        Neonatal lupus is a rare syndrome occurring In neonates with tranplacentally acquired maternal autoantibodies, In particular anti-SSA/Ro-SSB/Ra antibodies. Manifestations include congenital heart block, cutaneous lesions, thrombocytopenia, neutropenia, pulmonary or neurological disease and hepatitis. The non-cardiac manifestations resolve by 6 months of age, congenital heart block is irreversible. We experienced a case of neonatal lupus with congenital complete heart block in a newborn of asyptomatic mother with anti-SSA/Ro-SSB/Ra antibodies.

      • 미나리과 산채의 육가공품에 대한 저장성 및 관능특성 : 참당귀·돌미나리·고수 Angelica gigas Nakai, Ostericum sieboldii Nakai and Coriandrem sativum Linnaeus

        Water activity of sausage added Umbelliferaeceae wild plants powder was decreased as storage time was prolonged and all samples were lower than those of the control. During the storage period, pH values of all samples were decreased. Cooking loss and storage loss was decreased as storage time was prolonged and showed differential from added sample. L, a, b values had a tendency to decrease, during the storage period. As the percentage of the wild plants powder in sausage, L, a values were decreased and b value was increased. Total plate counts of bacteria were increased as storage time was prolonged and wild plants powder added sausages were lower than control. Coriander 3.0% added sausage was the lowest total plate counts of bacteria, 1.3×10^6 at four week' storage. As the percentage of wild plants powder in sausage was increased, total plate counts of bacteria decreased. Springiness, cohesiveness, chewiness, gumminess and hardness value of all sample were increased or decreased as storage time was longer. But they did not showed greatest various with storage time as well as additives. The sensory score of wild plants powder 0.5% added sausage were the highest, but wild plants powder 3.0% added sausage were the lowest. All the sensory score of coriander added sausage were high. The sensory score of all sample were decreased as storage time was prolonged. In wild plants powder 3.0% added sausage, the relationship between color, flavor, chewiness, wetness, taste and overall of sensory evaluation and all item of the textural characteristic showed high correlation score. When storaging sausage for two weeks, the correlation of relationship between hardness of sensory evaluation and chewiness of textural characteristic was high.

      • A rare case of extensive papillary muscle calcification in senile cardiac calcification syndrome

        광범위 유두근 석회화는 흔하지 앉으며, 원인과 임상적 중요성에 관한 문헌만 드물게 찾아볼 수 있다. 반면에 작은 석회 침착은 노인에서 흔하며 심장 첨부에 주로 나타난다. 유두근 석회화는 관상동맥질환, 확장성 심근병증, 승모관 질환, 고칼슘혈증, 그리고 말기신부전에서 인산칼슘의 중가와 연관성이 있다. 저자는 심장초옴파와 MDCT를 통해 진단된 전외측 유두근의 광범위 석회화의 드문 사례가 있어 보고한다.

        자생식물과 도입 화훼명의 어원 및 유형 비교

        This study was carried out to survey origins of native plants and introduced flowers. Naming of native plants was divided into two types : one type is based on the internal properties of plants(shape, color, length, thickness, size) and the other type on such properties as person, animal, object, place and time which are compared to shapes and properties of plants. Names of native plants originate from shape > color > length > thickness > size > width > number, and also from characteristics > state > inhaitation place and place of origin > flowering and fruiting season > function and use. Prefixes were added in order to describe shapes and properties of plants in detail. About 50% of the names of the introduced flowers, originates from their shapes, about 15% from persons' names and about 13% from mythologies and legends. This shows that countries have different ways naming flowers from other countries and that survey of origins of flower names helps understand the characteristics of the flowers.

        구강 내 디지털 인상채득을 통한 맞춤형 지대주와 시멘트 합착 후 나사형 임플란트 보철 수복 증례

        최근 디지털 인상채득 시스템이 개발되면서 이를 기존의 CAD/CAM 시스템과 접목한 인레이, 온레이, 크라운이나 간단한 계속가공의치 등의 고정성 보철물의 제작이 활발히 소개되고 있다. 그러나 임플란트 보철에서는 주로 기존의 아날로그식 인상채득 후 이를 스캔하여 제작하는 방법이 사용되어 왔으나, 구강 내 디지털 스캔한 데이터를 이용하여 나사형 보철물을 제작하는 술식은 아직 활발히 이루어지지 않았다. 이에 본 증례보고에서는 6명의 환자를 대상으로 구강 내 스캐너인 iTero를 사용하여 디지털 스캔을 시행한 후 그중 다섯명은 맞춤형 지대주와 시멘트 합착 후 나사형 임플란트 보철물(screw-retained implant prosthesis after cementation, or SCRP)을 제작하였고, 임상적으로 기능과 심미적인 측면으로 만족할 만한 결과를 얻었기에 이를 보고하고자 한다. Fixed dental prostheses such as inlay, onlay, crown, and bridge fabricated by CAD/CAM technique combined with digital impressions is getting popular due to the recent rapid progress of digital impression taking system. For the scope of implant prosthesis, however, digital intra-oral scan hasn’t been actively utilized for the fabrication of superstructures. In this case report, 6 cases of titanium-milled custom abutment based on the iTero intra-oral scan data were introduced, five of them were restored with screw-type prosthesis after cementation (SCRP) and the clinical results were satisfactory on restoring the function and esthetics.

        전자 서베잉을 이용한 서베이드 금관과 국소의치 프레임워크 디자인

        구강 스캐너와 CAD/CAM 기술로 인해 컴퓨터로 제작하는 방식이 활발히 보급된 고정식 보철 증례와는 달리, 의치를 디지털 방식으로 제작하는 데에는 아직 한계가 많다. 촉각을 느낄 수 있는 입력 장비와 전자 서베잉의 개념, 그리고 쾌속 조형법의 적용으로 인해 국소의치를 디지털로 제작할 수 있는 솔루션이 최근 소개되고 있다. 본 증례보고에서는 이러한 국소의치의 디지털 솔루션을 이용하여 총 다섯 명의 환자에서 서베이드 금관 및 국소의치를 제작하였다. 인상 과정상의 문제로 모형과 실제 치아 사이의 형태적 차이가 있었던 경우를 제외하고는 국소의치의 적합도는 우수했으며, 장착 후 안정성과 착탈 시의 유지력 또한 임상에서 적용하는 데에 문제가 없었다. There are still many limitations on fabricating dentures using digital method while computerized production of fixed prostheses utilizing intraoral scanner and CAD/CAM technology has propagated rapidly. Recently the digital solution of fabricating removable partial denture by applying haptic input device, electronic surveying, and rapid prototyping was introduced. In this case presentation, five patients were treated with surveyed crown and removable partial dentures by this digital solution. Fit of dentures was excellent except for one case which showed morphological difference between the actual teeth and that of master cast by the erroneous impression process. There also was not any problem of stability and retention after adaptation in the clinical setting.

        A novel approch of usage of Gram staining to indentify Mycobacterium tuberculosis from tissues

        Tuberculosis is a potentially deadly infectious disease caused by the Mycobacterium tuberculosis (M. tuberculosis). Tuberculosis is diagnosed by proving the M. tuberculosis in sputum samples based on the results of acid-resistant staining, culture, and nucleic acid amplification tests. However, there is a report that the detection rate of M. tuberculosis is low in acid-resistant staining using tissue specimens. It has been suspected that the cause is a potential loss of acid resistance by the organic solvents used for tissue specimen preparation. Therefore, this study was pursued to find out if Gram staining and fluorescent staining in addition to acid-resistant staining would be helpful in diagnosing tuberculosis. We used four tissue (lung, small intestine, large intestine, and lymph node) samples with chronic granulomatous inflammation observed in HE staining and positive results in real-time PCR. These detection rates and staining properties were investigated through microscopic examination using the Ziehl-Neelsen, Gram, and Auramin rhodamine staining. In this studies, M. tuberculosis were observed by Ziehl-Neelsen, Gram, and Auramin rhodamine staining in all four samples. In the evaluation of clinical microbiology proficiency testing (CMPT), the Ziehl-Neelsen and Gram staining were the same result, but the Auramin rhodamine staining was relatively low. These data indicated that Gram staining is useful for detecting M. tuberculosis in formalin-fixed tissue specimens. Therefore, if the Ziehl-Neelsen and Gram staining are combined as the M. tuberculosis staining method in tissue specimens, a better direction may be provided for tuberculosis diagnosis.

        Case Report : A Case of Perimenopausal Endometrial Cancer in a Woman with MSH2 Germline Mutation

        Lynch syndrome is a genetic malignancy syndrome affecting the colon, endometrium, and other organs. It is difficult to find a Lynch syndrome patient without any family history of cancer. We have recently examined an endometrial cancer patient with a MSH2 gene mutation without a family history of cancer. A 55-year old Korean woman was admitted to a local clinic for vaginal bleeding. An endometrial biopsy revealed the presence of adenocarcinoma (endometrioid type, grade 1). After surgical staging, no further adjuvant therapy was required. Analysis of the tissue using immunohistochemistry (IHC) showed the endometrium stained negatively for MSH2. Microsatellite instability (MSI) was analyzed for five markers. The patient was scored as unstable. Further, additional gene sequencing revealed one missense mutation in c.23C > T (p.Thr8Met). This is the first case of Lynch syndrome endometrial cancer in Korea in which the patient does not have any family history of cancer. (J Menopausal Med 2013;19:143- 146)

        Bioactive Materials and Biological Activity in the Extracts of Leaf, Stem Mixture and Root from Angelica gigas Nakai

        지리산 청정지역에서 재배되고 있는 참당귀의 생리활성 물질과 효능을 이용한 건강 기능성 식품 또는 화장품 소재 개발을 위한 기초연구의 일환으로 참당귀의 잎, 줄기 혼합물 및 뿌리의 추출방법에 따른 phenolic compounds, flavonoids, minerals, decursin 및 decursinol angelate 등의 생리활성 물질 분석과 항산화 활성 및 환원력을 측정하였다. 당귀 뿌리의 온수 추출물 수율은 21.89%로 가장 높았다. 페놀성 화합물 및 플라보노이드 함량은 참당귀잎, 줄기 혼합물의 에탄올 추출물에서 가장 높은 함량을 보였고, 참당귀 뿌리 보다는 잎, 줄기 혼합물에서 높았고, 수용성 추출물보다 에탄올 추출물에서 높았다. 미네랄함량은 참당귀 잎, 줄기 혼합물 및 뿌리 모두에서 K, Mg, Fe, Na, Ca, Mn, Zn 순으로 함유되어 있었다. HPLC 분석에 의한 참당귀 잎, 줄기, 뿌리의 decursin 및 decursinol angelate의 혼합 면적 비율은 각각 66.1%, 82.2%, 79.8%로 나타났으며, decursin은 13.0%, 37.6%, 47.1%로 뿌리에서 가장 높았고 decursinol angelate는 53.2%, 44.6%, 32.6%로서 잎에서 가장 높았다. Free radical scavenging 항산화 활성은 참당귀 잎, 줄기 혼합물 및 뿌리 각각의 열수 추출물에서 가장 높았으며, 뿌리보다는 잎, 줄기 혼합물의 모든 추출물에서 항산화 활성이 높았다. Fe 및 Cu 환원력은 참당귀 잎, 줄기 혼합물 및 뿌리의 온수, 열수 및 에탄올 추출물 순으로 높게 나타났으며, 참당귀 잎, 줄기 혼합물의 추출물이 뿌리 추출물 보다 높은 활성을 보였는데 이는 페놀성 화합물이나 플라보노이드 함량과 밀접한 관계를 보였다. Tyrosinase 저해 활성은 참당귀 잎, 줄기혼합물 및 뿌리의 열수 추출물에서 가장 높은 것으로 나타났다. 이상의 실험 결과 참당귀에탄올 추출물에서는 페놀성 화합물과 플라보노이드 함량 및 환원력이 높았으며, 열수 추출물에서는 항산화 효과가 높아 참당귀 유래의 생리활성물질과 그 활성을 이용한 제품 개발에 필요한 기초 자료를 제공해주는데 유용하게 사용될 수 있을 것으로 생각된다. The bioactive materials (phenolic compounds, flavonoids, minerals, decursin and decursinol angelate) and biological activities (DPPH [α,α'-diphenyl-β-picrylhydrazyl] free radical scavenging capability, reducing power, and tyrosinase activity) in the extracts of leaf, stem mixture (AGLS), and root (AGR) from Angelica gigas Nakai were examined by using water, hot water and ethanol solvent. The highest extract yield (21.89%) was found in the water extract of AGR. The highest concentrations of phenolic compounds and flavonoids in the ethanol extracts of AGLS and AGR were 14.99% and 14.79%. Major minerals of AGLS and AGR were K, Mg, Fe, Na and Ca. Decursin and decursinol angelate were the major ingredients of Angelica gigas, detected at 18.71 and 18.89 min of retention time by HPLC analysis, respectively. The highest concentrations of decursin and decursinol angelate in the Angelica gigas ethanol extract were found in root (41.7 ㎍/g ) and leaf (34.04 ㎍/g). The highest free radical scavenging activity was found in the hot water extracts of AGLS and AGR, and its activity was stronger in all extracts of AGLS than AGR. The highest reducing power was found in the ethanol extracts of AGLS and AGR and this was dependent on the sample concentration. The hot water extracts of AGLS and AGR revealed the highest inhibition activity on tyrosinase. Overall, these results may provide the basic data needed to understand the biological activities of bioactive materials derived from Angelica gigas.
