HBV : Effects of Pre-S Mutations of Hepatitis B Virus on Hepatitis B Surface Antigen Expression

Background: In chronic hepatitis B virus (HBV) infection, quantitative HBV surface antigen (qHBsAg) level for monitoring viral replication and treatment responses has been highlighted. However, whether mutations in pre-S region have any effect on qHBsAg levels remains uncertain. We aimed to determine patterns of HBsAg expression in naturally-occurring mutations in the pre-S region. Methods: Plasmids expressing 1-8 amino acid deletion in pre- S1 (“pre-S1 mutant”) and 3-25 amino acid deletion in pre-S2 (“pre-S2 mutant”) were constructed. At 72h post-transfection into Huh-7 cells, qHBsAg levels were analyzed in supernatant and cell lysate using electrochemiluminescence immunoassay analyzer. Results: Extracellular qHBsAg levels were on average 3.87-fold higher in cells transfected with pre-S1 mutant compared to wild-type. In contrast, those in cells transfected with pre-S2 mutant were slightly lower compared to wild-type (on average 0.92-fold lower, but without statistical significance). Conversely, compared to wild-type, intracellular qHBsAg levels were on average 0.57-fold lower in pre-S1 mutant, whereas those were on average 1.60-fold greater in pre-S2 mutant. Consistent with these results, immunofluorescence staining of cellular HBsAg showed that cells transfected with pre-S1 mutant were stained to a lesser degree and that those with pre-S2 mutant were more densely stained. Co-transfections of wild-type with either pre- S1 mutant or pre-S2 mutant were performed to mimic milieus of quasispecies at the following ratios; 0:10, 1:9, 3:7, 5:5, 7:3, 9:1, and 10:0. As ratios of either pre-S1 mutant or pre-S2 mutant:wild type increased from 0:10 to 10:0, relative extracellular qHBsAg levels increased accordingly from 1.0 to 3.85 in pre-S1 mutant co-transfection, whereas those decreased from 1.0 to 0.88 in pre-S2 mutant co-transfection. Conclusions: Pre-S deletion exhibit different phenotypes regarding HBsAg expression and these effects depend on precise locations of deletion. Thus, qHBsAg level for diagnosis and prognostification should be used more cautiously, considering the emergence of pre-S deletion mutants.

"This study has been designed to develop a menopause symptom scale. Data were collected through a survey over a period of three months. Subjects who participated in the study were 400 women living in Pusan, Masan and Chin-ju city. The author used a quota sampling method. The analysis of the data was done with SPSS PC+ for descriptive statistics, item analysis and factor analysis. Initially 74 items were generated ftom the interview data of forty middle-aged women and from a literature review. This preliminary scale was analyzed for reliability and validity. The results of this analysis are as follows; I. Initially 74 items were analyzed through the Index of Content Validity(CVI) and 56 items were selected. 2. Five factors were extracted through the principal component analysis, and these contributed to 45.6% of the variance in the total score. Finally 40 items on the scale loaded .30-.70 on one of 5 factors. 3. Each factor was labeled. Factor 1 was labeled 'social and family relationships and included 13 items, factor 2 was labeled 'physical and physiological reactions' and included 9 items, factor 3 was labeled psychiatric and psychological reactions' and included 10 items, factor 4 was labeled 'genitourinary reactions' and included 4 items, factor 5 was labeled 'relationship with sexual partner and included 4 items. 4. Cronbach Coefficient Alpha for internal consistency was 0.9219 for the total 40 items. Finally, the author suggests that this scale could be adequately applied in assessing menopause symptoms of women in general. The results of this study can contribute to designing an appropriate menopause symptom prevention strategy. "
생발음(生髮飮)-2 피부도포가 C57BL/6 마우스의 육모촉진에 미치는 효과

Objectives: Saengbal-eum-2(Shengfa-yin-2) is a hair care product which is composed of ten plant extracts used in oriental medicine. This study was carried out to investigate the effects of Saengbal-eum-2(Shengfa-yin-2) on hair regrowth and cytokine changes in a shaving model of C57BL/6 mice. Materials and Methods: Five-week-old mice were acclimated for 1 week at a temperature between 21-23℃, 40-60% relative humidity, and 12h of a light/dark cycle before beginning of the experiment. There were three experimental groups including 50% ethanol (EtOH, control), a positive control of 3% Minoxidil, and 30% Saengbal-eum-2(Shengfa-yin-2) in 50% ethanol in 18 female mice. The test compounds were topically treated once a day over 12 days. The hair regrowth was photographically and histologically determined during the experimental period of 12 days. Revelation of EGF, TGF-β1 and IL-6 in hair follicle were also determined using immunohistochemistry. In addition to that, IL-6, TNF-α, and IL-1β in skin tissue were determined using enzyme-linked immunosorbent assay(ELISA). Results: Hair regrowth in 3% Minoxidil and Saengbal-eum-2(Shengfa-yin-2) groups was promoted earlier and faster than the control group. Concentrations of hairs and thick-hair ratio in 3% Minoxidil and Saengbal-eum-2(Shengfa-yin-2) groups were promoted than the control group. EGF was moderately positive in hair follicle of 3% Minoxidil and Saengbal-eum-2(Shengfa-yin-2) groups, but negative in the control group. TGF-β1 was not significantly difference between the groups. IL-6 in hair follicle of Saengbal-eum-2(Shengfa-yin-2) group was negative, but weakly positive in 3% Minoxidil and control group. IL-6 and IL-1β in skin tissue were significantly decreased in Saengbal-eum-2(Shengfa-yin-2) group, but there was not significantly decreased in 3% Minoxidil and control group. TNF-α in skin tissue was significantly decreased in 3% Minoxidil and Saengbal-eum-2(Shengfa-yin-2) groups. Conclusions: These results suggest that Saengbal-eum-2(Shengfa-yin-2) has hair growth promoting activity and it can be used for treatment of alopecia. And these effects relate to EGF revelation of hair follicle and a decrease IL-6, TNF-α, and IL-1β in skin tissue.
Malignant melanoma associated with a plaque-type blue nevus of the cheek: a case report

Blue nevi, which are characterized by collections of pigment-producing melanocytes in the dermis, have a variety of clinicopathological characteristics. Plaque-type blue nevus (PTBN) is a variant of blue nevi. PTBN presents at birth or arises in early childhood, and it shows a combination of the features found in common blue nevus and cellular blue nevus. It is typically found on the dorsal surface of the hands and feet or on the head and neck, and it is usually benign and stable over time. However, reports have occasionally described malignant melanomas developing in or associated with a PTBN. Malignant blue nevi are most commonly found on the scalp. We report the case of an 88-year-old woman with a malignant melanoma associated with a PTBN of the cheek.

Mental health is one of the most important issues for disaster survivors, and many studies have reported higher rates of mental health problems after disasters. Because Japan has experienced frequent earthquakes, tsunami, typhoons, and volcanoes, mental health problems have been a matter of great concern after disasters in Japan. To serve the psychiatric services after the Hanshin-Awaji (Kobe) earthquake, Hyogo institute for traumatic stress was established. And when the disaster caused by the great east Japan earthquake and tsunami occurred, National information center of disaster mental health and Disaster psychiatric assistance team had the important role of reducing mental health problems.

Comprehensive analysis of the transcriptome of the P. chrysosporium is a useful approach to improve our understanding of its special and unique enzyme system and fungal evolution in molecular and industrial aspects. In order to unveil the functional diversity of this white-rot fungus in gene level and the expression patterns of its genes, in this study we carried out sequencing and annotation of 4,917 P. chrysosporium expressed sequence tags (ESTs). Through our bioinformatic ESTs analysis, we elucidated that 1,751 genes were derived from the present dataset of 4,917 ESTs, based on clustering and comparative genomic analyses of the ESTs. Of the 1,751 unique ESTs, 1,006 (57.5%) had homologues and orthologues in similarity searches. Our P. chrysosporium ESTs showed many genes for encoding 23 secreted proteins, many proteins for the degradation of cellulose and hemicelluloses, and heat shock proteins for stress resistance, which explain the reason why P. chrysosporium is very important and unique white-rot fungus in dealing with contaminated resources and in degrading lignin and in applying this organism to several industrial aspects. In addition, comparative analysis has shed the fresh light on the mystery about how its unique enzyme system and stress resistance have been evolved differently from its closest relatives.

In vivo and in vitro skin permeation of recombinant ^(125)I-EGF through normal. stripped and the first degree burn skin were studied. The in vitro skin permeation rate through the first degree burn skin (296 cpm/㎠/hr) and the stripped skin (1131 cpm/㎠/hr) were 3.5 times and 13 times higher, respectively, as compared with the one through normal skin. In vivo absorption study with the first degree burn skin, the peak concentration of EGF in the skin was achieved at 1-3 hr and decreased afterward up to 8 hr with an elimination constant of 1.31 × 10³g/㎖/hr. To investigate the higher elimination rate of EGF in burn skin, binding and metabolism studies were conducted. No significant metabolism of EGF in burn skin (100℃, 5second burning) was observed. With the presence of unlabelled-EGF, ^(125)I-EGF permeation through the burn skin showed higher permeation rate than the one without unlabelled-EGF. The result may indicate that EGF-receptor binding play a role in determining the skin permeation rate.
