The purpose of this study was to examine the handwriting accuracy and speed of children with cerebral palsy along with handwriting movements, muscle tone when posing to handwrite, and visual-motor integration ability. The Korean alphabet handwritings of 47 children with cerebral palsy and 9 children without disabilities were evaluated, and their handwriting movements were observed. Also, an electromyography (EMG) and the Developmental Test of Visual-Motor Integration (VMI) were carried out for the measurements. The summary of the analyzed results of the study is as follows:
The handwriting accuracy of children with spastic cerebral palsy showed a low level of 39.2%, and their handwriting speed was approximately 3.3 times slower than that of children without disabilities. Among children with spastic cerebral palsy, children with monoplegia in the lower limb, paraplegia, and hemiplegia could dictate alone and children with triplegia only needs a little help, but children with quadriplegia needs a lot of help to dicate. Also, when posing to ahndwrite there was over-tension in the elbow, shoulder, and neck whereas there was less tension in the wrist than that of children without disabilities. Compared to children without disabilities, their visual-motor integration ability was at a very low level of 38.2%.
The handwriting accuracy of children with athetosis cerebral palsy showed a very low level of 21.7%, and their handwriting speed was approximately 3.8 times slower than that of children without disabilities. Among the children with athetosis cerebral palsy, children with paraplegia could handwrite properly alone, but children with quadriplegia needs complete or a great deal of assistance to handwrite. Also, because of involuntary movements, their muscle tone was higher than that of children without disabilities, and unneccessary over-tension was caused in the elbow, shoulder, and neck to pose for handwriting. Compared to children without disabilities, thier visual-motor integration ability was at a very low level of 29.6%.
The handwriting accuracy of children with ataxic cerebral palsy showed a very low level of 4.2%, and their handwriting speed was approximately 5.9 times slower than that of children without disabilities. Children with ataxic paraplegia and quadriplegia could all properly perform sitting posture, arm movements, hand movements, and holding the pencil, but they needed a great deal of help to perform handwriting movements. Their ability to hold a pencil and control wrist movements in order to handwrite was weak. When posing to handwrite, over-tension appeared in the elbow and neck. Compared to children without disabilities, thier visual-motor integration ability was at a very low level of 32.7%.
The handwriting accuracy children with mixed cerebral palsy showed a very low level of 16.7%, and their handwriting speed was approximately 2.4 times slower than that of children without disabilities. Children with mixed quadriplega could perform sitting movements and arm movements with a little help, but they needed a great deal of help to perform hand movements and pencil holding movements, and complete help to perform handwriting movements. Their ability to hold a pencil and control wrist movement in order to handwrite was weak. When posing to handwrite, over-tension was caused in the elbow, shoulder, and neck. Compared to children without disabilities, their visual-motor integration ability was ay a very low level of 41.7%.
When comparing spastic and athetosis children, spastic children have higher handwriting accuracy, faster handwriting speed, and higher level of handwriting movement performance and visualpmotor integration ability than athetosis children. Also, the muscle tone of athetosis children during handwriting was higher than that of spastic children in the six muscles excluding biceps brachii and trapezius.
When putting together the above-mentioned results, the handwriting accuracy of children with cerebral palsy showed a low level of approximately 4.2-39.2% depending on the type of paralysis, and the handwriting speed was approximately 3.4 times slower than that of children without disabilities. Out of children with cerebral palsy, children with monoplegia in the lower limb and hemiplegia could dictate properly alone children with paraplegia only needed a little help, but children with triplegia and quadriplegia needed complete or a great deal of help to handwrite. Over-tension appeared even before they posed to handwrite. There was difficulty to perform the handwriting because of over-tension in the elbow, shoulder, and neck rather than in the fingers and wrist. Compared to children without disabilities, their visual-motor integration ability was at a very low level of 36.7%. Among children with cerebral palsy, spastic children wrote more accurately and faster than athetosis children, and their level of handwriting movement performance and visual-motor integration abilities were also higher.
