        Postoperative peritoneal adhesion is the most common cause of intestinal obstruction, and intestinal obstruction is one of the most difficult problems in abdominal surgery. In this experimental study for developing peritoneal adhesion model and its prevention, we divided rats into 5 groups; the talc powder treated group, the cotton-fluff treated group, the talc+fluff treated group, the ischemia group and the control group. There were 16 rats in each group. We made incisions on the midabdomen of the rats and administered talc powder, cotton fluff and talc powder+fluff into the peritoneal cavity respectively. In the ischemia group, we clamped the root of superior mesenteric artery for two minutes to induce ischemia on the small bowel of the rat. We counted the number of the peritoneal adhesive sites in peritoneal cavity on the 7th, 14th , 21st and 42nd postoperative day after peritoneal adhesion induced operation, and observed the characters of the adhesion state. The average number of the peritoneal adhesions were 1.25 per one rat in the talc powder group, 1.44 in the fluff group, 1.31 in the talc+fluff group and 1.31 in the ischemia group compared with 0.94 per one rat in the control group performed sham operation. 91.6% of the all adhesions were observed on the peritoneum of the previous incision sites and only 8.4% of the adhesions were adhered to the peritoneal surface of non-incision sites. The data of this study suggest that peritoneal injury and inflammatory reaction is the major cause of peritoneal adhesion. Also this study indicates that foreign bodies can develop peritoneal adhesion, but the effect is much lower than that of peritoneal injury with inflammatoty reaction.

      • 한국인의 각막에 관한 연구

        As the results of measurement of the radius of corneal curvature by keratometry for the right eye of 100 subjects, the steep meridian radius is 7.21㎜ to 8.27㎜ and flat meridian is 7.45㎜ to 8.65㎜, for the left eye the steep meridian radius is 7.30㎜ to 8.35㎜, and flat meridian is 7.45㎜ to 8.72㎜. The mean value of radius of corneal curvature is 7.8㎜ for night eye, 7.8㎜ for left eye. The results of measurement of P-value by topography, it is ranged from 0.45 to 0.91 for the night eye, 0.55 to 0.90 for the left eye, mean value is 0.756 for right eye, 0.735 for left eye respectively. That means the corneal shape is ellipses. The corneal thickness is measured 0.69㎜ 10°degree to the nasal, 0.76㎜ 20°degree to the nasal, 0.66㎜ 10°degree to the temporal, 0.72㎜ 20° degree to the temporal from the visual axis and 0.60㎜ at the central position. The horizontal size of corneal is 11.34㎜ for the night eye, 11.373㎜ for left eye. Those data can be used for contact lens design and fitting.

      • Rat의 복강내에 투여한 5-fluorouracil-polyglycolic acid 제형이 장기에 미치는 영향에 관한 연구

        For more effective intraperitoneal chemotherapy, the authers made 5-fluorouracil-polyglycolic acid composite disks(FU-PGA disks) with 5-fluorouracil and polyglycolic acid, a biodegradable polymer. We inserted the FU-PGA disk into the peritoneal cavites of Sprague-Dawley male rats, the control and three experimental groups; one FU-PGA disk insert group(300mg/kg), two FU-PGA disk insert group(600mg/kg), and three FU-PGA disk insert group(900mg/kg). The control group received a similar number of PGA disk inserts. A pharmacokinetic study was performed to measure the 5-fluorouracil concentration in the peritoneal fluid, blood, and tissues(liver, kidney and heart) at 24 hours, 72hours and 168 hours after insertion of the FU-PGA disk. Light and electron microscopic studies were done. The results were as follows: 1) The number of white blood cells and platelets decreased after FU-PGA insertion. The degree was proportional to the duration and amounts inserted. The change in the number of red blood cells varied slightly. 2) Light microscopically, slight changes were noted at 168 hours in the 3 disk insert group (900mg/kg). Mild fatty change and hepatocyte degeneration around the central veins of the liver were noted, with vacuolar degeneration in the proximal convoluted tubules of the kidney. 3) Electron microscopically,the liver showed focal increases of lipid droplets in the hepatocytes, and irregular nuclear membrane with focal nucleolar segregation of the fibrillar and granular elements. Also the double membranous structure of the hepatocytes and Kupffer cells was widened with bleb formation. The kidney showed wide separation of the nuclear double membrane in the proximal convoluted tubular cells and mesangial cells, with vacuolation and myelin figure formation in the mitochondria of the proximal convoluted tubular cells, at 72 hours and 168 hours in the 2 to 3 FU-PGA insert groups. The heart showed focal loss of cristae,vacuolation and myelin figure formation in the mitochondria of the 2 to 3 FU-PGA insert groups. In conclusion, despite the large amont of FU-PGA inserted, the histopathological changes in the liver, kidney and heart were slight, and consonant with the very low amount of 5-fluorouracil concentrations detected in the liver, kidney and heart. The above results suggest that the FU-PGA composite can serve as a new device for releasing drugs in a controlled manner and easily targeted to intraperitoneal organs. This device can improve the efficacy of intraperitoneal chemotherapy for gastric cancer.

      • 過量의 鹽分攝取가 白鼠腎臟 動脈에 미치는 影響에 對한 病理學的 硏究

        There are considerable data which indicate that the high salt intake is important or necessary to the development of hypertension in man and animals, and the low-salt diet is effective lowering the blood pressure. This study is made to evaluate the morphologic changes of small arteries on high-salt diet and salt-restrction in rats. Male albino rats were fed different amount of salt (0.06;%, 0.4%, 4. 0%, 8.0% of NaCI) for 2-20 weeks and changes of arterial calibers of different sized arteries are studied. The results were as follows: 1. In salt-fed groups, the arterial walls reveal medial thickening due to hyperplasia of smoo muscle cells, especially in medium-(4.0%) and high-salt(8.0%) groups. 2. There is no significant differences of external-to-luminal diameter(ED/LD among three rena arterial types with each experimental group(p>0.1). 3. In salt-fed groups, increment of ED/LD ratio in comparison to the control group is significant in all arterial types of medium-and high-salt groups (p<0.01), but insignificant in low-salt(0.4%) group (p>0.05). 4. Restriction of salt intake after 10 weeks salt feeding did not reverse altered ED/LD ratio in all arterial types (p>0.05). In summary, the results obtained by present study indicate that intake of a large amount of salts bring about hyperplastic thickening of the wall of renal arterial system with comparative narrowing of the lumen, and that these changes persist even after the restrictction of salt intake.

      • Lead nitrate 投與가 肝細胞增殖에 미치는 影響에 關한 形態學的 硏究

        To evaluate the effects of lead nitrate on the hepatic cell proliferation and its mechanism, the one group of male Sprague-Dawley rats were treated with a single dose of lead nitrate (100umol/ kg of body weight) intravenously and the another group were pretreated with phenobarbital (100 mg/kg) intramuscularly once a day for 4 days before lead nitrate treatment for the purpose of research the action mechanism of lead nitrate on liver cell proliferation. Then the relative liver weight, the mitotic activity and the relative amounts of hepatic protein and DNA of each group were compared with those of control group. The results were as follows; 1. The relative liver weight began to increase at 24 hours in the lead nitrate treatment group significantly (p<,0.01), reached the peak at 72 hours, and then decreased after 96 hours. 2. The mitotic activities increased at 48 hours in both experimental groups significantly compared with the control group, reached the peak at 72 hours, and diminished after 96 hours. There was no statistical difference between lead nitrate treated group and phenobarbital pretreated group. 3. The relative amount of hepatic protein showed increment from 24 hours significantly, reached the maximum at 72 hours and then diminished after 96 hours. The relative amount of hepatic DNA in the lead nitrate treated group revealed the maximum value at 96 hours and the phenobarbital pretreated group disclosed the maximal increment at 72 hours. The relative amounts of protein and DNA were more increased in the phenobarbital pretreatment group than those of lead nitrate treatment group.

      • 평행봉 몸 굽혀 2회전 뒤 공중 돌아 내리기 동작의 국면별 운동학적 분석

        This thesis has an aim to examine the influence on landing of successful salto by the difference of athletic factors of salto backward tucked dismount on the parallel bars. This thesis analyzed six national representative players dividing them into the skill group (G1) who got more than 9.5 point and the unskilled group (G2) who got less than 9.5 point based on the average point by four international referees utilizing three dimensional image analyses photographed with video camera, and it got the conclusion of the athletic factors dividing into each aspect as followings. 1. As for the time spent by the groups when the thesis divided them into each aspect, G1 spent more time than G2. It's because G1 acted in stability at the moment of staying in the space and landing as it spent much time in the space with fast down swing and release, but G2 on the contrary, acted fast at the moment of space of four aspects with slow down swing and release. And its landing was unstable at the movement of low height. 2. As for the positional change in each aspect, the parallel position of G2 increased than that of G1, and the increased parallel position made the unstable salto and landing . As for the change of vertical position, at the time of down swing in the first aspect, what G1 acted at the low position of the second aspect at the high position seemed to be an act to release fast falling the high joint down, and it made salto high and made landing stable. 3. As for speed change of high joint in each aspect, G1 acted with fast horizontal speed generally at the time of release, aspect of space, and aspect of landing because of right horizontal speed at the time of down swing, and it seemed to be an act to get rotary power of double salto. As for the vertical speed, G1 acted with a much more vertical speed than G2 in four aspects with a big up-down movement by the fast vertical speed at the time of down swing and release. It means that G1 acted fast with a fast vertical speed of the high joint to get the height to stay in the air. 4. As for the change of the angle of the high joint in each aspect, G1 acted a bigger down swing and release than G2 utilizing the stretched body of the high joint at the time of down swing and release. As for change of angle of shoulder, G1 acted down swing with the shoulder joint accessed nearer than G2 to the body. At the time of down swing, it acted with a big angle of shoulder joint to stretch high shoulder at the time of swing. In addition to it, at the time of release, G2 escaped from the hand more slowly than G1 because of big angle. It made the movement of space and landing movement unstable. 5. As for change of angular speed in each aspect, G1 acted with a fast high joint down swing from the start to the first aspect. It acted with a slow speed of the angle stretching the high joint in the second and the third aspect so that it could get fast angular speed in the forth and the fifth aspect. As for the change of shoulder joint, G1 acted with a fast angular speed in the aspect of space and that of landing movement by the fast speed of shoulder joint at the time of release. In addition to it, fast angular speed of the shoulder joint in the release aspect of space made the rotary power of double salto.

        공공서비스의 민간 위탁 성과에 영향을 미치는 요인

        이 연구에서는 민간 위탁에 관한 이론적 배경과 경험적 사례를 통해서 공공서비스의 민간 위탁 성과에 영향을 미치는 여덟 개 요인들을 추출하여 유의한 영향을 미치는 변수가 무엇인가를 밝혀내고 이를 바탕으로 어떠한 여건 하에서 어느 요인들이 민간위탁의 성과에 유의한 영향을 미치는가를 알아보기 위해 군집분석 결과 제공된 자료들을 가지고 회귀분석을 실시하였다. 그 결과, 민간 위탁의 성공에 영향을 미치고 있는 유의한 요인들로는 전체적으로 볼 때 저항성, 측정가능성, 비용효율성, 서비스 중단시 파급효과라는 네 가지 조건들임을 찾아냈으며, 이 가운데 가장 많은 영향을 미치는 요인은 비용효율성이고, 그 다음이 저항성, 측정가능성, 서비스 중단시 파급효과의 순서이다. 한편, 이들 요인 이외의 경쟁성, 전문 기술적 복잡성, 기관장의 추진 의지, 행정지원체계의 적절성 요건 등은 공공서비스의 유형에 따라 민간 위탁 성과에 영향을 미치고 있음을 밝혀냈다. 이러한 분석 결과를 논의하면서 제시한 행정적 시사점을 요약하면 다음과 같다. 첫째, 민간 위탁 여건의 정확한 파악 없이 같은 종류의 공공서비스라는 이유로 민간 위탁을 무리하게 추진하는 것은 현실적합성이 결여될 수 있다. 따라서, 민간 위탁 여건에 따른 공공서비스의 분류 유형에 따라 민간 위탁의 성과에 영향을 미치는 요인들이 달라지는 까닭에 민간 위탁 이전에 여기에서 제시한 여덟 가지 민간 위탁 조건들을 점검함으로써 어느 유형에 속하는지를 판단하고 난 후 민간 위탁 여부를 결정할 것을 제안한다. 둘째, 기술적 전문성이 요청되는 공공서비스의 경우, 민간부문에 위탁하는 것이 좀더 적은 비용으로 양질의 공공서비스를 공급할 수 있다는 것을 시사한다. 셋째, 경쟁성이 낮다고 판단되는 공공서비스의 경우 민간 위탁을 통해 비용 절감을 추구하기 위해서는 적절한 성과 평가지표의 개발이 선행되어야 할 것을 주장한다. 넷째, 서비스 공급 중단으로 인한 사회적 파급효과가 큰 공공서비스의 경우, 신뢰할 만한 책임감 있는 수탁자를 경쟁적으로 선정한 후 적절히 지도, 감독하는 일이 매우 중요하다. 왜냐하면, 서비스 공급이 중단되는 경우, 서비스 공급을 다시 시작하려면 많은 시간과 노력 비용이 소요되고 여론이 악화되기 때문이다. 이 이외에도 위탁 조건 중 저항성은 민간 위탁의 성과에 가장 부정적인 영향을 미치는 요인으로 밝혀졌다. 따라서 조직내외의 저항성이 강하게 나타나는 공공서비스는 민간 위탁에 신중을 기할 필요가 있다. 곧 민간위탁을 무리하게 추진하기보다는 저항을 완화, 극복할 수 있는 제도적 방안을 마련한 후, 경험적 학습화를 거쳐 점진적으로 민간에 넘겨주는 방안을 적용해야 한다. constituents. The OECD countries have been practicing a series of government reform programs in order to cope with the pressures of the environmental change. These programs are basically aimed at enhancing public sector productivity and customer satisfaction through the adoption of result-oriented management system. The development of accounting system is an indispensable step toward a successful result-oriented management in government. Especially the cash-based governmental accounting system makes it hardly provide accurate information on the status and soundness of governmental financial management. Thus, many OECD countries are transforming the accounting system from cash accounting to accrual accounting. This study explores the causes, consequences, performance, and limitations of Australian governmental accounting system reform for the successful adoption of the new accounting system in Korea. This study shows the successful Australian experience with its decentralization and strengthened accountability of the public bureaucracy, and the new system provides valuable information for better management decisions. Continuous efforts based on active participation of private and public sector, an official's will and capability to apply new information can play an important role in the system. But to reform the governmental accounting system successfully, it needs to clarify the limitation of financial information that new system products. And finally, under this circumstances, this study implies that supplemental measures like budget system reform and performance management system of measuring policy effects should be accompanied. This paper investigates the factors which have influenced on the local public services contracting-out outcomes. This study aims to provide the theoretical grounds of the factors improving local public services. Based not only on the theoretical backgrounds and empirical case study, but also on the interviewing results with public servants who deal with contracting-out, eight criteria are selected. With this conditions cluster analysis is done for the public services and the groups newly classified are discussed in terms of their characteristics. The variables influencing these newly classified five groups of the services are examined by multiple regression analysis. The research findings suggest as follows: The success in contracting-out is highly dependent on the higher cost-efficiency, on the lower resistance, on the higher measurability of outcomes, and on the higher spill-over effect in discontinued services when the services are stopped to be supplied with. Alongside introducing "research councils" in public R&D governance system, the Korean government has carried out the evaluation of member research institutes under research councils every year since 2000. However, the current evaluation system strongly needs to be improved, mainly due to both some deficits of system itself and unfavourable effects arose from the actual implementation process. Therefore, this paper aims to suggest some policy implications for the development of the Korean institute evaluation system, taking the Institute Assesment Exercise (IAE) of the Biotechnology and Biological Sciences Research Council (BBSRC) in the UK as a bench-marking model. As a result, the IAE practice of the BBSRC will be a good reference when three Korean research councils in S&T sector intend to develop their current institute evaluation system. In particular, the UK case will provide useful policy implications for the improvement of some limitations of the Korean system, including the focus of managerial performance, one year interval between evaluation exercises, lower expertise in both evaluation manpower and evaluation unit, the adoption of ranking system among the institutes and budget adjustment-focused utilisation, which have been regarded as main problems in the Korean case.

      • 백서의 복강내에 투여한 Mitomycin-C-Polyglycolic acid 제형이 장기에 미치는 영향에 관한 연구

        To evaluate the in-vivo effects of mitomycin-C-polyglycolic acid composite(MMC-PGA) the disk: shaped composite were implanted into the peritoneal cavity of the Sprague-Dawley rats. The pathologic changes were examined at various time points up to 12 weeks. Initially the soft tissue around the inserted disks showed capillary congestion at 3 days. Inflammatory infiltrates with foreign body giant cells appeared from the 2nd week and reached peak response at 6-8 weeks. These reaction diminished prominently at 12 weeks. No specific pathologic change was found in the liver, the kidneys, and the heart. The above results suggest that the MMC-PGA composite can serve as a new device for intraperitoneal chemotherapy of various types of cancers.

      • 특수교육 정보화 체제 구축 방안 연구

        본 연구는 특수교육 정보화 관련 국내 · 외 문헌자료와 사례를 소개하고, 국내 특수교육정보화 실태 조사를 실시하여 첫째, 기초 통계자료를 확보하고 둘째, 효율적이면서도 실제적인 정보화체제 구축방안을 제시하는데 그 목적이 있다. 그동안 국내에서 광범위하게 추진되었던 교육정보화 사업에 있어서 전체 학교 학내망 설치와 같은 가시적인 정보화기반 구축 사업에 대해서는 전반적으로 성공적이라는 평가를하고 있지만 특정 수요 집단에게는 정보격차의 심화 및 접근성 문제의 심각성 등 부정적인 결과를 초래하기도 하였다. 이에 본 연구에서는 국내·외 특수교육 정보화 사례와 자료를 소개하고, 국내 특수교육정보화 실태를 파악하기 위한 설문조사를 실시하였다. 설문조사 대상은 특수학교 교사, 학생, 장애학생 부모, 장애인관련 단체 및 기관, 시 · 도교육청이었고, 조사 결과 특수학교 교사의 경우 컴퓨터 보유, 인터넷 사용 등에 있어서는 매우 양호하게 나타났으나 정보 활용기술에 해당하는 영역 즉, 장애학생을 교수하기 위한 자료의 제작과 교수-학습과정에서의 적용 수준에서는 저조하게 나타났다. 비장애학생과 비교할 수는 없지만 장애학생의 컴퓨터보유와 인터넷 사용 유 ·무에 관해서는 40~60%의 응답자가 긍정적으로 응답하였고 체계적인 지원이 수반된다면 좋은 결과가 있을 것으로 분석되었다. 본 연구에서는 정보화 지원 체제 구축 방안으로 크게 예산지원과 행정지원을 포함하여 특수학교, 교사, 학부모, 장애학생, 행정가, 장애인관련 기관 등의 요소로 구분하여 지원 방안을 제시하였다. 소개된 지원내용을 간략히 정리하면 다음과 같다. 첫째, 특수교육정보화 체제 구축을 위한 예산 지원은 외국의 사례에서도 나타났듯이 안정성과 지속성을 확보하기 위하여 '초·중등교육법', '특수교육진흥법'을 통해 지원에 관한 세부 사항을 법으로 규정해야한다. 둘째, 특수교육 정보화 사업과 관련된 부처간 행정 협조체제의 수립과 사업 중복성 등을 피하기 위하여 전담기구나 기관의 지정이 요구된다. 셋째, 특수학교의 경우, 학내망 설치와 더불어 장애인 접근성 문제의 해결을 위한 프로그램 개발과 하드웨어 장비의 지원, 교실간 네트워크 구축, 학교간 WAN 구축 등의 사업이 추진되어야 하며, 특수학급의 경우는 정보화 환경구축이 미흡하기 때문에 소속 교육청 과의 네트워크 구축을 통한 정보공유 및 행정업무의 연계가 필요하고, 다양한 교수-학습자료의 지원이 절실히 요구된다. 넷째, 특수교육 교사의 연수 기회를 확대하고 전문성을 키워줄 수 있는 연수내용과 방법의 개선이 필요하다. 장애학생 교육을 위한 학습자료 제작 기법, 장애인용 프로그램 사용법, 특수단말 장비 및 정보매체 활용 기법 등의 프로그램을 개발하여 집체+원격+화상 형태의 입체적인 연수가 요구된다. 다섯째, 장애학생 부모를 위한 정보화 지원은 학교-교육청간 협력하에 부모 교육 프로그램을 개발하여 우선, 학교와 인근 지역의 장애인관련 기관을 활용하여 정보소양 교육을 실시하고, 언제, 어디서나 교육이 가능한 원격이나 사이버시스템 등을 매개로 부모의 여건을 배려해줄 수 있는 편리한 교육방법이 준비되어야 한다. 여섯째, 교육청별 특수교육 담당장학사의 정보화 수준은 양호한 것으로 평가되지만 특수교육 정보화를 위한 특수학교(급) 교사의 연수지원을 위한 예산규모의 확보나 프로그램 개발 지원 등은 매우 부족한 것으로 나타났는데, 교육부(국립특수교육원)를 중심으로 전체 교육청의 특수교육 정보화 업무를 단일 체제로 묶는 인프라를 구축하여 예산, 프로그램, 네트워크 등을 공유토록 하여 효율성을 확보해야 한다. 일곱째, 장애인관련 단체는 정보화 업무 담당자를 배치하여 장애인과 일반인을 대상으로 활발한 정보화 교육이 이루어지고 있었지만 재정지원이 아직도 부족하고, 장애인용 프로그램과 기기의 보급이 부족하여 한계가 있는 것으로 드러났는데, 마찬가지로 장애인관련 기관을 하나로 묶는 인프라의 구축을 통해 사용되는 프로그램을 표준화시키고 예산지원의 중복성을 피해야하며 기관 이용자에 대한 관리의 편리성도 함께 도모해야 할 것이다. This study intends to collect baseline data and to present effective and practical methods of establishing special education IT system. An extensive review of both domestic and foreign literature and case studies of best practices were conducted for this research as well as the evaluation of the current state of technology usage and adoption in special education in Korea. Previous nationwide educational technology projects have accomplished notable and successful results such as building Intranet for schools. However, such efforts have, in away, created and aggravated the disproportionate gap, often called as the digital divide, against certain groups of people with unique needs. Also, the accessibility to such technology and information has emerged as a serious problem as the role of technology expanded. This study introduced both domestic and foreign examples of technology in special education and relevant resources, and conducted a large-scale survey to examine the current stage of technology adoption and usage in special education in Korea. The survey was administered to special education teachers, students, parents of students with disabilities, disability-related organizations and relevant agencies, and to municipal and provincial department of education. The results found satisfactory levels of computer ownership and usage of Internet among special education teachers. However, utilizing and incorporating such Information and technology into classroom activities and into teaching material development were still limited. Although the data were not comparable to those from students without disabilities, about 40 to 60 percent of the respondents showed positive attitudes toward computer ownership and Internet usage of students with disabilities, suggesting desirable outcomes with appropriate systematic support. Based on the findings, this study recommended detailed methods of building broad financial or budgetary assistance systems for special education schools, teachers, parents, students, administrators, and disability-related organizations. First, as shown in the experiences of foreign countries, financial support from the government to establish the system for the special education information technology has to be mandated under the 'Elementary and Secondary Education Act', the 'Special Education Promotion Act' to ensure and enhance stability and continuity. Second, designation of a single commanding and responsible agency is called for to efficiently coordinate inter-agency collaboration among governmental agencies and departments, and to avoid redundancy of work. Third, for special education schools, tasks such as building school-wide Intranet, development of software solving accessibility issues, subsidizing funds for computer hardware purchase, connecting classrooms through networking system and establishing inter-school WAN are of critical importance. Fourth, the contents and methods of professional development and training for special education teachers should be improved to professionalize teachers, and expansion of such opportunities are also called for. Fifth, IT support for parents of students with disabilities is also important. Parent education and training programs should be developed through active collaboration between local department of education and schools, and such training programs may be offered at schools or disability-related facilities. Distance learning or online courses, which the parents can access regardless of time or location, are other feasible alternatives. Sixth, the results found that administrative staffs of special education at each local department of education displayed satisfactory competence levels. However, the amount of budget assigned for special education teacher training or program development was seriously insufficient. Therefore, the Ministry of Education (or KISE) should build a universal infrastructure of IT, which encompasses and shares budget, program and network systems across all the IT offices of the local department of education. Seventh, although most disability organizations have IT specialist and offered IT education programs for both persons with and without disabilities, they have been experiencing continued problems of insufficient financial support and lack of adaptive technology devices and accessible programs. Therefore, building a universal infrastructure across disability organizations is called for to standardize programs, to prevent overlaps in funding, and thus, to enhance administrative efficiency.
