      • Activity of Pralsetinib Versus Selpercatinib in Patients with RET Fusionpositive Non-Small Cell Lung Cancer (NSCLC)

        Background Pralsetinib and selpercatinib are anticancer drugs targeting RET fusions (oncogenic drivers) in non-small cell lung cancer (NSCLC). The US FDA approved both drugs in 2020 for the treatment of RET-fusion positive NSCLC. Methods We assessed the efficacy & safety of pralsetinib versus selpercatinib in RET fusion-positive NSCLC in two clinical trials: phase1/2 ARROW trial (n=354) and phase 1/2 LIBRETTO-001 trial (n=144). Results In patients who had previously received platinum-based therapy (n=105), Drilon et al., 2020 showed an overall response rate (ORR) of 64% (95% CI, 54-73) and a median duration of response (mDOR) of 17.5 months (95% CI, 12.0-NE) with selpercatinib in phase 1/2 LIBRETTO-001 trial. Furthermore, a complete response (CR) of 2%, partial response (PR) of 62%, stable disease (SD) of 29%, and median progression-free survival (mPFS) of 16.5 months ( 95% CI, 13.7 - NE) were reported. Treatment naive patients (n=39) demonstrated an ORR of 85% (95% CI, 70-94) and PR of 85%. The responses were determined by the independent review committee. Lee et al., 2019 (n=111) demonstrated a rapid lowering of RET circulating tumor DNA (ctDNA) in 81% of NSCLC patients with pralsetinib therapy. A phase 1/2 ARROW trial (n= 116) exhibited a median follow-up of 8.8 months in patients who received pralsetinib 400 mg daily. Additionally, ORR was 65%, CR 6%, disease control rate 93 (95% CI, 87 - 97), and resulted in overall tumor size reduction in 96% of patients. Refer to (Table 1) for further details. Conclusion Pralsetinib and Selpercatinib have shown promising anti-tumor activity and an acceptable safety profile in RET fusion-positive NSCLC. However, we cannot draw a firm conclusion regarding which one is better due to insufficient data. Clinical trials, including AcceleRET, are still ongoing, and data is pending. Further comparative studies are needed to derive meaningful survival outcomes.

      • Efficacy and Safety of Tepotinib and Capmatinib in the Treatment of Metastatic Non-Small Cell Lung Cancer (NSCLC) Harboring Mesenchymal- Epithelial Transition (MET) Exon 14 Skipping Alterations

        Background The United States Food and Drug Administration (FDA) approved tepotinib and capmatinib for the treatment of mesenchymal-epithelial transition (MET) exon-14 mutated non-small cell lung cancer (NSCLC) in February 2021 and May 2020, but no comparative study has been done so far. Here we compare Results of two-phase 2 trials evaluating the efficacy and safety of these new drugs. Methods Studies demonstrating the role of tepotinib and capmatinib in NSCLC patients were identified using PubMed, Scopus, and Google Scholar databases from inception till February 2021. Results Participants received tepotinib (n=152) and capmatinib (n=97) in two distinct studies. The main efficacy measures in the combined-biopsy group were overall response rate (ORR) of 46% and 56% with tepotinib therapy in VISION trial as per independent review committee and investigator assessment. In contrast, ORR of 41% was noted in previously treated patients vs. 68% in treatment naive patients with capmatinib monotherapy in GEOMETRY mono-1 study. Patients in the tepotinib group showed a median duration of response (mDOR) and a median progression free survival (mPFS) of 11.1 months and 8.5 months, respectively. While with capmatinib study, the mDOR varied from 9.7 months with a mPFS of 5.4 months in those who received prior therapy to mDOR of 12.6 months with a mPFS of 12.4 months in capmatinib monotherapy. The most commonly reported toxicity was grade 3-4 peripheral edema in both arms (tepotinib: 7% vs. Capmatinib: 11%/14%). Conclusions Both drugs exhibited promising activity in the treatment of NSCLC carrying MET exon 14 skipping mutation. However, capmatinib showed an improved ORR, mDOR, and mPFS in treatment naive patients compared to patients treated with tepotinib. Further large and long-term direct comparative clinical trials are warranted to elaborate on beneficial outcomes.

        Fabrication and Characterization of Carbon Nanotubes-Based Pressure Nanosensors: A Study on Piezoresistive Behavior

        This paper investigates the electrical resistivity of piezoresistive CNT/PVAc-based nanocomposites. Different CNTs wt. % containing multi-walled carbon nanotubes (MWCNTs) dispersed in polyvinyl acetate and deposited on a flexible polymer substrate of ethylene glycol methacrylate (PVAc) matrix using conventional methods. The morphological changes were observed using SEM analysis. The resulting composites were subjected to compression by applying different values of pressure (9.3-2348.8 kPa). The results show that for lower wt. % of CNTs, the value of resistance decreased (~0.01526MΩ) with increasing applied pressure, which could be attributed to increasing the conducting paths with compression. However, increasing the concentration of CNTs to a higher value > 1.0 wt. %, results show the opposite behavior, an increase in resistance with an increase in pressure, which could be ascribed to the reorientation, bending, and entanglement of CNTs blocking the conducting paths. The percolation threshold for CNT/PVAc nanocomposite is 0.1 wt. %. This study provides valuable insights into the structural and sensing properties of CNT-based pressure Nanosensors and highlights their potential for use in various applications.

        Failure Analysis of Vital Sign Monitoring System in Digital Healthcare with FTA

        Digital healthcare system requires safety critical software and more attention among human perception and assistance of computer. Appar-ently, it becomes serious challenge for engineers and doctors in assuring the secure and reliable healthcare system. The main target is patient safety here, thus important factor to consider in healthcare is system protection. In recent research vital signs such as heart rate (HR) and breathing rate (BR) are extracted using a non-invasive IR-UWB radar sensor. Hence, safety of the contact-less device is necessary for proper and accurate measurement of HR and BR in digital healthcare system. Therefore, in our research we drawn and performed fault tree analysis (FTA) of such system which analyze potential hazards of measuring subject’s HR and BR in a non-contact fashion. We emphasize on crucial issues and safety factors which are important in patient safety and protection before the proper deployment of such healthcare system.

        Use of wheat straw for effective binding of metal ions via a novel modification

        An innovative biacetyl-modified Triticum aestivum (BMTA) was prepared and its biosorption efficiency was investigated for the removal of Pb(II) and Cr(III) ions from synthesized single and binary metal solutions. The characterization using infrared spectrum, surface area determination and potentiometric titrations indicated an increased acid content of the biomass, indicating BMTA as a potential choice for the binding of metal ions at a relatively low pH. The isoelectric point was found to be at pH 4.5. Various biosorption parameters were optimized. Langmuir model best fitted the experimental equilibrium data with qmax 27.39mg/g for Pb(II) ions and 24.69mg/g for Cr(III) ions in single metal system and 24.21mg/g for Pb(II) and 12.97mg/g for Cr(III) ions in binary system, respectively. The kinetics was described by pseudo-second-order model both in single and binary metal system. The studies indicated relatively selective biosorption of Pb(II) ions by BMTA compared to other metal ions.

        Montmorillonite-Supported BiVO4 nanocomposite: Synthesis, interface characteristics and enhanced photocatalytic activity for Dye-contaminated wastewater

        Textile effluents may harm the human body as well as cause environmental pollution. For several decadesresearchers have been attempting to overcome this issue by introducing environmentally friendly technologiesthat degrade bulk dyes to mitigate hazards. Synthetic dyes are carcinogenic for humans as wellas for other living organisms. Various techniques have been developed for the removal of these toxiccompounds, advanced oxidation processes (AOPs) being the most used processes. In this study,Montmorillonite (MMT) supported BiVO4 nanocomposite was prepared by the sol–gel method to degradeBrilliant Red 80 dye using photocatalysis. The BiVO4/MMT composite was comprehensively characterizedby several characterization techniques including X-ray diffraction (XRD), Fourier-transform infraredspectroscopy (FTIR), Scanning electron microscopy (SEM), Raman, Photoluminescence spectroscopy(PL), and UV–Vis diffuse reflectance spectroscopy (UV–Vis-DRS). Interestingly, the composite materialshowed a narrow bandgap of 2.26 eV with strong light absorption in the visible range. A 1000-wattXenon Lamp was used for activity performance measurement. The photocatalytic Brilliant Red 80 degradationactivity was observed to be 99% degraded within 120 min of illumination compared to conventionalBiVO4 which showed around 80% degradation. Moreover, in this work, an acidic media wasfound to favor the degradation of Brilliant Red 80 dye.
