With the fast pace of modernization and changes in lifestyle, the pattern of disease has been transformed from primarily acute conditions to chronic illnesses. Cancer is expected to be the leading cause of death in Korea now and in the future. The incidence of breast cancer in Korean women has increased steadily, and has emerged as a major concern to women.
Most literature suggested that breast self-examination(BSE) is a low-cost, noninvasive, simple, and early screening method for the detection of breast cancer. Nurses play an important role as health care providers in practicing and educate female client about BSE. So, it would be meaningful to investigate the current status and the relevant factors affecting the their practice of BSE.
This cross-sectional survey was performed to investigate the nurses' current status of BSE practice and it's affecting factors. The subjects were 313 nurses working at 2 hospitals in Daejeon metropolitan city and 1 hospital in Chungnamprovince. The data was collected from September 1st, 2002 to September 30th, 2002and analyzed by using SPSS PC+(Version 10.0) program by means of t-test, ANOVA, Pearson's Correlation, and the Stepwise multiple regression analysis.
The results were as follows:
1. 97% of the subjectshad heard about BSE, and 80% of them from the nursing school. The mean score of the practice of BSE was 14.3 (7 at the lowest and 28 at the highest). The level of the BSE practice is significantly different according to the job position and level of nurses' interest in the health programs.
2. The mean score of the knowledge about breast cancer and BSE was 11.2 (0 at the lowest and 16 at the highest). The level of the knowledge is significantly different according to the type of nurses' working units, and status of menstration.
3. The mean score of the attitude toward BSE was 36.2(16 at lowest and 64 at the highest). There were no relevant factors significantly affecting the level of attitude toward BSE.
4. The mean score of the self-esteem was 25.2(10 at the lowest 40 at highest). There were no relevant factors significantly affecting the level of the self-esteem.
5. The mean score of health locus of control in all subjects was 55.2 (18 at the lowest and 90 at the highest). The score of health locus of control is significantly different according to the type of nurses' working unit and nurses' career.
6. There were statistically significant correlation noted between the practice of BSE and knowledge about breast cancer and BSE, but it's explanatory power as a major predictive factor affecting the BSE practice was very low.
Based on the above findings, it was found that nurses, who might be a role model and should play an importantrole as an educator and a counselor, had not been conducting BSE regularly and correctly. Besides, their knowledge in and attitude toward breast cancer and BSE were also far from being sufficient.
Among the factors affecting the practice of BSE, knowledge level was very closely related with the BSE practice. Therefore, it can be concluded that promoting knowledge can be a strategy to enhance the practice of BSE in the nurses. For this purpose, education about breast cancer and BSE should be carried out thoroughly and effectively at the nursing colleges. In the health care institutions as well, programs for BSE should be developed and actively provided to health professionals on the persistent basis. The nurses as well-prepared educators should also provide extensive education on breast cancer and BSE to the general women, the targets of nursing service, to reduce ever-increasing incidence rate of breast cancer and the seriousness of this disease by promoting this practice.
Itwas turned out that there was a significant relationship between knowledge and practice, however, the explanatory power seemed to be very low. So, the practice won't be enhanced only by providing educational programs. Therefore, the practice should be improved by finding out other factors more powerful in affecting the practice of BSE. In addition, subsequent studies should also be conducted on this issue.
