stimulation, which was used as a VAS value, showed no initial significant differences compared with the control, but was significantly decreased after 1, 2, and 4 weeks in the experimental group, with more than a 3-fold difference after 4 weeks. These findings confirmed that remineralization can alleviate DH as hydroxyapatite fills dentinal tubules and calcium, phosphorus, and tricalcium phosphate ion equilibrium is established.
