Purpose: The present study was carried out to determine if Paljeong-san extract (PJS) treatment exerts beneficial effect against the glycerol-induced acute renal failure in rabbits. Material and Method: PJS was selected in the basis of invigorating kidney which can eliminate pathogens. Rabbits were treated with PJS pharmacopuncture on Shin-shu ($BL_{23}$) point for 5 days right after the injection of 50% concentration of glycerol ($5m{\ell}/kg$ body weight). Results and Conclusions: Glycerol injection caused an increase in serum creatinine and BUN level and urine glucose secretion, which were accompanied by a reduction in GFR. PJS Pharmacopuncture treatment combined with peritoneal injection showed beneficial effect on glycerol-induced acute renal failure by inhibition of serum creatinine increase and GFR decrease.

Objective : This study was designed to investigate the anti-oxidative potentials to wide the usage of Morus alba L. derived from medicinal herbs on cosmoceutical fields. Methods : Anti-oxidative potentials were investigated by using several kinds of assays including electron donating ability, and the total contents of phenolic compounds were also measured. In some cases TLC (Thin Layer Chromatography) method were used to identify the patterns of phenolics in herbal extracts. Results : Aurones which are sub-types of flavonoids were observed in methanol extract of Mori Cortex Radidus, and total phenolic contents in Mori Cortex Radidus were more than in methanol extracts of Mori Folium and Mori Ramulus. Various kinds of phenolic compounds were observed in chloroform fraction of Mori Cortex Radidus. Conclusion : According to these results, the chloroform fraction of Mori Cortex Radidus could be the most possible resource that has potentials on anti-oxidant agents on cosmoceutical fields.

Objectives: We review the statistical considerations for superiority trial to design appropriate statistical hypothesis and decision-making. Methods: We plan superiority design especially for placebo-controlled trial or combination drug trial. Many previous studies employed two-sided hypothesis testing and misuse of significance level, and this resulted in wrong sample size and confidence interval. Therefore we compared sample size estimation and confidence interval according to one-sided and two-sided hypotheses, Results: There are differences in sample size estimation and confidence interval according to one-sided or two-sided hypotheses with the pre-determined power at the significance level, Conclusions: For superiority trial in the confirmatory stage, one-sided hypothesis is advisable, We can estimate a sample size from this hypothesis and recommend to use a confidence interval for decision-making.

Glenditsia spina (GS) can resolve carbuncle, relive swelling, dispel wind and destroy parasites. For these reasons, GS has been widely used as dermatologic agent clinically. In this study, the specific pathways of anti-proliferative effect of GS on human derived melanoma cells were identified. The molecular profile was measured using microarray technique to identify up- or down-regulated genes in SK-MEL-2 cell line. Pathway analysis was done by listing percentage of pathway involvement, and the represented pathways were obtained from KEGG. The transcription factor binding sequences were obtained by Transfac database. By the promoter analysis, up-regulated genes by GS were mainly associated with MAPK, Regulation of actin cytoskeleton, Wnt, Focal adhesion and Long term potentiation pathway. Down-regulated genes by GS were mainly associated with MAPK and Antigen processing and presentation pathway. And some of the transcription factors like Sp1 and NF-Y in up-regulated genes and Oct-1 in down-regulated genes by GS also identified.

Objectives : The source is from the flower of Carthamus tinctorius L., family Compositae. It is used in clinical medicine to promote blood circulation, remove blood stasis, promote menstruation and alleviate pain. In the present study, we investigated the genome wide analysis of Carthami Flos on the intra-cranial hemorrhage(ICH) model. Methods : ICH in rat was induced by injection of collagenase type IV and Carthami Flos extract(CFe) was administered orally. The molecular profile of cerebral hemorrhage in rat brain tissue was measured using microarray technique to identify up- or down- regulated genes in brain tissue. Results : Expression profile showed that diverse genes were up- or down-regulated by ICH induction. Administration of CFe restored the expression level of some of altered genes by ICH to normal expressional level. Interestingly, these recovered genes by CFe were involved in the same biological pathways which were significantly activated or suppressed by ICH. Conclusion : The above results might explain the therapeutic mechanism of CFe on ICH. Further, by analyzing interaction network, core genes was identified which could be key molecular targets of CFe against ICH.

Statistics are essential for clinical trials, and it is very important to have an accurate understanding of the purpose of the study and to apply the appropriate statistical design or methodology. Prior to conducting a clinical trial, the research protocol requires approval from the IRB (Institutional Review Board) or regulatory agency approval, and contains a variety of contents. The research protocol conducts clinical trials according to GCP (Good Clinical Practice) and describe clinical design and statistical methodologies for demonstrate the efficacy and safety of new drug or new medical devices using the obtained data in clinical trials. At this time, statistical design is applied for the protocol based on the primary study objective. The primary study objective is to influence the selection of primary endpoint to evaluate this, the sample size, the technique of minimizing the bias, the statistical analysis and method of testing the statistical hypothesis. However, statistical errors and contradictions are easy to find in the study protocol and the same errors are repeated many times, so in this paper we aim to avoid these errors in clinical studies and apply them correctly in clinical trials, intended to be helpful in derives scientific design, validity, and reliable results.
허혈로 유발된 토끼의 급성 신부전에 대한 甘草(감초) 약침액 정맥 주입의 영향

Objectives: The present study was undergone to determine whether Glycyrrhizae Radix pharmacopuncture intravenous injection exerts beneficial effect against the ischemia-induced acute renal failure in rabbits. Methods: Rabbits were treated with Glycyrrhizae Radix pharmacopuncture via i.v., followed by renal ischemia/reperfusion. The fractional excretion of glucose and phosphate were measured and the malondialdehyde content was also determined. The morphological changes of cortical part of kidney also observed with light microscope. Results: Renal ischemia/reperfusion caused increase of the fractional excretion of glucose and phosphate in ischemia-induced animals, which was prevented by Radix Glycyrrhizae extract treatment. Ischemia/reperfusion increased lipid peroxidation, which was prevented and morphological changes also altered by Radix Glycyrrhizae pharmacopuncture administration. Conclusions: These results indicate that lipid peroxidation plays a critical role in ischemia-induced acute renal failure and Glycyrrhizae Radix pharmacopuncture exerts the protective effect against acute renal failure induced by renal ischemia/reperfusion.

Objectives : Gleditsiae Spina has the effects of expelling toxins, draining pus, invigorating blood and resolving abscesses. Some clinicians apply the herb for patients suffering from cancer. However, its anti-cancer activities are not well understood. In the present study, anti-tumor agents from Gleditsiae Spina were purified. Methods : The viability of the PC-3 cell line was determined using MTT assay, and the induction of apoptosis by Gleditsiae Spina extract in PC-3 cells was measured by Annexin-V/propidium iodide double staining assay detected by flow cytometry. TLC and HPLC analysis were used to separate and identify the anti-cancer agents. Results : Treatment of the extract resulted in significant decreased cell viability of PC-3 cells in a dose- and time-dependent manner. Dose-dependent apoptotic cell death was also measured by flow cytometry analysis. The anti-cancer agents were successfully separated and identified by using TLC and HPLC analysis and the most potential agent among them was separated from EtOAC fraction. Conclusions : These results might be applied in developing new drugs from natural resources like Korean traditional medicine, and also support the clinical usefulness of herbal medicine.

The aim of the study was to investigate the efficacy of Boiogito for obesity. We examined the efficacy of Boiogito for obese patients and we expected the reaction of Boiogito would vary according to the single nucleotide polymorphism(SNPs). Methods 111 subjects(body mass index≥25m/kg2) were recruited and randomized to receive Boiogito(n=55) or Placebo(n=56) for 8weeks. Anthropometric factors, serum lipid profile, glucose, blood pressure(BP), pulse rate, resting metabolic rate and Korean version of obesity-related quality of life(KOQOL) scale measured at baseline and 8weeks. SNPs(β3-adrenergic receptor(ADRB3), G protein β3(GNB3), peroxisome proliferator activated receptor gamma 2 gene(PPAR-γ2), uncoupling protein(UCP2)) were conducted at baseline. Adverse reactions and safety outcome variables were also checked during trials. Results Both groups showed significant improvement on obesity after treatment. Boiogito group decreased triglyceride than did control group and improved KOQOL. Boiogito showed a significant higher efficacy in C/T and T/T genotype of GNB3 gene / in Trp64 and Arg64 genotype of ADRB3 gene / in D/D genotype of UCP2 gene / in Pro/Pro genotype of PPAR-γ gene. Conclusions Boiogito promoted obesity indexes without severe adverse reactions and proved its safety. Pharmacogenetical studies of Boiogito on obesity could be a effective method for the individualized treatment and prevention of obesity.
