점액종이란 소타액선 분비관의 폐쇄 또는 파열로 인한 타액의 저류로 나타나는 종창에 대한 임상적인 용어이다. 점액종은 조직학적으로 삼출형과 잔류낭종형으로 분류되어, 삼출형은 외상등으로 소타액선 분비관이 파열된 후 점액이 조직내로 유출되어 조직간격에 고여있는 상태이기 때문에 상피 피복을 볼 수 없고 낭종벽은 육아조직으로 둘러싸여있다. 그리고 잔류낭종형은 소타액선 분비관이 작은 타석에의해 폐쇄되거나 분비관 주위 반흔조직의 협착으로 발생하며 낭종강은 원주상피 또는 위중층 편평상피로 피개되어있다. 본 증례는 3년전 하순의 열창 치료를 위해 봉합된 후 발사되지 않은 silk 봉합사에 의해 발생한 점액종의 치험례로, 발사되지 않은 봉합사가 하순 소타액선의 분비관을 관통하여 파열시켜서 점액이 결체조직으로 유출되어 점액종이 발생하였다. Mucocele is clinical term used to describe swelling caused by the pooling of saliva at the site of a severed or obstructed minor salivary gland duct. Mucoceles are categorized into two subgroups, extravasation type and retention cyst type. The etiology of extravasation type mucocele is related to mechanical trauma to the minor salivary gland excretory duct, resulting in extravasation of mucus into the fibrous connective tissue so that a cystlike cavity is produced, but the epithelial lining is absent. Retention cyst type mucocele results from obstruction of minor salivary gland excretory duct, and cystic cavity is lined by epithelial cells. This case report presents a mucocele occurred on the lower lip, and caused by suture silk unremoved for 3 years. Suture silk penetrated and tore the minor salivary gland duct on the lower lip with subsequent extravasation of mucus into the fibrous connective tissue.

Pap smear were performed to 30,294 women living in the north-west of Chung-nam. The result are summarized below 1. Positive vaginal cytology was 1.09 percent. 2. 72 percents women in their 30's and 40's experienced vaginal cytology. 3. Vaginal cytology was performed at a decreasing rate as age increases. 4. Positive cytology rate is 2.42% higher than average for elderly women over the age of 65. The present date indicates that pap smear is being widely recognized to be important but they needs to be continued advocacy for pap smear.
고콜레스테롤혈증 치료에서 심바스타틴 10mg 과 20mg 사용시의 효능 및 안전성 비교 연구

Background: Elevated serum cholesterol level is a major risk factor for cardiovascular morbidity and mortality. Simvastatin is effective for treating hypercholesterolemia. The aim of the study was to evaluate efficacy and safety of 6-month therapy with simvastatin with relatively low dose, 10 mg and 20 mg/day. Methods: One hundred six patients with hyperlipidemia (triglycerides<400 mg/dL and low- density lipoprotein (LDL) cholesterol>130 mg/dL) were randomized to receive either simvastatin 10 mg/day (n=43) or 20 mg/day (n=63). Efficacy was determined by measuring changes from baseline in lipid parameters including LDL cholesterol, total cholesterol, triglycerides and high-density lipoprotein (HDL) cholesterol. Results: Of the one hundred six patients randomized to treatment, forty patients were men and sixty-six patients were women. Fifty-five percent of patients had hypertension, nine percent coronary artery disease and thirteen percent type 2 diabetes mellitus. Mean baseline lipid concentrations were 258 (total cholesterol), 201 (triglycerides), 50 (HDL) and 167 mg/dL (LDL). Both 10 mg and 20 mg of simvastatin produced statistically significant improvements in all measured serum lipid parameters (p<0.001). Compared with 10 mg of simvastatin, 20 mg of simvastatin produced significantly greater (p<0.001) reductions from baseline LDL cholesterol (34.9 mg/dL vs 20.8 mg/dL). But 10 mg of simvastatin was more effective than 20 mg of simvastatin at reducing triglycerides level (42.7 mg/dL vs 22.3 mg/dL). There was no significant difference in both doses at improving total cholesterol and HDL cholesterol level. Percentage of patients at goal LDL as recommended by NCEP guideline (ATP III) were 81% and 80% for patients in low risk but 35% and 50% for patients in coronary heart disease and its risk equivalents, taking 10 mg and 20 mg/day respectively. Both doses were well tolerated. Only 3 patients (4.8%) in the 20 mg group and one patient (2.3%) in the 10 mg group experienced mild adverse events. Most patients contacted by telephone wanted to take 10 mg of simvastatin. Conclusion: In patients with hypercholesterolemia in Korea, both doses (10 mg, 20 mg) of simvastatin were effective in improving serum lipid parameters and well-tolerated. We recommend, considering patients' preference, that 10 mg of simvastatin be intial dosage and in patients with coronary heart disease, higher doses than 20 mg should be prescribed to allow most patients to reach their NCEP target levels.(Korean J Med 63:46-53, 2002)
A study on the correlation of skin types with genetic factors and environmental factors in Koreans

Background: The Baumann Skin Type (BST) classification provides a new approach for clinical care of dermatologic patients, and it suggests that skin types should be studied as a new area of dermatology. Objectives: To investigate the correlation of skin types with genetic and environmental factors in Koreans comparing and analyzing the correlation between phenotype and genotype. Methods: 774 adults aged 19 years and above who visited the Cosmic Dr.G® online shopping mall were subjected to skin BST questionnaire and gene single-nucleotide polymorphism (SNP) tests. Oral epithelial cells of the subjects were collected using a Genoplan DNA analysis kit, and 14 genes were analyzed by a genetic analysis agency. Results: The most abundant skin type was OSNT (Oily, Sensitive, Non-pigmented, Tight) observed in 205 (26.5%) participants. Of the filaggrin (FLG) SNPs, 143 (18.5%) participants had the GG (good) SNP; 391 (50.5%), GA (normal) SNP; and 240 (31.0%), AA (poor) SNP. Among those whose FLG SNP was GG (good), 121 (84.6%) were sensitive and 22 (15.4%) were resistant. There was also a statistically significant difference in the fruit-vegetable consumption and use of a sunbathing bed between sensitive and resistant type in the groups that classified the FLG SNP as GA (normal). Conclusion: This is the first study to investigate the association of BST with genetic factors and environmental factors in Koreans.
병원의무기록과에서 생성되는 각종 통계와 그전산화율에 대한 조사

정확한 국가보건통계 데이터베이스는 보건정책의 기초가 되지만, 우리나라에서는 아직 신뢰할만한 통계를 생산해내지 못하고 있는 것이 사실이다. 의료보험자료를 기초로 해서 만드는 보건통계가 있지만 여러 면에서 신뢰도가 떨어진다는 의견들이 많다. 이에 저자들은 대안으로서 각 병원의 의무기록실에서 생성하고 있는 통계들이 국가보건통계의 기초가 될 수 있는 지를 알아보기 위해 78개 병원을 대상으로 통계 생성여부와 전산화 정도를 조사하였다. 설문은 병원표준화 심사와 서비스평가, OECD에서 요구하는 통계들을 참고로 하여 작성하였으며, 해당 통계를 생성하고 있는지, 통계생성 작업이 어느정도 전산화되어 있는지를 물었다. 응답한 병원의 50% 이상이 통계를 생성하고 있다고 하였으며, 통계를 생성하고 있는 병원에서 전산망을 이용하지 않고 수작업으로 하고있는 비율은 1.3%에서 7.7% 사이였다. 설문서에서 질문한 통계를 생성하지 않고 있다고 응답한 병원의 대부분은 전산화 되어있지 않았다. 이상의 결과로부터 각 병원의 의무기록실에서는 많은 부분 국가보건 통계에 필요한 각종 통계량들을 생성해 내고 있으며, 통계 생성 방법은 전산화 되어있고, 따라서 이를 수집, 가공하면 국가보건통계 데이터베이스를 구축할 수도 있을 것이라는 결론을 얻었다. 이를 위해서는 통계량을 정의하는 표준화 사업이 선행되어야 할 것이다 We surveyed the generation rate of health statistics by medical records offices of the 78 hospitals and its automation rate using computerized hospital information system. Structured questionnaire was given to one medical record officer of each hospital. Items in the questionnaire was selected from statistics required for hospital service evaluation or OECD health statistics. More than 50% of the medical record office generated questioned health statistics, and most of them was automated. Because many of the medical record offices of the hospitals are producing essential health statistics and automated, the is a possibility that we can collect and use these datas to build up national health database if adequate standardization procedure can be implemented.
Effective dose from direct and indirect digital panoramic units

Purpose: This study aimed to provide comparative measurements of the effective dose from direct and indirect digital panoramic units according to phantoms and exposure parameters. Materials and Methods: Does measurements were carried out using a head phantom representing an average man (175 cm tall, 73.5 kg male) and a limbless whole body phantom representing an average woman (155 cm tall, 50 Kg female). Lithium fluoride thermoluminescent dosimeter (TLD) chips were used for the dosimeter. Two direct and 2 indirect digital panoramic units were evaluated in this study. Effective doses were derived using 2007 International Commission on Radiological Protection (ICRP) recommendations. Results: The effective doses of the 4 digital panoramic units ranged between 8.9 μSv and 37.8 μSv. By using the head phantom, the effective doses from the direct digital panoramic units (37.8 μSv, 27.6 μSv) were higher than those from the indirect units (8.9 μSv, 15.9 μSv). The same panoramic unit showed the difference in effective doses according to the gender of the phantom, numbers and locations of TLDs, and kVp. Conclusion: To reasonably assess the radiation risk from various dental radiographic units, the effective doses should be obtained with the same numbers and locations of TLDs, and with standard hospital exposure. After that, it is necessary to survey effective doses from various dental radiographic units according to the gender with the corresponding phantom.
