What is the Most Effective Strategy for Improving the Cancer Screening Rate in Japan?

Background: Cancer screening rates in Japan are much lower than those in Western countries. This study evaluated the relationship between cancer screening rates and strategies used to improve screening rates, and determined which strategy is the most effective. Materials and Methods: All municipalities are responsible for conducting gastric, lung, colorectal, cervical, and breast cancer screenings in Japan. Of the 1,746 municipalities in total, 92-99% were included in the analyses for each cancer screening. Using national data in 2009, the correlations between cancer screening rates and strategies for improving screening rates of all municipalities, both large (populations of over 30,000) and small (populations of under 30,000), were determined. The strategies used were as follows: sending personal invitation letters, personal visits by community health workers, use of a clinical setting for screening, and free screening. Results: Of all four strategies used to improve cancer screening rates, sending personal invitation letters had the highest correlations with all screening rates, with the exception of breast cancer screening. The partial correlation coefficients linking this strategy with the screening rates in all municipalities were 0.28, 0.32, 0.30, and 0.26 for gastric, lung, colorectal, and cervical cancer screening, respectively. In large municipalities, the correlations between the number of examinees in a clinical setting and the screening rates were also relatively high, particularly for cervical cancer screening (r=0.41). Conclusions: Sending personal invitation letters appears to be particularly effective in improving cancer screening rates in all municipalities. All municipalities should implement a system that sends personal invitation letters for cancer screening. In large municipalities, increasing the availability of screening in a clinical setting is also effective in improving cancer screening rates.

Nephroblastomatosis (NBM) is a precursor of Wilms tumor. We herein report a case in which Wilms tumor was initially suspected and the affected kidney was removed. The tumor was subsequently diagnosed as intralobar NBM and a favorable outcome was achieved with postoperative chemotherapy. A 2-year-old boy who presented with gross hematuria was found to have an enlarged left kidney with hydronephrosis. Needle biopsy of the left kidney suggested Wilms tumor and left nephrectomy was performed. The tumor was histopathologically diagnosed as intralobar NBM. Although NBM is regarded as a precancerous lesion, a definite treatment plan has not yet been established. In the present case, we used a similar chemotherapy regi-men to that for Wilms tumor. Eight years after the completion of chemotherapy, Wilms tumor has not developed or recurred. Appropriate management plans need to be developed by accumulating similar cases.

Objective: The purpose of this clinical study is to determine the effect of dentifrice containing 0.05% Isopropyl methylphenol (IPMP) and 0.05% Dipotassium glycyrrhizinate (GK2) on improvement of gingivitis related symptoms. Materials and Method: Double blind clinical study was carried out in 120 healthy Korean adults to evaluate the improvement effect of test dentifrices for 3 month. All subjects (range of age: 23-35) were randomly assigned into 4 groups, 1) control (without both IPMP and GK2), 2) 0.05% IPMP/ 0.05% GK2 group, 3) 0.05% IPMP group and 4) 0.05% GK2 group. All subjects were provided with their assigned test dentifrice and soft-bristled toothbrush for home use. Subjects were instructed to brush their teeth for 3 minutes three times daily using only the dentifrice and toothbrush provided. Results: After 3 month test period, IPMP/GK2 group showed statistically significant (p<0.05) reductions for halitosis (17.5%), Plaque index (36.5%), gingival crevicular fluid (34.7%), periodontal pocket depth (13.1%), and Gingival index (16.0%) compared to control group. Conclusion: The dentifrice containing of 0.05% IPMP and 0.05% GK2 showed the most efficient in improving the gingivitis related symptoms. These findings might suggest that a combination of IPMP and GK2 showed a synergistic effect or special mode of action.

<P>In current study, we report the direct interaction force comparison for a synthesized peptide probe to actin and cofilin protein to actin using atomic force microscopy. The peptide probe was synthesized following the actin binding module of cofilin protein. Thus, the functionality of the peptide probe was similar with that of the cofilin protein. The difference between the peptide probe and cofilin protein was the molecular size. The small peptide probe enables highly dense surface modification, thus produces different interaction force curves compared with that of original cofilin protein to actin. The results showed the peptide probe was possible to measure many numbers of the interaction force though the measurement of single molecular order interaction force was a weak point. These imply that the peptide probe has a merit when that was applied surface related applications such as protein sensor and protein-protein interaction separation.</P>
Genomic Profiling Shows Increased Glucose Metabolism in Luminal B Breast Cancer

We had previously reported a close association between pathological response and the maximum tumor standardized uptake value (SUVmax) measured by 18F-fluorodeoxyglucose positron emission tomography prior to chemotherapy in estrogen receptor (ER)-positive breast cancer. We hypothesized that glucose hypermetabolism by luminal B tumors may result in chemotherapy responsiveness. Using a single-gene expression assay, TargetPrint® (Agendia) and a 70-gene expression classifier,MammaPrint® (Agendia), we divided 20 patients with ERpositive primary breast cancer into luminal A and luminal B subtypes and compared the tumor SUVmax value between the two groups. A significantly higher SUVmax was measured for luminal B tumors (n=10; mean±SD, 7.6±5.6) than for luminal A tumors (n=10; mean±SD, 2.6±1.2; p=0.01). Glucose hypermetabolism could help predict intrinsic subtyping and chemotherapy responsiveness as a supplement to ER, progesterone receptor, HER2,and Ki-67 histochemical scores.

<P>This paper presents a direct interaction force measurement between histidine molecules using AFM force-distance curve measurement. AFM force-distance curves between the histidine-modified cantilever and substrate in the different conditions with or without intercalating Cu2+ ion were measured and interpreted via Gaussian curve fitting analyses. The adhesion force between histidine molecules was shown to be 110 pN under the presence of Cu2+. The result was compareable to the measured adhesion force about 0 pN, which was measured by the removal of Cu2+ ion with the addition of EDTA. The result indicated the direct histidine-histidie interaction was difficult without the role of the bridigible ionic component. From the results, the possibility of direct measurement on chemical affinities between biomolecules was suggested by using AFM force-distance curve analyses. Especially, the current approach showed the possible affinity measurement techniques that elucidate the role of bridge ions.</P>
Distribution of Elements on Tobacco Trichomes and Leaves under Cadmium and Sodium Stresses

When tobacco (Nicotiana tabacum) plants are exposed to toxic level of cadmium (0.2 mM Cd), their trichomes actively excrete crystals (Choi et al., 2001). In this study, we investigated the distribution of Cd and NaCl on trichomes and leaf surfaces. Energy dispersive x-ray (EDX) analysis revealed that, under toxic Cd stress, crystals exudated from the trichomes contained high amounts of Ca, Mg, and Cd, as well as low levels of P, S, and Mn. Electron spectroscopic imaging (ESI) from trichomes and attached crystals showed that these crystals emitted denser radiation energy for Ca and Cd than did the head cells of the trichomes. However, no Cd was detected on the trichome surface itself or within the leaf epidermis. In contrast, treatment with salt (NaCl) did not stimulate crystal formation; instead, it induced the abnormal expansion of trichome cells. Although Na was not accumulated within the crystals, a consider-able amount of both Na and Cl was sequestered within the stalk cells of the long trichomes. Therefore, we believe that tobacco trichomes play an important role in Cd crystal exudation through crystallization, but that, under NaCl stress, the long trichomes sequester those elements within their stalks.
