        부식제 중독 환자에서 시간에 따른 내시경 합병증 비교

        Purpose: Endoscopy has been recommended as a primary procedure for determining the extent of damage and prognosis in patients with caustic ingestions. Endoscopy within the first 24 hours has been suggested, however, such immediate endoscopy is not always possible. Therefore, we wanted to determine complications and possible delayed sequelae after the endoscopy performed dependent on time, including less than 24 hours and more than 24 hours, after ingestion of relatively high toxic caustic agents. Methods: From January 2005 to May 2013, 105 consecutive patients were diagnosed with caustic poisoning in the emergency department of the Wonju Severance Christian Hospital. Out of 95 patients who underwent endoscopy, while excluding 49 patients who ingested sodium hypochlorite and 15 patients due to insufficient data, 41 patients were ultimately included. We compared general characteristics, complications related to endoscopy, late sequelae, total admission length, and mortality between two groups. Results: Twenty eight patients (68.3%) were diagnosed with acid ingestion. Median endoscopy time was 17.8 (IQR 9.7-36.9) hours and performed in 16 patients (39%) after 24 hours. There were no complications, such as perforation and bleeding in either endoscopy within 24 hours group or endoscopy after 24 hours group. In addition, no difference in ingested materials, endoscopy grade, or late sequelae was observed between endoscopy within 24 hours group and endoscopy after 24 hours group. Conclusion: No difference in complications and late sequelae was observed between endoscopy within 24 hours group and endoscopy after 24 hours group when endoscopy was performed based on a clinician’s assessment.

        <P>Recently, the permanent magnet-assisted synchronous reluctance motor (PMa-SynRM) that can replace induction motor has been more studying because of the needs of the high efficiency motor development for minimum energy performance standard. PMa-SynRM requires the speed and position information of rotor to control motor correctly. However, sensor for rotor information has many problems such as mounting space shortage and the additional system cost. Therefore, in this paper, the speed-sensorless control based on reference model adaptive control is introduced to eliminate the sensor. And mechanical design is improved for control. The proposed method is verified by simulation.</P>

        포스트코로나 시기 놀이치료 인식 변화에 대한 빅데이터 연구

        In this study, we explored public perceptions of play therapy in the news media before and after the occurrence of COVID-19 through big data analysis. To achieve this, we collected news articles containing the keyword ‘play therapy’ for the periods before and after the occurrence of COVID-19. Then we analyzed big data using Textom, extracting key keywords related to play therapy, and examined the relationships between keywords, and topics. As a result, at first, in the post-COVID-19 era compared to the pre-COVID-19 era, ‘school violence’ emerged as a significant word in the category of ‘primary concern’ and ‘recovery’ appeared as a significant word in the category of ‘intervention purpose’. Second, in the pre-COVID-19 era, keywords such as ‘child’, ‘program’, ‘parents’, ‘counseling’, ‘business’, ‘student’, ‘emotion’, ‘family’, ‘psychology’ appeared. Whereas in the post-COVID-19 era, keywords such as ‘child’, ‘program’, ‘counseling’, ‘children’, ‘business’, ‘therapy’, ‘family’, ‘health’, ‘psychological therapy’ and ‘strengthening’ appeared. Third, topics related to play therapy in the pre-COVID-19 era included ‘development and mental health’, ‘child and adolescent counseling’, ‘psychological support and welfare services’, and ‘family support counseling’ as four clusters. In contrast to this, in the post-COVID-19 era, four clusters emerged as ‘infant and toddler development and mental health intervention services’, ‘institutional therapy intervention services’, ‘support for children with disabilities’, and ‘child and adolescent counseling’. This study provided basic data that can be reflected in play therapy by better apprehending and understanding the public’s perception of play therapy, which has increased since the pandemic.

        1970년대 이후 한국 연안의 적조 발생 변화

        국립수산과학원과 지역자치단체의 적조모니터링 결과를 바탕으로 해양환경 변동이 적조발생에 미치는 영향을 규명하였다. 1972년 적조 모니터링이 시작된 이후, 1980년대에서 1990년대까지 적조 발생은 지속적으로 증가를 하였으며, 1998년 109건의 최다 적조발생 이후 2010년대까지 감소 추세를 보이고 있다. 1970년대는 대부분 규조 적조가 발생하였으며, 1980년대에는 연안성 와편모조류가 주로 적조를 일으켰으며, 1993년 이후 Cochlodinium polykrikoides 적조가 지속적으로 발생하고 있다. 우리나라에서 수산피해를 일으킨 유해 적조생물은 3종이다. 1981년 진해만에서 Karenia mikimotoi에 의한 고밀도 적조가 발생하여 패류가 대량 폐사하였다. 1992년 통영해역에서 Karenina sp.에 의한 적조가 발생하여 양식어류를 폐사시켰으며, 1995년 C. polykrikoides 적조로 765억 원의 최대 규모의 수산피해가 발생한 이후 지속적으로 발생하고 있다. 연안해역의 영양염 농도는 1980년대에 가장 높았으며, 1990년 중반 이후 매우 감소하고 있다. 이러한 영양염 감소는 적조발생 감소를 잘 설명해 준다. 2016년 이후 30℃이상의 여름 고수온이 나타나며 C. polykrikoides의 적조 발생 범위와 규모는 매우 감소하였다. 2016년 K. mikimotoi 적조가 전남 장흥~고흥 해역에 발생하였으며, C. polykrikoides 적조는 여수해역에만 발생하였다. 2017년은 C. polykrikoides 적조 발생이 없었으며, Alexandrium affine 적조가 전남 여수~경남 통영해역까지 발생하였다. 2018년은 평년에 비해 소규모 C. polykrikoides 적조가 발생하였다. 본 연구결과 우리나라 연안의 영양염 감소와 기후변화로 인한 고수온은 적조 발생에 영향을 주는 것으로 판단된다. Based on the results of harmful algal blooms (HABs) monitoring by the National Institute of Fisheries Science and local governments, the effects of changes in the marine environment on HABs are described. Since the beginning of HABs monitoring in 1972, they continued to increase from the 1980s to the 1990s. After the largest number of HAB incidents (109) in 1998; the trend declined until the 2010s. Most HABs in the 1970s were caused by diatoms. In the 1980s, coastal dinoflagellates caused HABs; Cochlodinium polykrikoides blooms have been occurring continuously since 1993. There are three HAB species that cause damage to fisheries in Korea. The high-density bloom of Karenia mikimotoi caused mass mortality in shellfish in Jinhae Bay in 1981. Karenia sp. blooms occurring around Tongyeong in 1992 killed aquaculture fish. Since the occurrence of the largest fisheries damage of KRW 76.6 billion in 1995 caused by C. polykrikoides blooms, they have been occurring continuously. The concentration of nutrients in coastal waters was the highest in the 1980s and has declined since the mid-1990s. This reduction in nutrient concentration is a good explanation for the decreasing number of HABs. Since 2016, a summer high water temperature of 30℃ or more has appeared, and the range and scale of C. polykrikoides blooms have been greatly reduced. In 2016, K. mikimotoi blooms occurred around Wando, Jangheung and Goheung and small scale blooms of C. polykrikoides occurred around Yeosu. There were no C. polykrikoides blooms in 2017; however, Alexandrium affine blooms occurred from Yeosu to Tongyeong. There was a small-scale blooms of C. polykrikoides in 2018 compared to those in the previous years. Our results show that reduction in nutrients and the high water temperature owing to climate change are a good explanation for variation in HABs in Korean coastal waters.

      • Hepatoprotective Effect of Gallotannin-enriched Extract Isolated from Gall on Hydrogen Peroxide-induced Cytotoxicity in HepG2 Cells

        <P><B>Background:</B></P><P>Gall (Galla Rhois [GR]) is known to have antibacterial, anti-inflammatory, antimetastatic, and anti-invasion activities and exert hepatoprotective effects. However, the hepatoprotective effects of gallotannin-enriched GR (GEGR) and their mechanisms have not yet been investigated.</P><P><B>Objective:</B></P><P>The potential protective effect of GEGR against hepatotoxicity induced by hydrogen peroxide (H<SUB>2</SUB>O<SUB>2</SUB>) was investigated.</P><P><B>Materials and Methods:</B></P><P>Changes in cell viability, apoptosis protein expression, and reactive oxygen species (ROS) generation were determined in HepG2 cells that were pretreated with four different concentrations of GEGR (6.25–50 μg/ml) for 24 h before H<SUB>2</SUB>O<SUB>2</SUB> exposure.</P><P><B>Results:</B></P><P>GEGR consisted of gallotannin (69.2%), gallic acid (26.6%), and methyl gallate (4.2%) and showed remarkable 2,2-diphenyl-1-picrylhydrazyl scavenging activity (inhibitory concentration 50% = 0.212 μg/ml). The lethal dose 50% and effective dose 50% values for the response of HepG2 cells to GEGR were determined to be 178 and 6.85 μg/ml, respectively. Significant reductions in the immunofluorescence intensity indicating apoptosis were also detected in the nuclei of HepG2 cells stained with 4’,6-diamidino-2-phenylindole and Annexin V after GEGR treatment. The Bax/Bcl-2 ratio and active caspase-3 level were higher in H<SUB>2</SUB>O<SUB>2</SUB> + vehicle-treated cells. However, these levels gradually decreased to those of the No-treated group in the GEGR pretreated group even though little or no decrease was observed in response to low GEGR concentrations. Furthermore, the GEGR pretreated group showed a reduced level of 2’,7’-dichlorofluorescein diacetate stained cells, indicating ROS generation relative to the H<SUB>2</SUB>O<SUB>2</SUB> + vehicle-treated group.</P><P><B>Conclusion:</B></P><P>The results of this study provide strong evidence that GEGR can prevent cell death induced by H<SUB>2</SUB>O<SUB>2</SUB> in HepG2 cells through the induction of antioxidant conditions.</P><P><B>SUMMARY</B></P><P><P>The gallotannin (69.2%), gallic acid (26.6%), and methyl gallate (4.2%) are the main constituents of water extracts of GR</P><P>GEGR was more potent in DPPH scavenging, and gallotannin contributes to this extract activity</P><P>GEGR significantly reduced the increase of apoptosis, Bax/Bcl-2 ratio, and active caspase-3 level after H2O2 treatment</P><P>GEGR pretreatment showed protection against H<SUB>2</SUB>O<SUB>2</SUB>-induced ROS production in DCFH-DA staining analysis.</P></P> >[FIG OMISSION]</BR><P><B>Abbreviations used:</B> COX: Cyclooxygenase; DAPI: 4’,6-diamidino-2-phenylindole; DMSO: Dimethyl sulfoxide; DPPH: 2,2-diphenyl-1-picrylhydrazyl; GEGR: Gallotannin-enriched Galla Rhois; GR: Galla Rhois; HPLC: High-performance liquid chromatography; H<SUB>2</SUB>O<SUB>2</SUB>: Hydrogen peroxide; MMP: Metallopeptidase; MTT: 3-(4,5 dimethylthiazol-2-yl)-2,5-diphenyl tetrazolium bromide; ROS: Reactive oxygen species; UV-Vis: Ultraviolet-visible.</P>

        Neuroprotective Effects of a Butanol Fraction of Rosa hybrida Petals in a Middle Cerebral Artery Occlusion Model

        The neuroprotective effects of a butanol fraction of white rose petal extract (WRPE-BF) were investigated in a middle cerebral artery occlusion (MCAO) model. Seven week-old male rats were orally administered WRPE-BF for 2 weeks and subjected to MCAO for 2 h, followed by reperfusion. Twenty-four h later, MCAO-induced behavioral dysfunctions were markedly improved in a dose-dependent manner by pretreatment with WRPE-BF. Moreover, higher dose of WRPE-BF not only decreased infarction area but also effectively reduced astrogliosis. The expression of inducible nitric oxide synthase, cyclooxygenase-2, and glial fi brillary acidic protein in MCAO model were markedly inhibited by WRPE-BF treatment. Notably, WRPE-BF decreased nitric oxide and malondialdehyde levels in the striatum and subventricular zone of stroke-challenged brains. These data suggested that WRPE-BF may exert its neuroprotective effects via anti-oxidative and anti-infl ammatory activities against ischemia-reperfusion brain injury and could be a good candidate as a therapeutic target for ischemic stroke.
