Atopic dermatitis(AD) has long been considered as an allergic and inmmunologically-mediated disease. Most of the pathogenetic as well as therapeutic approaches focused on the underlying immunological abnormalities and relied upon inhibiting allergen-specific Th2 cells and their consequent inflammatory mediators thus alleviating their clinical symptoms. Recent understanding of the physiology of altered epidermal barrier functions and their associated genetic abnormalities underlying AD opened the possibilities of new fields to effectively manage this troublesome disease. These also gave further insights into the possible mechanisms of expression of extrinsic AD and secondary respiratory atopic diseases resulting from the disturbed barrier functions of AD. It is hoped that adequate management of altered skin barrier with suitable barrier repairing topicals will satisfactively manage AD and further prevent atopic march not relying on high-risk systemic immunsuppressive drugs traditionally used to manage severe atopic diseases.
