The ectopic infection of lung fluke is a rather common occurrence in Korea, but the etiology in the majority of instances has remained a matter of conjecture and little information is available on this problem. The present investigation was undertaken to secure the etiological factor, course of migration and pathogenesis of lesion in the extra-pulmonary paragonimiasis. Based on the report by Beaver in 1953 that Visceral Larva Migrans is caused by the invasion of nematodes. not adaptable to human, both favorable (cats, dogs) and unfavorable hosts (rats, rabbits, guinea pigs) were infected with the metacercariae of P westermani in present experiment. In addition, young larvae after excysted in Tyrode's solution and adult worms removed from the cysts of definitive hosts were respectively transplanted into the loose connective tissue of neck region, gluteus muscle tissue, peritoneal cavity, cranial cavity or common carotid artery by mean of surgical operation. Thus treated animals were sacrificed at the different periods up to 10 months, and the location, fate of worms and the pathological changes were observed. 1. It was found, in favorable hosts, either infected with metacercariae or transplanted with larvae or adults, the majority of worms migrated to the lung and formed worm-cysts. A few worms were found in the ectopic location in the case of younger hosts and in the massive infection. 2. When the unfavorable hosts were given metacercariae orally or young larvae surgically, the worm did not develope successfully and form the ectopic lesion not-infrequently. The developed worms, when transfered into non-suitable hosts, some made their way to the lung and some produced ectopic infection. 3. It was observed that young larvae actively migrate not only through, the loose connective tissue or muscular tissue but also through the blood stream. 4. Adult worms normally grown for 1-10 months in the previous hosts also migrated through the tissue and appeared in the lung or other location either in proper or non-proper hosts. 5. When transferred the adult worms into proper hosts, the characteristic pathological and parasitological changes of ectopic infection were observed in a few cases. Typical findings of cerebral pargonimiasis could be produced in one dog by the intracranial transplantation of worms. 6. Some larvae or worms transplanted into the non-proper hosts were degenerated or followed by destruction, necrosis or liquefaction and only eggs were abundantly found in the lesions in some cases. 7. Thus, it was thought, the ectopic infection usually occur in the unfavorable hosts, so human might be less suitable host comparing to dogs or cats, or it possibly may occur by the infection of non-adaptable species different from P. westermani. It is of special interest that both young larvae and even adult worms migrate through various tissues or by blood stream. The possibility of the action of certain enzymes secreted by the fluke on ectopic infection in addition to the mechanical invasion was briefly discussed, and further histochemical study might give helpful information on this problem.

A local anesthetic agent which is most commonly used for outpatients is lidocaine hydrochloride that contains epinephrine, which is for vasoconstriction in 1:100,000 concentration. This agent is known as a safe local anesthetic agent and has been used widely for topical use or injections. However, the allergic reaction that we will report in this case occurred when common local anesthesia was done intraorally, and the patient complained of hyperventilation, tachycardia, abdominal pain and unintentional tears. We experienced an allergic reaction after injecting the lidocaine hydrochloride and therefore report the case to suggest that local anesthesia should be always carried out very carefully.
