        Lipomatosis: a diverse form of hemifacial hyperplasia

        A case of hemifacial hyperplasia that presented with muscular, skeletal, and dental hyperplasia along with lipomatous infiltration was described. Advanced imaging was useful in identifying the lipomatous infiltration present in the lesion, which raises the possibility of lipomatosis having a diverse presentation in hemifacial hyperplasia. As there was a scarcity of related literature in the field of dentomaxillofacial radiology, this report would make us familiar with its computed tomographic and magnetic resonance image findings.

        Low-pressure pneumatic muscles: development, phenomenological modeling, and evaluation in assistive applications through sEMG analysis

        Compliant actuators have received much attention from researchers over the last two decades. Specifically, pneumatic artificial muscles (PAMs) have been used in several human-in-loop assistive and rehabilitative devices due to their inherent behavior resembling biological muscles. We presented a lucid customizable fabrication process to develop lowpressure actuated PAMs, named pneumatic silicone tube artificial muscles (pSTAMs), and to cater activity-specific actuator requirements. Two constructions of pSTAMs with varying lengths were rigorously experimented at different pressure-load conditions for their isobaric static and stiffness characterizations. Estimation of bandwidth and use of empirical correction factors in the conventional analytical models for quasi-static characterization of pSTAMs were demonstrated. Lumped parameter-based phenomenological model was employed to closely model their dynamic characteristics. A detailed integrated electromyography analysis with surface electromyography signals from the targeted muscle groups was performed to determine the efficacy of using pSTAMs in two activities.

      • Ring Reinforced Silicone based Steering Head for Endoscopy like Applications: FEM simulation, development and force characterization

        Physically interactive applications like tissue sampling have become essential in present day minimally invasive endoscopy/colonoscopy applications. This calls for specialized steering heads with force exertion ability, still maintaining simplified actuation mechanisms having inherent softness and steerability. This article presents development of soft hyperelastic material (silicone) based pneumatically actuated steering head with improved performance achieved through O-ring reinforcements. The design attains omnidirectional bending operations as well as extension capability enabling needle insertion by the steering head. The ring reinforcement not only contains the bulging effect but also enhances extension ability and range of bending. Additionally, it improves stiffness, structural stability and force exertion capability. The presented work implements two layer design principle - the inner actuator is made from silicone of harder shore hardness followed by a thin outer layer of softer silicone, embedding the O-rings in between. The steering head has been designed iteratively through in depth Finite Element modeling and analyses after obtaining material models of silicones experimentally. The actuator has undergone detailed characterization through simulation for its workspace and blocked-tip force capabilities at various configurations. The characteristics have been experimentally validated on the developed prototype.

        Vascular Injuries Due to Penetrating Missile Trauma in Anti-Terrorism Ops

        Purpose: Penetrating vascular trauma though less common poses a challenge to all Surgeons. This study was designed to analyse the profile, management modalities of vascular trauma and the outcomes thereof at a Trauma Care Centre in a Tertiary care setting in hostile environment in India. Methods: A prospective review of all patients with arterial and venous injuries being transferred to the Trauma Center at out Tertiary Care Center between June 2015 and May 2018 was done. Demographics, admission data, treatment, and complications were reviewed. Results: There were a total of 46 patients with 65 vascular injuries, 39 arterial injuries and 26 venous injuries. The age range was 21 to 47 years. Nineteen patients had both arterial and venous injuries. A total of 42 cases presented within 12 hours of injury and complete arterial transections were found in 33 cases (80.49%). There were three mortalities (6.52%) and three amputations (8.33%). The overall limb salvage rate was 91.67% with popliteal artery being the commonest injured artery. Poor prognosticators for limb salvage were increasing time to present to the trauma centre, hypovolemic shock, multi-organ trauma and associated venous injuries. Conclusions: Penetrating missile trauma leading to vascular injuries has not been widely reported. Attempting limb salvage even in cases with delayed presentation should be weighed with the threat to life before revascularisation and should preferably be done at a centre with vascular expertise. A team approach with vascular, orthopaedic, general surgeons, and critical care anaesthesiologists all aboard improve the outcomes manifold. Use of tourniquets and early fasciotomies have been emphasized as is the use of native veins as the bypass conduit. This is probably the largest study on penetrating Vascular trauma in anti-terrorism ops from the Indian subcontinent. It highlights the significance of prompt recognition and availability of vascular expertise in optimally managing cases of vascular trauma.
