The presence of septa in the region of the sinus floor can cause complications during sinus lift procedures; accordingly, it is necessary to identify the sinus septa prior to the operative procedure. Various treatment options have been described for management of septa. This study described three cases of lateral window approach for maxillary sinus bone graft in the presence of sinus septa. Panoramic radiograph and dental cone-beam computed tomographic scans were obtained preoperatively. In the first and third case, two bony windows were created after infracture, while in one case the septal bone was cut off and in the other it was left intact. In the second case, one bony window was prepared and the sharp tip of the septum was reduced. After treatment, all of the cases achieved sufficient bone height and no membrane perforation occurred. Modification of the basic sinus lateral approach is a predictable technique to increase bone volume of the edentulous posterior maxilla; therefore, it can be used for placement of dental implants.
성견에서 SLA®와 Joy 1 implant®의 골-임플란트 계면의 조직형태계측학적 연구

Purpose: The objective of this study was to compare the osseointegration of SLA® to these of Joy 1 implant®. Materials and Methods: In 6 beagle dogs, 3rd and 4th mandibular premolar were extracted bilaterally. After 3 months, four transmucosal screw-shaped implants were placed in each mandibular edentulous premolar region. Control groups were SLA® and experimental groups were Joy 1 implant®. The implants were allowed to heal for 4 or 8 weeks. At the end of the experiment, the dogs were sacrificed and each of implant sites was dissected and processed for histomorphometric analysis. Results: Histomorphometric analysis showed direct osseous integration for both implants. No significant difference between both types of implants could be detected after 4 and 8 weeks healing periods in bone to implant contact (BIC) and bone ingrowth (BI). Conclusion: The results indicate that there was no difference in osseointegration between SLA® implant and Joy 1 implant® regarding BIC and BI, Joy 1 implant® showed similar histological response with SLA® implant. The significance of keratinized mucosa in implant maintenance is a controversial issue. As such, there is no consensus regarding the relationship between the width of keratinized mucosa and the health of peri-implant tissue. However, clinicians prefer to provide enough keratinized mucosa around implants for long-term implant maintenance and stabilization of the gingival margin position. The purpose of this case report is the clinical evaluation of an apically positioned flap with a porcine collagen matrix to provide keratinized mucosa and deepen the vestibule around implants during second stage implant surgery. In this case report, a clinically relevant amount of keratinized mucosa was achieved around implants and the vestibule was deepened. Thus, an apically positioned flap with a porcine collagen matrix is an available technique for providing keratinized mucosa and deepening the vestibule around implants during second stage implant surgery. This technique could be potentially used in routine autogenous or another graft procedures to provide keratinized mucosa around implants.

Connective tissue grafts (CTGs) are generally viewed as the most successful technique for root coverage. The aim of this case report was to describe the treatment and follow-up outcome of two complicated cases of gingival recession in the maxillary and mandibular anterior teeth using subepithelial CTGs. The operation was performed by the following steps: recipient site formation, removal of grafts from the donor site, and graft placement and immobilization. The grafted connective tissue was submerged by the envelope, modified double papilla flap, and coronally advanced flap techniques. There were no postoperative complications. Soft tissue volumes increased, and gingival biotypes were acquired. Improved esthetics and oral hygiene were obtained. Root coverage using subepithelial CTGs is considered as the proper treatment choice, but it requires long-term observations due to additional coverage by creeping movement or deterioration by relapse.
