      • A Role of Autonomous Motivation in the Effects of Family Function and Healthcare Providers Support Affecting Health Behavior in Breast Cancer Survivors

        Aim(s): Health-related behavior is an essential issue to maintain and improve physical and psychological quality of life among cancer survivors. The aims of this study were to investigate the relationships between perceived family function, healthcare providers’ support, and motivation of self-regulation and to examine a role of self-regulatory motivation mediating the effects of family function and healthcare providers’ support on health-promoting behaviors in breast cancer survivors. Method(s): This is a cross-sectional study. A convenience sample of 218 breast cancer survivors (Mage =51.5 yrs., range age = 32~69) was recruited from a University hospital in C city of South Korea. Data were collected using a self-administered questionnaire including Health care Climate, Family APGAR, Treatment Self-Regulation, and Health-Promoting Lifestyle Profile from March 22 to April 23, 2021. The data were analyzed with descriptive statistics and Pearson’s correlation analysis, and a mediation effect was tested using PROCESS macro for SPSS program 26.0. Result(s): Both family function and healthcare providers’ support were significantly correlated with autonomous motivation (r =.244, p <.001 for family function; r =.203, p =.003 for healthcare providers) as well as health-promoting behavior (r =.403, p <.001 for family function; r=.299, p <.001 for healthcare providers). There was a significant correlation between autonomous motivation and health-promoting behavior(r=.574,p<.001). Both family function (β=.097, t=2.91) and healthcare providers’ support (β = .21, t=3.95) were significant predictors for self-regulatory motivation, after controlling for the covariates (i.e., age, periods since mastectomy, education and income levels, and marital status). A significant indirect effect of family function, but not healthcare providers’ support, on health-promoting behavior through self-regulatory motivation (95% CI = .005, .040) was detected, supporting a mediation effect of self-regulatory motivation in the relationship between family function and health-promoting behavior. Conclusion(s): The findings highlight the importance of supportive family function and healthcare providers for improving the autonomous motivation for self-regulation among breast cancer survivors. In particular, the finding demonstrates a critical role of autonomous motivation for self-regulation that mediates the effect of family function on health-promoting behavior. Developing a program focusing on the enhancement of supportive family function as well as autonomy for self-regulation is warranted to improve health-promoting behaviors in breast cancer survivors.

      • The Impact of Autonomy Support in the Relationship between Illness Perception and Health Behavior in Patients with Cardiovascular Risk Factors

        Aim(s): Individuals’ perceptions of their illness and interaction with health care providers in medical encounter could affect various types of health behavior in patients with chronic diseases. This study aimed to examine the impact of autonomy support from healthcare providers in the relationship between illness perception and health behavior in patients with cardiovascular risk factors. Method(s): A cross-sectional study was conducted using a convenience sample of 196 patients with at least one risk factor of cardiovascular diseases (Mage =58.3 yrs). Participants were recruited from three local clinics of two cities of South Korea. The data were collected by self-administered survey questionnaires including Illness Perception Questionnaire, Healthcare Climates, and Health Behavior scales and analyzed using Pearson’s correlation and Process macro bootstrapping approach using SPSS WIN 26.0 program. Result(s): Health behavior was significantly correlated with illness perception about controllability by medical treatment (r = -.157, p =.024) and autonomy support from healthcare providers (r =.323, p < .001). The effects of some dimensions of illness perception (i.e., self-controllability, coherence) on behavioral practice were significantly different by levels of perceived autonomy support from health care providers, indicating an interaction of illness perception regarding self-controllability (β = - .0277, p < .001) and illness perception regarding coherence (β = -.0187, p = .027) with autonomy support in predicting health behavior. Conclusion(s): Autonomy support plays a role as a moderator affecting the relationship between illness perception and health behavior. Although illness perception may be a critical factor affecting health behavior, the influence is associated with autonomy support from healthcare providers. The findings emphasize that healthcare providers should recognize their significance affecting behaviors in patients with cardiovascular risk factors. The efforts for setting autonomy supportive clinical environments needs to be highlighted.

        노인간호에 대한 바람직한 태도형성을 위한 교육방안 고찰

        연구목적: 본 연구는 미국의 학부 간호교육에서 노인간호에 대한 바람직한 태도를 강화하기 위해 시도된 새로운 교육적 접근법을 고찰하고 국내 간호교육에서의 적용방안을 모색하고자 수행되었다. 연구방법: PubMed, CINAHL의 검색엔 진을 사용해 미국 John A. Hartford 재단의 지원으로 노인간호교육을 위한 획기적인 전략개발 프로젝트를 수행한 간호대학 의 연구 8편과 그 외 다양한 교육적 방안을 고찰한 논문 4편을 포함한 12편의 연구를 고찰하였다. 연구결과: 노인간호에 대한 올바른 태도형성을 위해 제시된 교육적 접근법은 다음의 네 가지로 구분할 수 있다: 1) 노인간호에 대한 간호전공교과 간의 연계교육, 2) 지역사회 노인의 교육적 멘토 역할, 3) 시청각 매체의 활용, 4) 자기성찰을 통한 토론학습. 향후 응용: 국내 간호교육에서의 효과적인 적용을 위해서는 연계교육을 위한 간호전공교과목 간의 긴밀한 협의 및 교육과정의 탄력적 운영을 위한 교육행정부서의 협조가 수반되어야 한다. 또한, 교육기관과 지역사회와의 긴밀한 파트너십을 구축함으로써 지 역사회 노인이 교육적 멘토로 참여하는 방안을 적용해 나갈 수 있다. 무엇보다, 노인간호에 대한 바람직한 태도형성을 위한 간호교육의 중요성을 인식하고 교수자의 교육역량강화와 새로운 교수법 개발을 위한 지속적 노력이 요구된다. Purpose: This study reviewed the innovative educational strategies that have been implemented successfully in US baccalaureate nursing education programs to facilitate appropriate attitudes about caring for older adults. Methods: The data were collected through a search of PubMed and CINAHL using the key words, gerontological or geriatric, attitudes about aging or older people, baccalaureate or undergraduate nursing education or curriculum. Results: The successful strategies are categorized as follows: 1) a multidisciplinary approach integrating aging issues and gerontological contents into diverse nursing courses, 2) active participation of community-dwelling healthy older individuals as an educational mentor, 3) use of audiovisual materials to investigate the misconceptions and attitudes about aging, and 4) discussion through critical thinking and self-reflection toward aging. Implications: For the interdisciplinary approach within nursing courses, it is essential to derive the key contents for gerontological nursing applicable to integration into diverse nursing courses. In addition, it is necessary to provide administrative support for implementing innovative strategies and constructing consistent partnerships with the community for active participation of the elderly as a mentor. Lastly, recognizing the significance of educational strategies for enhancing desirable attitudes toward gerontological nursing and supporting the development of educational capability of a faculty are key issues.

        간호학생의 업무기대감과 진로계획의 영향요인

        본 연구는 졸업 직전 간호학생들의 전공만족도, 간호역량성취도, 업무기대감 간의 관련성을 중심으로 진로계획에 미치는 영향을 파악하기 위한 서술적 상관관계 연구이다. 연구자료는 D시와 K시 소재 두 개 대학의 졸업을 앞둔 간호학생 255명이 자가보고식 설문지에 기입하는 방식으로 2016년 1월4일부터 1월7일까지 수집되었다. 자료는 SPSS와 AMOS를 사용해 기술분석, Pearson"s correlation coefficients, t-test, bootstrapping을 활용한 SPSS macro, 경로분석을 실시하였다. 연구결과로, 간호학생들의 전공만족도, 간호역량성취도, 업무기대감은 유의미한 상관관계가 있으며, 전공만족도와 간호역량성취도는 업무기대감에 유의미한 영향을 미쳤다. 또한 롤 모델의 유무에 따라 전공만족도와 간호역량성취도가 업무기대감에 미치는 영향이 달라지는 조절효과가 있었다. 전공만족도는 졸업 후 진로계획에 직접적인 영향은 유의미했으나, 전공만족도가 업무기대감을 거쳐 진로계획에 미치는 간접적인 효과는 유의미하지 않았다. 반면, 간호역량성취도가 졸업 후 진로계획에 미치는 직접적인 영향은 유의미하지 않았으나, 간호역량성취도가 업무기대감에 영향을 미침으로써 진로계획에 영향을 미치는 간접적인 효과는 유의미한 것으로 나타나, 업무기대감이 간호역량성취도와 향후 진로계획 간의 관계에 매개요인으로 작용하는 것으로 나타났다. 본 연구는 간호학생의 졸업 후 진로계획을 위해 전공만족도와 간호역량성취도를 향상시킴으로써 업무기대감을 강화하고, 이를 위해 임상전문가와의 적극적인 상호작용으로 롤 모델을 형성하고, 향후 간호업무 수행을 위한 동기와 목표를 탐색할 수 있는 교육방안이 강화되어야 함을 시사한다. This cross-sectional correlational study investigated factors affecting career planning of nursing students, focusing on the associations between satisfaction of academic major, perceived professional competence, and job expectations. Data from 255 nursing students were collected through self-administered questionnaire surveys on 2016 January 4 and 7 and analyzed using descriptive statistics, Pearson"s correlation coefficients, t-tests, bootstrapping SPSS macro and path analysis using AMOS. There were significant correlations between major satisfaction, professional competence, and job expectations. Both major satisfaction and professional competence were significant factors influencing job expectations, but their effects on job expectations were significantly different depending on whether there was a role-model, which support a moderating effect of having a role-model. Major satisfaction had a significant direct effect on career planning, but its indirect effect on career planning through job expectations was not significant. Professional competence did not have a significant direct effect on career planning, but its indirect effect on career planning through job expectations was significant. Overall, the results highlight the importance of providing educational support and opportunities for nursing students to explore their own motivation and to have a role-model during pursuit of a nursing career.

        대사증후군 위험군의 가족기능과 건강행위의 관계에서 질병인식의 매개효과

        Purpose: This study aimed to examine the mediating effect of illness perception on the relationship between family function and health behavior of patients with risk factors for metabolic syndrome. Methods: This is a cross-sectional correlational study. Data were collected from 160 patients using self-administered questionnaires including the Illness Perception Questionnaire-Revised, Family APGAR, and the Health Behavior Scale. The data were analyzed using Pearson's correlation coefficients, independent t-test and multiple linear regression analysis by the SPSS 23.0 program. Results: Health behavior was significantly correlated with family function (r=.30, p<.001) and illness perception of controllability by treatment (r=-.21, p=.007). Family function was a significant predictor of health behavior and illness perception, and the influence of family function on health behavior was partially mediated by illness perception of controllability by treatment. Conclusion: The findings of this study indicate that family function is a critical factor affecting health behavior and illness perception, and illness perception of controllability by treatment mediates the influence of family function on health behavior. It is necessary to develop a psycho-cognitive intervention program for enhancing supportive family function and for modifying negative illness perceptions to improve health behavior in patients with risk factors for metabolic syndrome.

      • Gender differences in climacteric symptoms and associated factors in Korean men and women

        <P><B>Abstract</B></P> <P>Both men and women may experience multifaceted symptoms that are part of natural aging throughout the climacteric period. This study compared the prevalence and severity of climacteric symptoms between genders and identified the underlying clusters of climacteric symptoms and associated factors in midlife men and women. A cross-sectional study was done with 254 middle-aged Korean men (n = 129, M = 50.4) and women (n = 125, M = 49.5). Data were collected by self-administered surveys and analyzed using t-tests, chi-square tests, exploratory factor analysis, and regression analysis. Significant gender differences in overall climacteric symptoms were not detected except for muscle weakness, weight gain, and hot flashes. Climacteric symptoms were clustered as physical, vasomotor-genital, psychological, and metabolic dimensions, with the physical dimension being the most explanatory cluster. A significant gender effect was found only in the metabolic dimension after adjusting for the relevant covariates, and regular eating was significantly associated with all symptom clusters. This study offers evidence that most climacteric symptoms are shared by both men and women and emphasizes the importance of healthier lifestyles in the climacteric transition period. The findings highlight the critical need for integrated assessments of the multifactorial symptoms and of modifying poor lifestyles in both genders throughout the climacteric transition period.</P> <P><B>Highlights</B></P> <P> <UL> <LI> Both men and women experience multiple symptoms that are part of natural aging throughout the climacteric period. </LI> <LI> This study offers evidence that middle-aged men and women share most climacteric symptoms. </LI> <LI> Careful assessment of climacteric symptoms and lifestyles should be highlighted for healthy aging in both men and women. </LI> </UL> </P>

      • Undergraduate Nursing Students’ Experiences in a Virtual Simulation Program as a Substitute for Clinical Practicum during the COVID-19 Pandemic

        Aim: The COVID-19 pandemic has been a considerable challenge for education and clinical practicum for nursing students, requiring them to transition to virtual education. The purpose of this study was to explore undergraduate nursing students’ learning experiences in a web-based virtual simulation program that replaced clinical practicum during the COVID-19 pandemic. Methods: This study used a qualitative approach with ten senior nursing students who completed a web-based virtual simulation program as a substitute for clinical practicum at a university in South Korea. Data were collected through in-depth interviews and analyzed using theme analysis by Braun and Clarke. Results: Four main themes emerged from the interviews on nursing students’ learning experiences with a web-based virtual simulation program: 1) lacking a sense of reality; 2) indirect experience with various nursing roles; 3) learner-led nursing practicum; and 4) safe and comfortable learning environment. The nursing students reported difficulty handling complex problems with limited information in web-based virtual simulations, which were different than their previous practicum experience in real clinical settings. They also perceived that the program helped them recognize the specialty of nursing professionals and improved their critical thinking based on diverse simulations, which is difficult to learn from an observation-focused clinical practicum. In addition, students recognized the benefits of web-based virtual simulations in promoting clinical competence based on scientific knowledge through self-directed learning and flexible use of time. Conclusions: The findings suggest that a web-based virtual simulation program may be an effective educational strategy that could augment or replace clinical nursing practicum, but there are limitations and weaknesses. This study provides basic data for introducing a new paradigm of practical training.
