심리적 피학대 여성의 수치심, 죄책감 및 사회적 지지가 외상 후 스트레스장애(PTSD)에 미치는 영향

국문 초록 (Abstract)

본 연구는 학대받는 여성에게 있어서, 학대와 PTSD증상 사이의 관계를 횡단적으로 연구한 것이다. 지금까지 심리적 학대의 효과는 상대적으로 잘연구되지 않았으나, 여러 연구들은 심리적 학대가 신체적 학대만큼 여성에게 유해하다고 밝히고 있다. 따라서, 본 연구는 심리적 학대와 PTSD와의 관계를 명확하게 하기 위해, 신체적 학대의 효과를 통계적으로 통제한 후심리적 학대의 독립적 효과를 보았다. 또한 학대받는 여성의 PTSD발달에 있어서 중요한 정서적 변인으로서 수치심, 죄책감을 조사하였고, PTSD증상을 중재하는 변인으로서 사회적 지지를 조사하였다.
연구결과, 심리적/신체적 학대 모두 PTSD증상을 유의미하게 예측하였고, 신체적 학대를 통제한 후에도 심리적 학대의 독립적 효과가 유의하였다. 그리고 심리적 학대는 수치심을 통해 PTSD와 연결되었으나, 죄책감을 통해PTSD와 연결되지는 않았다. 사회적 지지는 학대와 PTSD 증상을 중재하지못했으나, 사회적 지지 중 가족지지는 PTSD 증상과 유의한 부적상관을 나타내었다. 따라서, 학대받는 여성을 돕기 위해서는 사회적으로 지지해주기보다 우선적으로 그들이 가진 수치심을 제거하는데 초점을 두어야 할 것이다.
그리고, 신체적 학대뿐만 아니라 심리적 학대에 초점을 둔 가정폭력피해자와 가해자를 위한 프로그램, 가정폭력예방프로그램을 만들 필요성을 제기하였다.

다국어 초록 (Multilingual Abstract)

This study examined the relationship between psychological/physical abuse and PTSD in 177 married women. The effect of psychological abuse has been relatively understudied compared to physical abuse.
However, many empirical studies have showed that psychological abuse can be as harmful as physical abuse. In order to clarify the relationship between psychological abuse and posttraumatic stress disorder(PTSD), the present study attempted to investigate the independent effect of psychological abuse after statistically controlling for the effects of physical abuse. This study also examined shame and guilt as important affective experiences in the development of PTSD in abused women and social support as a moderator for PTSD symptoms.
Result indicated that both psychological and physical abuse predicted PTSD symptoms significantly and that psychological abuse was a significantly independent predictor of PTSD symptoms even after controlling for the effects of physical abuse. In addition, psychological abuse was related to PTSD symptoms moderated by shame but not by guilt. Social support was not significant moderator between abuse and PTSD symptoms. However, support from family among social supports was negatively correlated with PTSD symptoms.
Results suggest that clinician need to give a first priority to eliminating guilt that abused women may have. Programs for domestic violence abused and abuser and preventive programs focusing not only on physical abuse but also psychological abuse are need.
