Objective: In Japan, perioperative prophylaxis of pulmonar y embolism (PE) in gynecologiccancer patients with preoperative asymptomatic venous thromboembolism (VTE) has notbeen well established yet. The GOTIC-VTE trial was a prospective, multi-center, single-armclinical trial to investigate the prevention of postoperative symptomatic PE onset by seamlessanticoagulant therapy from the preoperative period to 4 weeks after surger y instead of usingintermittent pneumatic compression. Methods: Anticoagulant therapy was started immediately after asymptomatic VTE diagnosisand stopped preoperatively according to the rules of each institution. Unfractionatedheparin administration was resumed within 12 hours postoperatively, and this was followedby the switch to low-molecular-weight heparin and subsequently, edoxaban; this cycle wascontinued for 28 days. Primar y outcome was the occurrence of symptomatic PE in 28 dayspostoperatively. Secondar y outcomes were the incidence of VTE-related events in 28 days and6 months postoperatively and protocol-related adverse events. Results: Between Februar y 2018 and September 2020, 99 patients were enrolled; of these, 82 patients were assessed as the full analysis set, including 58 for ovarian cancer, fallopian tube,or peritoneal cancer; 21 for endometrial cancer; and 3 for cer vical cancer. No symptomatic PEwas obser ved within 28 days postoperatively; two patients had bleeding events (major bleedingand clinically relevant nonmajor bleeding) and three had grade 3 adverse events (increasedalanine transaminase, aspartate aminotransferase, or gamma-glutamyl transferase). Conclusion: The multifaceted perioperative management for gynecologic malignancies withasymptomatic VTE effectively prevented postoperative symptomatic PE. The reduction of the ripple of driving force is especially required for the practical utilization of linear synchronous motor for rope-less elevator. In this paper, the magnetic region of the linear motor is optimized by using topology optimization techniques (density method and ON/OFF method) in order to reduce the ripple of driving force. The optimal results of both methods are compared, and useful information for the optimal design of linear motor is obtained.

Objectives: As the population ages worldwide, including in Japan, there is a growing expectation for older adults to remain active participants in society. The act of sharing one’s experiences and knowledge with younger generations through social engagement not only enriches the lives of older individuals, but also holds significant value for our society. In this study, we examined both positive and negative perceptions of older adults and investigated the correlation between these perceptions and generativity among older citizens. Additionally, we evaluated the impact of life satisfaction on these factors. Methods: We conducted a survey of 100 older adults in Japan (mean age, 71.68 years) and utilized multiple regression analyses, using positive and negative perceptions of older adults, life satisfaction, and demographic factors as independent variables. The sub-categories of generativity—namely, generative action, concern, and accomplishment—were used as dependent variables. Results: Participants who held a more positive perception of older adults demonstrated a higher level of generative actions and concerns. Additionally, participants who reported higher levels of life satisfaction also exhibited more generative actions, concerns, and accomplishments. Conversely, those who held a more negative perception of older adults were found to have higher levels of generative actions. Conclusion: Enhancing positive perceptions of older adults among them can boost the sub-categories of generativity. This study, which was conducted from an exploratory perspective, has several limitations, including a potential sampling bias. A more comprehensive examination of the relationship between perceptions of older adults and generativity is anticipated in future research.
Modified Open-Door Laminoplasty Using a Ceramic Spacer and Suture Fixation for Cervical Myelopathy

Purpose: To introduce a new simple technique using suture anchors and ceramic spacers to stabilize the elevated laminae in open-door cervical laminoplasty. Although ceramic spacers were placed in the opened laminae and fixed with nylon threads in this series, it was occasionally difficult to fix the nylon threads to the lateral mass. Materials and Methods: Study 1: A preliminary study was conducted using a suture anchor system. Sixteen consecutive patients who underwent surgery for cervical myelopathy were prospectively examined. Study 2: The second study was performed prospectivelyto evaluate the feasibility of this new technique based on the result of the preliminary study. Clinical outcomes were examined in 45 consecutive patients [cervical spondylotic myelopathy (CSM)] and 43 consecutive patients (OPLL). The Japanese Orthopedic Association scoring system (JOA score), axial neck pain, and radiological findings were analyzed. Results: 1) In one case, re-operation was necessary due to dislodgement of the ceramic spacer following rupture of the thread. 2) In all patients, postoperative CT scans showed that the anchors were securely inserted into the bone. In the CSM group, the average JOA score improved from 9.5 points preoperatively to 13.3 at follow-up (recovery 51%). In the OPLL group, the average JOA score improvedfrom 10.1 (5–14) points preoperatively to 14.4 (11–16) at follow-up (recovery 62%). There were no serious complications. Conclusion: The use of the suture anchor system made it unnecessary to create a hole in the lateral mass and enabled reliable and faster fixation of the HA spacers in open-door laminoplasty.

<P>Artificially designing the crystal orientation and facets of noble metal nanoparticles is important to realize unique chemical and physical features that are very different from those of noble metals in bulk geometries. However, relative to their counterparts synthesized in wet-chemical processes, vapor-depositing noble metal nanoparticles with the desired crystallographic features while avoiding any notable impurities is quite challenging because this task requires breaking away from the thermodynamically favorable geometry of nanoparticles. We used plasma-generated N atoms as a surface-active agent, a so-called surfactant, to control the structural development of Ag nanoparticles supported on a chemically heterogeneous ZnO substrate. The N-surfactant-facilitated sputter deposition provided strong selectivity for crystalline orientation and facets, leading to a highly flattened nanoparticle shape that clearly deviated from the energetically favorable spherical polyhedra, due to the drastic decreases in the surface free energies of Ag nanoparticles in the presence of the N surfactant. The Ag nanoparticles successively developed a nearly unidirectional (111) orientation aligned by stimulating the crystalline coupling of Ag along the orientation of the ZnO substrate. The experimental and simulation results not only offer new insights into the advantages of N as a surfactant for the orientation and shape-controlled synthesis of Ag nanoparticles <I>via</I> sputter deposition but also provide the first solid evidence validating that immiscible, nonresidual gaseous surfactants can be used in the vapor deposition processes of noble metal nanoparticles to manipulate their surface free energies.</P>
