The antifungal activity of the PR-5 family of plant defense proteins has been suspected to involve specific plasma membrane component(s) of the fungal target. Osmotin is a tobacco PR-5 family protein that induces apoptosis in the yeast Saccharomyces cerevisiae. We show here that the protein encoded by ORE20/PHO36(YOL002c), a seven transmembrane domain receptor-like polypeptide that regulates lipid and phosphate metabolism, is an osmotin binding plasma mrmbrane protein that is required for full sensitivity to osmotin. PHO36 functions upstream of RAS2 in the osmotin-induced apoptotic pathway. The mammalian homolog of PHO36 is a receptor for the hormone adiponectin and regulates cellular lipid and sugar metabolism. OS-motion and adiponectin, the corresponding "receptor" binding proteins, do not share sequence similarity. However, the β barrel domain of both proteins can be overlapped, and osmotin, like adiponectin, activates AMP kinase in C2C12 myocytes via adiponectin receptors.
Blood Transfusions for Emergency Laparotomies in General Surgery

Purpose: Decisions regarding perioperative blood transfusions are subject to clinical and laboratory factors. Blood transfusions are associated with increased risk of infection, sepsis, organ failure, and length of stay. Current guidelines on transfusions are based on elective settings. There is a paucity of data on blood transfusion use in emergency surgery. This study reviews the appropriateness of blood transfusions in patients undergoing emergency general surgical laparotomies.Methods: Patients undergoing emergency general surgical laparotomies at Peninsula Health from January 2013 to May 2015 were reviewed. Hemoglobin (Hb) levels triggering transfusion and overall blood utilization were obtained. Transfusions were classified based on whether they were given pre-, intra- or postoperatively. Transfusions with Hb >80 g/L in the absence of bleeding or preoperative anemia were deemed ‘inappropriate’ as per Australian Blood Authority guidelines.Results: Over the 29-month period, 368 patients underwent 398 emergency laparotomies. Blood transfusions were given to 102 patients (27.7%). These patients required 240 transfusion episodes. Patients were given a median of three units of blood. One hundred and sixty-six transfusions (69.2%) were postoperative. Forty-six transfusions (19.2%) were given with Hb >80 g/L in the absence of other indications, and were deemed inappropriate. Inappropriate trans-fusions occurred more frequently on the ward compared to ICU (p<0.05). Almost two thirds of inappropriate transfusions were given for Hb 80∼85 g/L.Conclusion: Nearly one in five patients received an inappropriate transfusion. More judicious use of blood products in emergency patients is required, especially on surgical wards. (J Acute Care Surg 2017;7:15-22)

Study Design: Prospective observational study. Purpose: To determine the incidence of postoperative urinary retention (POUR) in patients undergoing elective posterior lumbar spine surgery and identify the risk factors associated with the development of POUR. Overview of Literature: POUR following surgery can lead to detrusor dysfunction, urinary tract infections, prolonged hospital stay, and a higher treatment cost; however, the risk factors for POUR in spine surgery remain unclear. Methods: A prospective, consecutive analysis was conducted on patients undergoing elective posterior lumbar surgery in the form of lumbar discectomy, lumbar decompression, and single-level lumbar fusions during a 6-month period. Patients with spine trauma, preoperative neurological deficit, previous urinary disturbance/symptoms, multiple-level fusion, and preoperative catheterization were excluded from the study. Potential patient- and surgery-dependent risk factors for the development of POUR were assessed. Univariate analysis and a multiple logistical regression analysis were performed. Results: A total of 687 patients underwent posterior lumbar spine surgery during the study period; among these, 370 patients were included in the final analysis. Sixty-one patients developed POUR, with an incidence of 16.48%. Significant risk factors for POUR were older age, higher body mass index (BMI), surgery duration, intraoperative fluid administration, lumbar fusion versus discectomy/ decompression, and higher postoperative pain scores (p <0.05 for all). Sex, diabetes, and the type of inhalational agent used during anesthesia were not significantly associated with POUR. Multiple logistical regression analysis, including age, BMI, surgery duration, intraoperative fluid administration, fusion surgery, and postoperative pain scores demonstrated a predictive value of 92% for the study population and 97% for the POUR group. Conclusions: POUR was associated with older age, higher BMI, longer surgery duration, a larger volume of intraoperative fluid administration, and higher postoperative pain scores. The contribution of postoperative pain scores in the multiple regression analysis was a significant predictor of POUR.

While there is a viable increase in the incorporation of gardens and outdoor spaces in residential facilities for elderly which offers them great opportunity to have a healthy life style to be active and to interact with neighbors, however these spaces remain less utilized despite many identified benefits. This can be due to combination of increasing frailty in late life and barriers in accessing the outdoor environments. Therefore, a balance of well-designed easily accessible outdoor environment, and an environment that brings the great natural outdoors into the built indoor environment is much needed in elderly housing design. Biophilic design concept in elderly care design can be approached to refine and rethink the outdoor space and moreover, in the indoor space by consciously incorporating greenery, we are unconsciously reconnecting them with nature. These biophilic design spaces becomes a great source of active social interaction among the users as well. The purpose of this study is to identify on what spaces do the elderly take satisfaction in when it comes to interactive amenities spaces that is provided for them in their residential community and how those spaces can be approached by adopting biophilic design principles. The empirical material of the research consists of case study and an online questionnaire. To study the impact of biophilia design in elderly housing, specific cases which were designed by adopting biophilic design in elderly care were studied, then an online survey was conducted with 30 elderly participants from India and Republic of Korea to know their preferences towards providing them a biophilic interactive space in their housing environment.

This paper investigates numerically the combined effect of employing multiple Phase change materials (PCMs) and dispersion of high conductivity particles on the thermal performance of an energy storage unit. The key parameters that govern the performance have been identified following a simple reduction of variables. Among the different parameters, melting temperature of the PCMs, latent heat and melting temperature difference between two consecutive PCMs (DT m ) are found to have a significant influence on the performance. A case study considering a 3-PCM unit to study the effect of key parameters during melting has been presented. The results show that for the 3-PCM unit, the performance is better when the second and third PCMs placed have their latent heats larger than the first PCM. It has been recommended to have DT m1 > DT m2 for the 3-PCM unit in order achieve more melting.
Direct Organogenesis in Geophila reniformis D. Don., an Important Medicinal Herb

Adventitious multiple shoots were developed from leaf, petiole and internode explants of Geophila reniformis D. Don. on MS medium supplemented with various concentrations of $N^6$-benzylaminopurine (BAP) or Kinetin (KIN) alone or in combination with indole-3-acetic acid (IAA). Leaf showed maximum organogenetic potential, followed by petiole and internode. Murashige and Skoog (MS) medium supplemented with 22.22 $\mu{M}$ BAP and 4.57 $\mu{M}$ IAA induced maximum shoot buds from leaf explants. Internodal segments showed low potential of direct organogenesis. The regenerated shoots rooted the best in presence of 10.75 - 13.44 $\mu{M}$ $\alpha$-naphthalene acetic acid (NAA) along with 2.22 $\mu{M}$ BAP, and were successfully established in the field with a survival rate of 89.11%.
