        Endoscopic Assessment of Esophagitis with Transnasal Esophagoscopy in the Prediction of Treatment Response

        Objectives : The purpose of this study was to investigate the endoscopic evidence of esophagitis in laryngopharyngeal reflux disease (LPRD) patients using transnasal esophagoscopy (TNE) and to correlate these findings with treatment response. Methods : Fifty patients underwent TNE at Korea University Anam Hospital from July 2007 to Feb 2009. Participants were selected from patients that presented with various laryngeal symptoms. One experienced otolaryngologist assessed esophagitis according to the Los Angeles classification system using the TNE findings. Results : Fifteen of 50 LPR patients (30%) were found to have esophagitis (12 patients with Grade A, 3 patients with Grade B, no patients with grade C/ D esophagitis). Among the 15 patients positive for esophagitis based on the endoscopic findings, 12 (80%) showed symptom improvement after pharmacological therapy. Symptom improvement was correlated with evidence of esophagitis (p=0.002) but not with RFS (p=0.749). Conclusion : Endoscopic evaluation of esophagitis using TNE is a potentially valuable tool for predicting treatment response in LPRD patients.

        Application of Coblation Resection in Various Benign Laryngotracheal Diseases

        Objectives : The aim of this study was to evaluate the effectiveness and feasibility of coblation resection for the treatment of laryngotracheal disease. Methods : We conducted a retrospective review, evaluating 7 patients with laryngotracheal disease treated using coblator. Information collected included demographic data of the patients, diagnosis, size and location of the disease, procedure time, the number of previous operation, and the postoperative complication. Results : Among the etiology, granulation is most frequent (n=4), followed by recurrent repiratory papillomatosis (n=2) and tracheal stenosis (n=1). The location of lesions was peristomal area (n=2), glottis (n=2), subglottis (n=2) and mid-trachea. Coblation resection could remove the lesions completely and there was no significant complication including local burn, bleeding, and hypertrophic scar. The procedure time was shorter than the previous operation using $CO_2$ laser. Conclusion : Coblation resection is an effective and safe method for layngotracheal disease and can substitute the classic method such as $CO_2$ laser.

        Development of Two-Dimensional Scanning Videokymography for Analysis of Vocal Fold Vibration

        Objectives : We developed two-dimensional (2D) scanning videokyomography to evaluate the mucosal wave of whole vocal cords in real time to overcome the limit of preexisting stroboscopy and line scanning videokymography which could not evaluate it. Methods : We implemented a continuous light source with high brightness, a high-definition CMOS camera, and capture board for saving the data. We created the software program to analyze the image data from the system. The test of the functionality of the 2D scanning videokymography camera was performed in one of the authors (P.H.J 32 years old male). Vocal cord images were obtained during normal phonation and falsetto phonation. Images were obtained also during cough, diplophonia. Results : The system made it possible to measure objective parameters, including fundamental frequency, amplitude, regularity, mucosal wave, and phase difference, medial and lateral peak, opening versus closing duration related to vocal fold vibration. Simultaneously, it enabled analysis of the whole mucosal wave of the entire vocal fold in real time. 2D scanning videokymography was also effective for evaluating the dynamic status of the vocal fold when the subject phonated aperiodic voice. Conclusion : In conclusion, 2D scanning videokymography can support the analysis of the whole mucosal wave of the entire vocal cord with objective vocal parameters, overcoming the limitations of stroboscopy and previous line scanning videokymography techniques.

        A Case of the Laryngopharyngeal Burn Caused by A Microwave-Heated Egg Bite

        Nowadays, microwave ovens are commonly used for cooking. However, the cooking certain food in microwave ovens can be hazardous. It has been reported that the heating eggs in a microwave oven can cause an explosion and injuries. The exploding eggs can cause burn injury of face, neck, eyelid, pharynx and larynx. Direct thermal damage to the larynx after swallowing microwave-heated eggs is rare. The authors report a larynx thermal injury due to microwave-heated eggs and review the cause and the prevention methods of exploding eggs.

        Huge Subglottic Polyp Treated with Tracheotomy and Laryngofissure

        Vocal polyps are benign laryngeal lesions which arise from the Reinke's space abd hoarseness is the most common symptom. However, airway compromised is rarely presented in the vocal polyp. A rare case of large subglottic polyp causing dyspnea is reported. Tracheostomy was performed under local anesthesia and then the mass was resected under general anesthesia using a laryngofissure approach. The dyspnea and hoarseness disappeared after surgery immediately. The histopathological findings indicated a diagnosis of vocal cord polyp with chronic inflammatiuon. We consider that tracheostomy is the safest and most useful procedure to guarantee the upper airway in cases of large vocal polyp showing dyspnea. We hereby report a case of huge subglottic polyp in which a tracheostomy and laryngofissure was required for removing the subglottic mass successfully.

        A Case of Left Vocal Cord Paralysis Caused by Aortic Arch Aneurysm : A Variant of Ortner's Syndrome

        Ortner's syndrome is a rare clinical entity with hoarseness attributable to recurrent laryngeal nerve palsy in cardiovascular diseases. The mechanism of this condition is thought to be due to compression of the recurrent laryngeal nerve by a dilated, tense cardiovascular structure against its adjacent tissue; thus, this is also known as cardio-vocal syndrome. We experienced the case of a 81-year-old female suffering from hoarseness due to a large aneurysm of the aortic arch, and reviews the literature for possible cardiovascular causes of Ortner's syndrome.

        Primary Laryngeal Aspergillosis in Immunocompetent Patient - A Case Report and Review -

        Primary laryngeal aspergillosis is rare, It is most often found in immunocompromised patient, such as leukemia, malignant disease, diabetes or immunosuppressive drugs. These days the occurrences of laryngeal aspergillosis in immunocompetent patients are increasing. The cause of laryngeal aspergillosis in immunocompetent patients is not clear, but a few factors are considered such as iatrogenic factors, vocal abuse, vocal fold cyst and occupational factors. The histopathologic characteristics are somewhat different between that of immunocompromised patients and immunocompetent patients. We report a case of primary vocal cord aspergillosis in immunocompetent patient who had treated with only surgery and brief review of the pertinent literature.

        Evaluation of Vocal Efficiency for the Polyps and Nodules

        The vocal efficiency(VE) can be calculated as the ratio of acoustic power to aerodynamic power. It relates to the vocal intensity, air Sow rate and subglottic pressure. In this study, we treated 20 cases of vocal polyps and 10 cases of vocal nodules by way of laryngo-microsurgery or laser laryngo-microsurgery. The VE was measured preoperatively and postoperatively in all cases. The results showed that there was a significant improvement of VE postoperatively than that of preoperative measurement(p<0.01) in vocal Polyp group. However, there was no significant difference(p>0.05) in vocal nodule group. Through comparing the results, we obtained the conclusion : The laryngo-microsurgery is the reliable method of management for the vocal polyps, but f3r the vocal nodules, the laryngo-microsurgery should be selected after other more conservative approaches fail to produce the desired results.

        An EMG Study of the Feature 'Tensity'

        Previous studies reveal that in English there is no EMG evidence fur the feature tense-lax distinction. The technique of electro-myography(EMG) was used to see if the existing claim holds true, particularly in unstressed syllable. It was found that in unstressed syllable, the peak EMG amplitude from the orbicularis oris superior muscle was significantly greater in /p/ than in /b/, while in stressed syllable this difference was negligible. It was hypothesized that in stressed syllable, /p/ and /b/ may be differentiated by the EMG activities from a muscle other than the orbicularis oris superior muscle, e.g. the respiratory muscles relating to 'aspiration' or depressor anguli oris muscle. In Korean, there was a clear labial gestures for the feature tense-lax distinction. The phoneme-sensitive manifestation of stress and some possible reasons for the inter-speaker variability in the data and the variability within a given speaker were discussed.

        Incidental Ignition of a Pulsed Dye Laser Fiber During Laryngomicrosurgery : A Case Report

        Surgical fires require an ignition source, oxidizer, and fuel. The pulsed dye laser (PDL) has been shown to be effective in the treatment of hypertrophic scars and keloids in dermatology. With the increasing number of applications of $CO_2$, laser as ignition source has been associated with operating room fires in the otorhinolaryngologic field. There have been many case reports on PDL-induced fires in dermatology, but until now, there were no reports in the larynx. We describe a 57-year-old patient diagnosed with laryngeal hyperkeratosis treated by PDL-assisted laryngomicrosurgery. In this case, we experienced incidental PDL tip flaring during pulsed dye laser vaporization.
