체외충격파쇄석술후 요중 신세뇨관 효소분비의 변화에 관한 연구

Urinary enzyme testing has been used by many investigators to diagnose and monitor various types of renal injury caused by extracorporeal shock wave lithotripsy. Two urinary enzymes, N-acetyl-beta-glucosaminidase (NAG), B_2-microglobulin, were monitored in 22 patients 1 day before and 1 and 3 days after a single unilateral extracorporeal shock wave lithotripsy for treatment of renal stone. All patients had non-obstructed and infection-free, previously untreated renal stones. Extracorporeal shock wave lithotripsy was performed on the EDAP-LTOl + piezoerectric lithotriptor. Urine specimens were collected before extracorporeal shock wave lithotripsy, 1 and 3 days after treatment. Two enzyme levels were elevated 1 day after treatment and decreased to pre-procedure levels at 3 days after, with the increases being statistically non-significant. These findings indicate that there is a transient, nonsignificant renal tubular damage after extracorporeal shock wave lithotripsy.
흰쥐의 요도 손상후 협착 형성에 있어서 TGF-β_1의 역할

Transforming growth factor-β_1 (TGF-β_1)is a homodimeric peptide of 25kDa involved in many fundamental biological processes including cell growth, extracellular matrix deposition and degradation, and inflammatory responses. Especially TGF-β_1 is released by platelet and inflammatory cells, and it affects all phases of the wound healing after injury. It contributes to the regulation of fibroblast chemotaxis and proliferation, and also controls the synthesis and degradation of extracellular matrix necessary for tissue repair. Clinically, scar tissue formation and subsequent stricture after urethral injury often result in troublesome problems to urologists. We intended to know the involvement of TGF-β_1 in the formation of stricture and histological changes after urethral injury of rats. We injured urethrae of 24 adult male Sprauge-Dawley rats(200-250g) by urethrotome and Dormia basket, and then observed histological changes and analysed TGF-β_1 mRNA levels by Northern blot. Northern blot analysis showed that TGF-β_1 mRNA was expressed for 1,3,and 5 days after injury. Fibroblasts and deposition of extracellular matrix were markedly increased on day 5. Reepithelialization was completed and urethral lumen was narrowed on day 10. Our results indicate that TGF-β_1 is involved in the formation of stricture after urethral injury and plays a critical role in early phase of tissue repair and fibrosis.
EDAP LT-01+ 체외충격파쇄석기를 이용한 요로결석 치험 1,237례

Between February 8, 1990 and February 28, 1993, 1,237 renal units in 1,081 patients with urinary stones were treated by extracorporeal shock wave lithotripsy(ESWL) with the EDAP LT-01+lithotriptor. The results were obtained as follows ; 1. The average patient age was 44.3 years, with a range of 16 months to 92 years. The sex ratio was about 1.46 : 1,641(59.3%) male and 440(40.7%) female patients. 2. The locations of stones were kidney in 564 cases(45.6%) including 35 cases of staghorn calculi, ureter in 665 cases(53.8%) and bladder in 8 cases(0.6%). 3. The average number of treatments was 3.5 sessions and average treatment time was 36.5 minutes. 4. Of the 1,237 cases, 105 cases(8.5%) complained of severe pain and requires intravenous or intramuscular analgesics. 5. The success rate of treatment-ended patients(including particles less than 4㎜ in size) was 95.0%. 6. Additional measures used were : push up procedures in 30 cases and double-J stenting in 54 cases. 7. The post-ESWL complications were persistent gross hematuria lasting more than three days in 22 cases, high fever(>38℃) in 26 cases and steinstrasse in 51 cases. We conclude that extracorporeal shock wave lithotripsy with EDAP LT-01+lithotriptor is a safe and effective procedure for the treatment of urinary stones.
