      • Impact of Interferon-Based Treatment on Quality of Life and Work Related Productivity from the Korean Cohort in the MOSAIC Study

        Aims: Chronic Hepatitis C Virus (HCV) infection increases the risk for progressive liver disease, hepatocellular carcinoma and negatively impacts the patient’s quality of life. HCV treatment is evolving with direct acting antivirals but IFN based therapy has been the standard of care for many years and remains available in some countries. The MOSAIC study aims to characterize patients with chronic HCV infection and assess the impact of IFN-containing treatment on health-related quality of life, work related productivity and health care utilization. Methods: MOSAIC is an international prospective multicenter observational study that has been conducted in 20 countries. Consecutive patients with chronic HCV infection were enrolled and those who initiated an IFN based regimen were prospectively followed for 48 weeks. We report results from the Korean cohort Results: 100 patients were enrolled: 86 were treatment naïve and 14 were treatment experienced. 33 patients initiated an IFN based regimen: 6 patients started IFN + RBV, 26 patients started Peg-IFN + RBV, none started Peg-IFN + RBV + DAA and 1 patient received other treatment. Among the treated cohort, demographic and disease characteristics were the following: the mean age was 54.5 years; 14 patients were male. 14 had minimal or no fibrosis, 2 portal fibrosis, 3 bridging fibrosis and 6 patients suffered from cirrhosis. HCV Genotype distribution was as follows: genotype 1: 11; genotype 2: 19 and genotype 3: 3. Table 1 describes the results at baseline and changes over 4, 12 and 48 weeks and end-of-treatment (EOT) for the quality of life and work productivity outcome measures (EQ-5D-5L, HCV-PRO and WPAI). Conclusions: Results from the Korean cohort of the MOSAIC study show a moderate trend for deterioration of health-related quality of life and work productivity associated with IFN based treatment for patients with chronic HCV infection during treatment period.         원위 경골 간부 골절에서 최소 침습적 경피적 금속판 고정술과 관혈적 금속판 고정술의 비교

        목적: 경골 원위부 골절의 치료에 있어 최소 침습적금속판 고정술과 관혈적 고정술에 대한 비교를 통한 효율성에 대한 연구고찰을 하고자 한다. 대상 및 방법: 2001년 6월부터 2004년 6월까지 경골 원위부 골절에 대하여 수술을 시행한 최소 1년 이상의 추시가 가능하였던 30예 중 최소 침습적 금속판 고정술을 시행한 15예를 A군, 관혈적 금속판 고정술을 시행한 15예를 B군으로 나누어 수술시간, 술 후 재활, 술 후 운동범위, 동통에 대하여 McLennan과 Ungersma의 기준을 이용한 임상평가, 술 후 염증반응에 대한 C-반응성 단백 및 술 후 합병증을 후향적으로 비교 분석하였다. 결과: A군과 B군의 수술시간은 각각 63분 (45∼105)/129분 (80∼120), 능동적 관절 운동가능시기는 8.3일 (6∼14)/15.8일 (13∼21), McLennan과 Ungersma의 기준에 의한 임상평가는 두 군 모두에서 대부분 양호 이상의 결과를 보였으나, B군에서는 보통 1예가 있었다. 술 후 3일, 7일째 각각 시행한 CRP는 A군에서는 4.0 mg% (범위: 0.9∼7.2)/0.5 mg% (0.1∼1.5), B군에서는 7.97 mg% (2.8∼14.6)/1.0 mg% (0.3∼1.6)로 A군에서 조직손상이 적고 회복이 빨랐다. 술 후 합병증으로는 A군에서는 표재성 감염 1예와 5도 이상의 족관절 외반변형 1예가 있었고, B군에서는 표재성 감염 1예, 감염성 불유합 1예와 10도 미만의 족관절 배굴장애를 보이는 경우가 2예 있었다. 결론: 경골 원위부 골절의 수술적 치료에서 최소 침습적 금속판 고정술이 관혈적 고정술에 비해 조직의 염증반응과 합병증이 적고 수술시간 및 술 후 재활 등에서 보다 효율적인 술식으로 사료된다. Purpose: To evaluate the efficacy of the surgical treatment through the comparison of MIPPO vs open plate fixation in the treatment of the distal tibia fracture retrospectively. Materials and Methods: 30 patients with distal tibia fracture from Jun. 2001 to Jun. 2004 were divided into two groups depending on the surgical method. Minimum follow up was 12 months. Group A consisted of 15 patients treated with MIPPO, Group B was 15 patients treated with open plate fixation. The clinical outcomes were evaluated retrospectively from operation time, rehabilitation, ROM, interval change of postoperative CRP to assess postoperative inflammatory reaction, postoperative complications and clinical result with the use of McLennan and Ungersma criteria. Results: There were no significant differences in clinical result by McLennan and Ungersma criteria in both groups. The postoperative 3 days and 7 days CRP were 4.0 mg% (0.9∼7.2)/0.5 mg% (0.1∼1.5) in group A and 7.97 mg% (2.8∼14.6)/1.0 mg% (0.3∼1.6) in group B, shows more minimal tissue injury and early recovery in group A. Operation time in group A was shorter than group B. Normal recovery of ROM was quicker in Group A. In complications, group A showed one superficial infection and one angular deformity and group B showed one superficial infection, one infected nonunion and two ankle stiffness. Conclusion: There were no significant differences in clinical result and bony union. MIPPO technique is superior to group B in view of the minimal tissue injury, complications, operation time and postoperative rehabilitation.

        제 3 대구치 발치시 환자의 호흡 패턴이 스트레스 변화 양상에 미치는 영향

        Purpose: Heart rate variability(HRV) is the clinical consequence of various influences of the autonomic nervous system(ANS) on heart beat. HRV can estimate the potential physiologic rhythm from the interval between consecutive beats(RR interval or HRV data). It is known as RSA which represents respiration-related HR rhythmic oscillation. Previous studies demonstrated a specific breathing pattern(0.1Hz, 6breaths/min) to improve a physiological body condition related to the stress. In this paper, the level of stress would be evaluated in terms of three phases of the dental treatment, combined with 6breaths/min. Methods: These phases include before, during and after tooth extraction or anesthesia or something.36 patients' stresses were assessed using HRV stress analyzer in each phase in Kangdong Sacred Heart Hospital, and Chuncheon Sacred Heart Hospital, Hallym University Medical Center from Jun. to Sept. of 2007. HRV 5-min data collected were analyzed in time-domain and frequency-domain to evaluate the activity of autonomic nervous system(ANS) which represents the level of stress. Results: All HRV parameters including HF(high frequency), LF(low frequency) and LF/HF ratio showned a significant change affecting the ANS balance. There was a 6.4% difference between R(LF/HF)s on general breathing pattern for balance of Autonomic nervous system, but on controlled breathing pattern, 0.1Hz, was made narrow till 1.4%. The activity of ANS has increased by 1.4% on general breathing pattern, and by 2.9% on controlled breathing pattern, 0.1Hz. Conclusion: After analysis of preoperative stress changes and effect of breathing pattern of 0.1 Hz on the stress in 36 patients who have undergone third molar extraction, following was concluded. In the preoperative stage, the sympathetic change was the greatest?after the?anesthetic injection, and stress was relieved by controlling the breathing pattern to a frequency of 0.1Hz.

      • 제주도 연안 인공어초 시설어장의 자원조성과 생산효과에 관한 연구

        The adhesive condition of shellfish and seaweeds and effects of production for artificial reefs were surveyed through field investigation during the period of 1994~1998 in the coast of Jeju Island. The obtained results are summarized as follows : 1. 4 species of shellfish & 44 fishies (950g/m2) were found at Overbridge type reefs area in the coast of Hado-Ri and 2 species of shellfish & 21 fishies (475g/m2) were found at Horn trangular type reefs area in the coast of Ilgwa-Ri and 1 species of shellfish & 6 fishies (30g/m2) were found at Horn trangular type reefs area in the coast of Biyang-Do. This shows a high density of shellfish in that area except for Biyang-Bo reefs area. 2. Adhesive living condition of seaweeds was showed 9 species (8.270g/m2) in the Hado-Ri and 5 species (4 330g/m2) in the Ilgwa-Ri and 5 species (10.490 g/m2) in the Biyang-Do grounds. This showed a high density of Ecklonia cava and Sargassumm fulvellum in that area except far Biyang-Bo reef area, but Biyang-Do area was a high density of Codium fragile. 3. The result are catch per unit from trammel nets, artificial reef area was showed 5 species & 16 fishier (2.950g) and natural reef area was showed 4 species & 34 fishies (2.300g). Thus. a catched fishes in the artificial reef area was large size. natural reef area was small size fishes and control area was floating fishes.

        지형공간 특성자료를 이용한 하천유역의 강우-유출해석

        The subject basin of the research was the basin of Yeongcheon Dam located in the upper reaches of the Kumho River. The parameters of the model were derived from the results of abstracting topological properties out of rainfall-runoff observation data about heavy rains and Digital Elevation Modeling(DEM) materials. This research aimed at suggesting the applicability of the CELLMOD Model. a distribution-type model, in interpreting runoff based on the topological properties of a river basin, by carrying out runoff interpretation for heavy rains using the model. To examine the applicability of the model, the calculated peaking characteristics in the hydrograph was analyzed in comparison with observed values and interpretation results by the Clark Model. According to the result of analysis using the CELLMOD Model proposed in the present research for interpreting the rainfall-runoff process, the model reduced the physical uncertainty in the rainfall-runoff process, and consequently, generated improved results in forecasting river runoff. Therefore it was concluded that the algorithm is appropriate for interpreting rainfall-runoff in river basins. However, to enhance accuracy in interpreting rainfall-runoff. it is necessary to supplement heavy rain patterns in subject basins and to subdivide a basin into minor basins for analysis. In addition, it is necessary to apply the model to basins that have sufficient observation data, and to identify the correlation between model parameters and the basin characteristics(channel characteristics).

        연속 퍼콜레이션 모형의 감수율의 진폭비의 보편성

        We study the universality-related problem of continuum percolations. We specifically consider the amplitude ratio of susceptibilities, R=C_-/C_+, which is supposed to be universal but found to be different from the lattice value. We discuss several possible sources of errors in estimating R by computer simulations and present simulation data for penetrable-concentric-shell model, with the finite size effect being carefully taken into account. Results from any finite-sized system were found to be significantly larger than the known lattice values: however, after taking extrapolations to the infinite-system-limit, we obtained R very close to the lattice values for both two and three dimensions. Based upon these, we conclude the strong universality between continuum and lattice percolation models for at least PCS model.

        압축하중을 받는 무리말뚝의 주면지지력 특성

        H-pile can be more easily driven than pipe pile by pile driver and shows high skin friction and plugging effect. And lately It is well known that the high strength H-pile has been widely used for pile foundations. To compare the skin frictions of H piles under different density soil conditions, this paper presents results of a series of model tests on vertically loaded group piles. Model piles made of steel embedded in weathered granite soil were used in this study. Pile arrangements(2 x2, 3 x 3), pile space(2 D, 4 D, 6 D), and soil density(D_r, = 40%, 80%) were tested. The main results obtained from the model test can be summarized as follows. The series of tests found that compression load for group piles increases as number of piles increase and piles space ratic decrease to D_r = 40% of soil density. The analysis also found that the theoretical value of skin friction for group piles is greater than practical value as piles space ratio increases to D_r = 40% of soil density. Piles showed the greatest difference of the skin friction in case that the pile space ratio(S/D) is 6. The theoretical value by Meyerhof and DM-7 showed 1.83 times and 1.32 times respectively as great as practical value in case of S/D = 6 and 2x2
