Health Economics Evaluation of a Gastric Cancer Early Detection and Treatment Program in China

Objective: To use health economics methodology to assess the screening program on gastric cancer in Zhuanghe, China, so as to provide the basis for health decision on expanding the program of early detection and treatment. Materials and Methods: The expense of an early detection and treatment program for gastric cancer in patients found by screening, and also costs of traditional treatment in a hospital of Zhuanghe were assessed. Three major techniques of medical economics, namely cost-effective analysis (CEA), cost-benefit analysis (CBA) and cost-utility analysis (CUA), were used to assess the screening program. Results: Results from CEA showed that investing every 25, 235 Yuan on screening program in Zhuanghe area, one gastric cancer patient could be saved. Data from CUA showed that it was cost 1, 370 Yuan per QALY saved. Results from CBA showed that: the total cost was 1,945,206 Yuan with a benefit as 8,669,709 Yuan and an CBR of 4.46. Conclusions: The early detection and treatment program of gastric cancer appears economic and society-beneficial. We suggest that it should be carry out in more high risk areas for gastric cancer.

Gastric cancer is the second most common cause of cancer-related deaths in the world. Although certain dietary factors and lifestyles have been suggested to be associated with gastric carcinogenesis, there have been few investigations focusing on rural areas. A case-control study was therefore carried out to investigate the risk factors of gastric cardia cancer (GCC) in rural areas of Linzhou. A total of 470 newly diagnosed cases of GCC and 470 healthy controls were included. Face-to-face interviews were conducted, using a uniform questionnaire containing questions on demographics, per capita income, living habits, dietary habits and family history of tumors. The relationship between putative risk factors and GCC was assessed by odds ratios (OR) and their 95% confidence intervals (95%CI) derived from conditional logistic regression model by the COXREG command using SPSS 12.00. Multivariate logistic regression analysis was used to evaluate simultaneously the effects of multiple factors and other potential confounding factors. Multivariate logistic analysis showed that smoking (OR=1.939, 95%CI:1.097-3.426), alcohol drinking (OR=2.360, 95%CI: 1.292-4.311), hot food consumption (OR=2.034, 95%CI: 1.507-2.745), fast eating (OR=1.616, 95%CI: 1.171-2.230), mouldy food (OR=4.564, 95%CI: 2.682-7.767), leftover food (OR=1.881. 95%CI: 1.324-2.671), and family history of tumor (OR=2.831, 95%CI: 1.588-5.050) were risk factors for GCC. High per capita income (OR=0.709, 95%CI: 0.533-0.942), high education level (OR=0.354, 95%CI: 0.163-0.765), consumption of fresh fruits (OR=0.186, 95%CI: 0.111-0.311) and vegetables (OR=0.243, 95%CI: 0.142-0.415), and high BMI (OR=0.367, 95%CI: 0.242-0.557) were protective factors for GCC. Our data indicate that unhealthy lifestyle and dietary habits might be important contributors to GCC in this population.
The Cloud Terminal Online Monitoring System of UPS Battery Performance based on MSP430 MCU

Aiming at some problems in the process of UPS battery real-time monitoring, on the basis of the cloud terminal technology is proposed in this paper, a new method of remote online monitoring, and based on MSP430 single chip microcomputer, combined with modular guiding ideology, in order to improve the UPS battery remote monitoring automation as the core, through the reasonable design of hardware structure, the implementation of UPS battery on-line real-time monitoring of voltage, current and temperature. At the same time, the system based on ADO database access technology and C++ programming to complete the data management, human-computer interaction, and system control, by the cloud terminal implementation monitoring system data transmission between lower place machine and super-ordination machine. Practice has proved that the technology security and stability, has the very good practical and promotional value.

Background: The association between calcium channel blockers (CCBs) and survival in cancer patients remains unclear and the results of related studies are conflicting. The objective of the study was to investigate the association between calcium channel blockers (CCBs) use and survival in cancer patients. Materials and Methods: We searched PubMed, EMBASE, Web of Science and Cochrane Library for studies published before January 2016 with the terms related to CCBs and survival in cancer patients. The information was reviewed and extracted by two evaluators independently. Data of publications was extracted and calculated into hazard ratios (HRs) for overall survival (OS). Statistical analysis was performed by using Review Manager 5.3. Results: There were 11 studies included in our meta-analysis. Analysis of all studies showed that CCBs use was not associated with survival in cancer patients (HR=1.07; 95% CI: 0.91-1.25; P=0.42). No association between CCBs use and overall survival in cancer patients was existed whether in Asian (HR=1.18, 95% CI: 0.72-1.93; P=0.52) or Caucasian population (HR=1.03, 95% CI: 0.89-1.20; P=0.66). Conclusions: There is no evidence that CCBs use is associated with a better or worse outcome of survival in cancer patients.
Affecting Factors of Secondhand Smoke Exposure in Korea: Focused on Different Exposure Locations

<P>Exposure to secondhand smoke (SHS) not only can cause serious illness, but is also an economic and social burden. Contextual and individual factors of non-smoker exposure to SHS depend on location. However, studies focusing on this subject are lacking. In this study, we described and compared the factors related to SHS exposure according to location in Korea. Regarding individual factors related to SHS exposure, a common individual variable model and location-specific variable model was used to evaluate SHS exposure at home/work/public locations based on sex. In common individual variables, such as age, and smoking status showed different relationships with SHS exposure in different locations. Among home-related variables, housing type and family with a single father and unmarried children showed the strongest positive relationships with SHS exposure in both males and females. In the workplace, service and sales workers, blue-collar workers, and manual laborers showed the strongest positive association with SHS exposure in males and females. For multilevel analysis in public places, only SHS exposure in females was positively related with cancer screening rate. Exposure to SHS in public places showed a positive relationship with drinking rate and single-parent family in males and females. The problem of SHS embodies social policies and interactions between individuals and social contextual factors. Policy makers should consider the contextual factors of specific locations and regional and individual context, along with differences between males and females, to develop effective strategies for reducing SHS exposure.</P>
Data Fusion-based Resilient Control System under DoS Attacks: A Game Theoretic Approach

In this paper, the resilient control under the Denial-of-Service (DoS) attack is rebuilt within the framework of Joint Directors of Laboratories (JDL) data fusion model. The JDL data fusion process is characterized by the so-called Game-in-Game approach, where decisions are made at different layers. The interactions between different JDL levels are considered which take the form of Packet Delivery Rate of the communication channel. Some criterions to judge whether the cyber defense system is able to protect the underlying control system is provided. Finally, a numerical example is proposed to verify the validity of the proposed method.
