    본 연구의 목적은 알코올 중독자들을 대상으로 가족력 유무에 따른 재발요인을 파악하여 다양한 치료적 접근을 할 수 있는 근거를 제시하며 알코올중독의 재발을 방지할 수 있는 근거자료로서 마련하는데 있다.
    이 연구를 위하여 연구대상으로는 DSM-Ⅳ(Diagnostic Statistical Manual of Mental Disorders Ⅳ)의 진단기준에 의해 알코올 사용 장애로 진단되어 전문 정신병원 및 종합병원의 정신과 폐쇄 병동에 입원 중인 알코올중독자로 입원횟수가 2번 이상인 환자이다. 또한, 알코올중독이라는 동일한 진단명을 지닌 전국에 위치한 전문 정신병원 및 정신과가 설치된 종합병원의 폐쇄병동에 입원한 환자 중 총307명을 연구대상으로 하였다. 자료분석방법으로는 SPSSWIN 11.0을 이용하여 빈도분석(Frequency), t-test, 상관관계(Pearson Correlation), 다중회귀분석(Multiple regression analysis) 등을 사용하였다.
    본 연구의 주요한 연구결과는 다음과 같다.

    둘째, 재발과 재발요인에 관한 관계를 살펴보면, 우선 가족력이 있는 집단의 경우는 자기효능, 음주기대, 충동성, 사회적 지지요인이 유의미한 상관관계가 나타난 반면에 대처방식과 자기주장의 유의미한 상관관계가 나타나지 않았다. 다음으로 가족력이 없는 집단의 경우는 자기효능, 음주기대, 대처방식, 충동성은 유의미한 상관관계가 나타나지 않은 반면에 사회적지지, 자기주장이 부적상관관계로 유의미한 상관관계를 보였다.
    셋째, 재발요인이 재발에 미치는 영향을 살펴보면, 가족력이 있는 집단의 경우는 음주기대와 사회적 지지가 유의미한 관계를 보이며, 가족력이 없는 경우는 자기주장이 유의미한 관계가 있는 것으로 나타났다.
    본 연구의 결과를 토대로 한 함의는 다음과 같다.
    첫째, 본 연구는 기존의 대상을 고려하지 않은 치료적 접근과는 달리 가족력이라는 알코올에 있어서 중요한 요인을 고려하였다는 것에 의의가 있다. 집단의 구분 없이 재발요인만을 탐색하였던 기존 연구들과는 달리 집단을 구분하고 개인과 환경을 고려하여 다각적인 접근을 할 수 있는 토대를 마련하였다. 또한 치료접근에 있어서 좀 더 차별적이고 집중적인 접근이 요구됨을 시사하였다.
    둘째, 가족력이라는 것은 그 동안 의학분야에서 유전적인 소인으로서 아형을 구분하는데 유용한 정보로 사용되었으나, 사회복지분야에서 접근은 전무할 실정이었다. 그러나 가족력이 유전적인 것뿐만 아니라 사회학습을 통하여 가정 내에서 성장하면서 한 개인의 삶에 영향을 미칠 수 있기 때문에 중요한 요인으로 다룰 필요성이 있으며 이는 사회복지프로그램 개발에도 근거자료로서 유용한 정보가 될 수 있다.
    셋째, 가족력이 있는 알코올중독자의 경우는 잘못된 신념을 통해서 음주에 대하여 긍정적인 기대를 지니고 있는 것을 고려해 보았을 때, 인지행동치료를 통하여 신념의 변화를 가져오는 것이 중요한 치료방법이라 하겠다. 따라서 알코올 재발의 예방적 차원의 효과성을 평가하고 검증된 치료모델이 개발되어서 효과성이 입증된 인지행동접근의 프로그램으로서 치료와 재활이 요구된다.
    넷째, 가족력이 없는 알코올중독자의 경우는 후천적으로 학습되고 부정적인 생활패턴의 반복으로 인하여 재발하는 것을 고려했을 때, 집단 활동을 통한 치료와 재활로서 대인관계형성을 향상시키고 대처능력을 증진시키는 것이 요구된다.
    다섯째, 알코올중독에 있어서 재발을 예방하기 위해서는 다차원적인 프로그램 개입뿐 아니라 지속적인 사후관리가 될 수 있도록 사례관리(Case- iv -Management)가 필요하다.
    이상의 연구결과를 토대로 하여 다음과 같은 제언을 하고자 한다. 첫째, 본 연구에서는 알코올중독에 있어서 가족력 유무에 따른 재발에 미치는 요인이 상이한 것은 알 수 있었다. 그러나 가족력이 없는 경우에는 본 연구에서 살펴본 요인 외에 좀 더 다양하고 구체적이며 직접적인 영향을 미치는 요인연구가 필요하며 그에 따른 개입방안의 마련이 재발을 예방하는데 토대마련이 될 것이다. 둘째, 본 연구는 남성알코올중독자로 국한하여 연구를 하여 일반화에는 다소 무리가 있다. 여성알코올중독자를 함께 고려하여 차후에 지속적인 연구가 이루어진다며 좀 더 심층적인 접근이 가능할 것이다. 셋째, 사회복지적 측면에서 가족력을 고려한 재발요인들을 검증해 낸 것에 대한 함의는 있으나, 그에 뒷받침할 프로그램에 대한 효과가 부재하다. 따라서 클라이언트의 복합적인 욕구에 맞는 프로그램을 개발하고 효과성을 검증하는 연구가 필요하다.

    The paper aims to examine recurrence of alcohol dependence of alcoholic depending upon the existence of their family history, and to suggest the bases of various treatment approaches, and to give material preventing recurrence of alcohol dependence.
    The objects were 307 patients who were in hospital two times or more because of alcohol dependence and were in the isolation ward for psychiatric patients of general hospital as well as psychiatric clinic in accordance with the diagnosis of the DSM-Ⅳ(Diagnostic Statistical Manual of Mental Disorders Ⅳ). The SPSSWIN 11.0 was adopted to conduct frequency analysis, t-test, Pearson correlation analysis and multiple regression analysis, etc.
    The findings were as follows:
    Firstly, the group having family history had higher recurrence frequency, lower self-efficiency, higher expectation on drinking and weak preparations. In addition, the group had high impulse, low social support, and low self-assertion. However, the group having no family history had lower recurrence frequency, high self-efficiency, low expectation on drinking and high preparations. The group having no family history had low impulse, high social supports and high self-assertion than the one having family history had.
    Secondly, both the recurrence and its factors had significant correlation at self-efficiency, drinking, impulse and social supports, etc: However, the countermeasure and the self-assertion had no significant correlation. The group having no family history had no significant relation at self-efficiency, drinking, countermeasure and impulse, etc, but had negative correlation at social supports and self-assertion.
    Thirdly, the effects of recurrence factors upon recurrence were as follows: The group having family history had significant relation at drinking expectancy and social supports, while the one having no family history had significant relation at self-assertion.
    The findings were as follows:
    Firstly, being different from the approach to medical treatment not considering existing target, the study regarded the family history as an important factor of alcohol dependence. The precedent studies examined only recurrence factors without classifications of groups, while this study classified groups and considered individuals and environment to establish the bases for multi-sided approaches. More differential and intensive approach was demanded at medical treatment access.
    Secondly, the family history was used to classify genetic trend at medical science field, but had no access at social welfare field. However, the family history is genetic and grows up at home through social learning to have influence upon an individual's life, so that it is important factor and useful information at the development of social welfare program.
    Thirdly, the alcoholic having family history has affirmative action of drinking because of wrong thinking: Considering such a fact, the cognitive behaviour therapy making change of thinking is thought to be important. Therefore, the alcoholic needs to be given medical treatment and rehabilitation by using the cognitive behaviour therapy that is found to prevent recurrence of alcohol dependence effectively.
    Fourthly, the alcoholic having no family history needs to improve not only personal relations but also preparations by being given medical treatment and rehabilitation through group activities, because the recurrence occurs by learned and negative behaviour pattern.
    Fifthly, the case management, which includes multidimensional program and assures of continuous follow-up, is needed to prevent the recurrence.
    In conclusion, the paper suggests as follows:
    Firstly, in this paper, the family history of alcohol dependence had different influence upon the recurrence. However, the ones having no family history need to be researched regarding various, concrete and direct influential factors in addition to the factors mentioned in the paper, and associated ways can establish the base preventing recurrence.
    Secondly, the paper examined only men alcoholic to have less generalization. The research including women alcoholic can examine alcohol dependence more deeply in the future. Thirdly, the recurrence factors, which consider family history from point of view of social welfare, can be verified: However, there was no effect of supporting programs.
