Objective: To evaluate the etiology, clinical and electrodiagnostic manifestation of winged scapula in Korean soldiers. Method: Reports of conscripts with winged scapula were reviewed retrospectively on details associated with the condition onset, symptoms, signs, and electrodiagnostic study results. Results: We identified 36 patients with winged scapula. The causal events were backpack carriage in 10, closed trauma in 10, surgical trauma in 4, osteochondroma in 1, and idiopathic in 11 patients. The accessory nerves were affected in 16, the long thoracic nerves in 12, the upper brachial plexus in 5, and the dorsal scapula nerve in 3 patients. Poor outcome was rated as 14 (37.8%) and good outcome rated as 20 (54.1%) patients. Conclusion: The winged scapula was most commonly caused by nerve compression associated with backpack carriage in Korean soldiers. Careful physical examination for winged scapula should be performed in patients with shoulder problems associated with backpack carriage.
