최근 열차운행에 대한 효율화와 안전성, 그리고 열차운행 시스템에 대한 국내기술 확보를 위해 많은 연구개발사업을 수행하고 있다. 하지만 국내철도 신호시스템은 사용실적이 있는 고가의 해외 시스템을 도입하고 있기 때문에 해외 의존도가 높아짐에 따라 국내기술의 경쟁력 약화, 사업비 증가가 우려되는 실정이다. 그래서 해외 의존도를 최소화 하기 위해 열차제어시스템의 가장 기본적인 열차위치검지기능 시스템인 하이브리드 궤도회로 시스템을 개발 중이다. 본 논문에서는 하이브리드 궤도회로 시스템 실용화를 위해 국내·외 철도신호시스템 구축현황을 살펴보고, 향후 하이브리드 궤도회로 시스템의 적용성을 검토해보고자 한다. A lot of research recently has done for efficiency and reliability of the train operation, and securing domestic technology of train operation system. However, as increase dependence level of foreign system which is expensive, weakening the competitiveness of domestic technology and increasing the operation cost are concerned. Thus hybrid track circuit system, basic position detection system of train, is being developed for minimizing foreign system dependence. With this paper, we inspect current situation of the train signal system and examine applicability of the hybrid track circuit system from now on.
3차 병원에서 시행된 제왕절개술의 예방적 항생제 사용에 대한 전향적 다기관 연구

Objective: To determine whether the duration and timing of prophylactic antibiotics influence maternal postoperative infectious morbidity in cesarean section performed at tertiary center. Methods: This study was a prospective, randomized trial. Pregnant women who underwent cesarean section between December 2008 and September 2009 at tertiary center were enrolled and divided into two groups: Group A, antibiotic prophylaxis was applied for 24 hours and Group B, antibiotic prophylaxis was applied for 48 hours. First generation of cephalosporin was administrated within 30 minutes prior skin incision or after cord clamping. The occurrence of postoperative infectious morbidity such as febrile morbidity, wound infection, endometritis, urinary track infection, pneumonia, sepsis and pelvic abscess and hospital stays were compared. Results: There were 413 pregnant women enrolled and then randomized into 220 for group A and 197 for group B. No demographic differences were observed between two groups. The infectious morbidity was 1.9% (8/413) and wound infection was the most common postoperative infections morbidity. No significant difference was found between the groups for infectious morbidity and hospital stays. Also timing of prophylactic antibiotics did not result in significant difference for infectious morbidity. Conclusion: Short course of prophylactic antibiotics has been shown to be as efficacious as multidose of prophylactic antibiotics for preventing infectious morbidity in cesarean section and timing did not influence on infections morbidity. Further studies focusing on duration and timing of prophylactic antibiotics for cesarean section are needed.

Objective: Adrenomedullin (AM), a potent vasorelaxing agent associated with the maintenance of pregnancy, is synthesized in response to inflammation, which is also associated with the biosynthesis of prostaglandin (PG) and nitric oxide (NO). To clarify the interrelationships of PG, NO and AM in the inflammatory process, we tested the effects of the PG synthase inhibitor indomethacin and the NO synthase inhibitor Nω-nitro-L-arginine methyl ester (L-NAME) on AM production in lipopolysaccharide (LPS)-stimulated cultured rat macrophages. Methods: RAW 257.8, rat macrophages were incubated with LPS, and AM production was measured by ELISA. Cells were pretreated with indomethacin, L-NAME, or both, and the effect on LPS-induced AM production was assayed. To exclude the effect of cell death, a cell viability test on these cultures was performed. Results: The largest increase of AM was seen between 1 μg (36.33±2.05 pg/ml) and 10 μg (89.33±6.02 pg/ml) of LPS concentration (p<0.01), making the latter the optimal LPS dose to stimulate AM production. AM secretion was proportional to time in culture (p<0.006). Addition of indomethacin, L-NAME, or both 1 hr before LPS stimulation decreased AM production 2 hr later, with the AM decrement greatest in cells pretreated with both indomethacin and L-NAME, followed by L-NAME alone and then indomethacin. Conclusion: These findings indicate that PG and NO increase AM synthesis in rat macrophages with the presence of LPS. These results suggest that the biosynthetic pathways of PG, NO, and AM may be linked.

Objective: Large Loop Excision of the Transformation Zone (LLETZ) has been widely used for evaluating cervical dysplasia/cancer. We performed this study to evaluate if colposcopically suspected cervical high-grade dysplasia or cancer could be directly evaluated and treated with LLETZ. Methods: 93 women who were scheduled for colposcopic evaluation because of an abnormal cervical cytology report were included in the study in Sanggye Paik Hospital from Jan to Dec 2001. Mean age of the study population was 38.6±9.0. colposcopy directed punch biopsy was not performed and histologic diagnosis was made by LLETZ. Sensitivity and specificity of colposcopic evaluation was calculated and compared with those of cervical cytology. Results: On histopathological examination by LLETZ, 11(11.8%) patients had invasive cancers including 3(3.2%) adenocarcinomas. The sensitivity and specificity of colposcopy for the detection of cervical dysplasia or cancer was 71.7% and 72.3% respectively, wile those of cytology was 56.5% and 74.5% respectively. Conclusion: LLETZ may be adequately performed for the diagnosis and treatment of cervical dysplasia/cancer based on the results of pap smear and colposcopy without punch biopsy. Cost benefit might be anticipated while cost-effectiveness analysis is necessary in the future.

In order to investigate the possibility of the microbial degradation of the persistent pollutant 3, 3', 4, 4'-tetrachloroazobenzene (TCAB) in our environment, four strains of microorganisms were isolated from industrial wastes by the enrichment technique. They were identified as Achromobacter group VD, Pseudomonas alcaligenes, Moraxella spp., and Alcaligenes faecalis, respectively. These microorganisms utilized TCAB as a sole carbon source in the MM₂ salt medium.

When [U-^(14)C] 3. 3', 4, 4'-tetrachloroazobenzene([U-^(14)C] TCAB) was added to the MM₂medium as a sole carbon source for the isolated microorganisms and incubated, some radioactive metabolites were detected by autoradiography. No ^(14)CO₂ was evolved from [U-^(14)C] TCAB which was added as a sole carbon source to an organic matter-free soil inoculated by the isolates, wetted with the MM₂ salt medium, and incubated at 30℃. One of the metabolites in pure culture of Achromobacter group VD, which was isolated and identified, was tentatively identified as a compound of m/z 250 by means of GC/MS. The possible pathways for its formation are thought to include dechlorination from the TCAB structure, hydroxylation, ortho fission of the two benzene rings, and reduction of the resulting carboxyl group.
