A Case of Sarcoidosis That Improved upon Discontinuation of Etanercept

A 31-year-old man who had been prescribed etanercept over a 3-year period for treatment of ankylosing spondylitis presented with newly developed dry cough, chills, myalgia, and weight loss. Chest computed tomography showed multiple reticulonodular pulmonary infiltrates and bilateral mediastinal, hilar, and peribronchial lymphadenopathy. Biopsy of a paratracheal lymph node revealed chronic granulomatous inflammation without necrosis, and the serum angiotensin-converting enzyme level was elevated. Sarcoidosis was diagnosed. His laboratory and radiological findings, and clinical symptoms improved only after discontinuation of etanercept without treatment. Although etanercept-induced sarcoidosis is rare, this case report suggests that sarcoidosis should be considered in the differential diagnosis of patients treated with the tumor necrosis factor inhibitor. Alkaloids are the most extensively featured compounds of natural anti-tumor herbs, which have attracted much attention in pharmaceutical research. In our previous studies, a mixture of major three alkaloid components (5, 6-dihydrobicolorine, 7-deoxy-trans-dihydronarciclasine, littoraline) from Hymenocallis littoralis were extracted, analyzed and designated as AHL. In this paper, AHL extracts were added to human liver hepatocellular cells HepG-2, human gastric cancer cell SGC-7901, human breast adenocarcinoma cell MCF-7 and human umbilical vein endothelial cell EVC-304, to screen one or more AHL-sensitive tumor cell. Among these cells, HepG-2 was the most sensitive to AHL treatment, a very low dose ($0.8{\mu}g/ml$) significantly inhibiting proliferation. The non-tumor cell EVC-304, however, was not apparently affected. Effect of AHL on HepG-2 cells was then explored. We found that the AHL could cause HepG-2 cycle arrest at G2/M checkpoint, induce apoptosis, and interrupt polymerization of microtubules. In addition, expression of two cell cycle-regulated proteins, CyclinB1 and CDK1, was up-regulated upon AHL treatment. Up-regulation of the Fas, Fas ligand, Caspase-8 and Caspase-3 was observed as well, which might imply roles for the Fas/FsaL signaling pathway in the AHL-induced apoptosis of HepG-2 cells.

Hepatocellular carcinoma (HCC) has a relatively higher incidence in many countries of Asia. Globally, HCC has a high fatality rate and short survival. Epirubicin, a doxorubicin analogue, may be administered alone or in combination with other agents to treat primary liver cancer and metastatic diseases. However, the toxic effects of epirubicin to normal tissues and cells have been one of the major obstacles to successful cancer chemotherapy. Here, we investigated the effects of epirubicin in combination with kappa-selenocarrageenan on mice with H22 implanted tumors and HepG-2 cell proliferation, immune organ index, morphology, cell cycle and related protein expressions in vivo and in vitro with sequential drug exposure. The inhibitory rate of tumor growth in vivo was calculated. Drug sensitivity was measured by MTT assay, and the King's principle was used to evaluate the interaction of drug combination. Morphological changes were observed by fluorescent microscopy. Cell cycle changes were analyzed by flow cytometry. Expression of cyclin A, Cdc25A and Cdk2 were detected by Western blotting. In vivo results demonstrated that the inhibitory rate of EPI combined with KSC was higher than that of KSC or EPI alone, and the Q value indicated an additive effect. In addition, KSC could significantly raise the thymus and spleen indices of mice with H22 implanted tumors. In the drug sensitivity assay in vitro, exposure to KSC and EPI simultaneously was more effective than exposure sequentially in HepG-2 cells, while exposure to KSC prior to EPI was more effective than exposure to EPI prior to KSC. Q values showed an additive effect in the simultaneous group and antagonistic effects in the sequential groups. Morphological analysis showed similar results to the drug sensitivity assay. Cell cycle analysis revealed that exposure to KSC or EPI alone arrested the cells in S phase in HepG-2 cells, exposure to KSC and EPI simultaneously caused accumulation in the S phase, an effect caused by either KSC or EPI. Expression of cyclin A, Cdc25A and Cdk2 protein was down-regulated following exposure to KSC and EPI alone or in combination, exposure to KSC and EPI simultaneously resulting in the lowest values. Taken together, our findings suggest that KSC in combination with EPI might have potential as a new therapeutic regimen against HCC.

The aim of this study was to explore the angiographic diagnosis and embolization therapy for renal artery pseudoaneurysms due to acute urinary tract hemorrhage after conservative medical management failed. Seven out of ten cases had fever symptoms after the kidney surgery. The pseudoaneurysms were treated with gelatin sponge and (or) spring coil and the majority demonstrated rapid blockage of hemorrhage. Angiography diagnosis and trans catheter embolization are rapid, safe and effective methods for diagnosis and treatment of renal artery pseudoaneurysms.

<P>A Gram-stain-negative, aerobic bacterium, designated strain YIM 77409T, was isolated from the Niujie hot spring in the Eryuan county of Dali, Yunnan province, south-west China. Cells of the strain were rod-shaped and colonies were yellow and circular. The strain grew at pH?6.0-8.0 (optimum, pH?7.0) and 50-70C (optimum, 60-65C). The predominant menaquinone was MK-8 and the DNA G+C content was 66.4?mol%. Major fatty acids (>10?%) were iso-C15?:?0 and iso-C17?:?0.The polar lipids consisted of one aminophospholipid, one phospholipid and two glycolipids. 16S rRNA gene sequence analysis showed that strain YIM 77409T formed a cluster with Thermus scotoductus DSM 8553T, Thermus antranikianii DSM 12462T, Thermus caliditerrae YIM 77925T and Thermus tengchongensis YIM 77924T, with highest 16S rRNA gene sequence similarity to T. scotoductus DSM 8553T (97.57?%). However, DNA-DNA hybridization indicated that strain YIM 77409T should be viewed as a representative of a novel species, as there was only 30.6??1.6?% reassociation with T. scotoductus DSM 8553T. On the basis of the morphological and chemotaxonomic characteristics, as well as the genotypic data, it is proposed that strain YIM 77409T represents a novel species of the genus Thermus, with the name Thermus amyloliquefaciens sp. nov. The type strain is YIM 77409T (?=?DSM 25898T?=?KCTC 32024T).</P>

<P>Alterations in metabolic pathways are gaining attention as important environmental factors affecting life span, but the determination of specific metabolic pathways and enzymes involved in life span remains largely unexplored. By applying an NMR-based metabolomics approach to a calorie-restricted yeast (<I>Saccharomyces cerevisiae</I>) model, we found that alanine level is inversely correlated with yeast chronological life span. The involvement of the alanine-metabolizing pathway in the life span was tested using a deletion mutant of <I>ALT1</I>, the gene for a key alanine-metabolizing enzyme. The mutant exhibited increased endogenous alanine level and much shorter life span, demonstrating the importance of <I>ALT1</I> and alanine metabolic pathways in the life span. <I>ALT1</I>’s effect on life span was independent of the TOR pathway, as revealed by a <I>tor1</I> deletion mutant. Further mechanistic studies showed that <I>alt1</I> deletion suppresses cytochrome <I>c</I> oxidase subunit 2 expression, ultimately generating reactive oxygen species. This paper presents a moth-flame optimization (MFO)-based maximum power point tracking (MPPT) method for photovoltaic (PV) systems. The MFO algorithm is a new optimization method that exhibits satisfactory performance in terms of exploration, exploitation, local optima avoidance, and convergence. Therefore, the MFO algorithm is quite suitable for solving multiple peaks of PV systems under partial shading conditions (PSCs). The proposed MFO-MPPT is compared with four MPPT algorithms, namely the perturb and observe (P&O)-MPPT, incremental conductance (INC)-MPPT, particle swarm optimization (PSO)-MPPT and whale optimization algorithm (WOA)-MPPT. Simulation and experiment results demonstrate that the proposed algorithm can extract the global maximum power point (MPP) with greater tracking speed and accuracy under various conditions.

Trehalosemetabolism is critical for production of ATP, provision of carbon source, and facilitation of carbohydrate absorption. Trehalose-6-phosphate synthase (TPS) is a rate-limiting enzyme in trehalose biosynthesis. In the present paper, a TPS gene (LdTPS)was cloned inLeptinotarsa decemlineata. At young larval instars, the expression levels of LdTPS were high just before and right after ecdysis, and were low at the mid instar stages. In the fourthinstar larvae, two peaks occurred at 24 h after ecdysis and at the wandering stage. In vitro midgut culture and an in vivo bioassay revealed that 20E stimulated LdTPS transcription. Conversely, a reduction of 20E by RNA interference (RNAi) of a prothoracicotropic hormone receptor gene LdTorso and an ecdysteroidogenesis gene LdSHD repressed LdTPS expression. Moreover, disruption of 20E signaling by knockdown of LdEcR, LdE75 and LdFTZ-F1 reduced LdTPS transcript levels. Similarly, in vitro culture and an in vivo bioassay showed that exogenous JH or JH analog methoprene and pyriproxyfen activated LdTPS expression. An increase in endogenous JH by RNAi of an allatostatin gene LdAS-C enhanced the transcription. In contrast, a decrease in JH by silencing of a JH biosynthesis gene LdJHAMT downregulated LdTPS expression. Moreover, knockdown of LdILP2 repressed LdTPS transcription. Therefore, LdTPS transcription is tuned by 20E, JH and ILP signaling pathways in L. decemlineata.
