본 논문은 방문요양보호사의 노인 죽음 경험을 분석을 통해 방문요양보호사의 직업적 특성을 밝혀 전문적인 직업으로서의 방향성을 제시하는 데 목적이 있다. 연구의 목적을 달성하기 위해 방문요양보호사로 2년 이상 근무하였고 노인 죽음 경험이 있는 연구참여자 5명을 선정하여 심층 면담을 통해 자료를 수집하였다. 수집된 자료는 사례 내 분석 및 사례 간 분석을 통해 상·하위범주를 추출하였으며, 이에 따른 분석결과는 다음과 같다.
첫째, 방문요양보호사는 간접적인 방식으로 자신이 방문요양 서비스를 제공했던 노인의 죽음에 대해서 경험하였다. 노인 돌봄 계약이 종료된 후에 센터장이나 보호자로부터 혹은 본인이 궁금해서 노인의 소식을 물어보았다가 노인의 죽음에 대해서 알게 된 경우가 가장 많았다. 방문요양 근무 중에 노인이 임종의 증상을 보이는 경우 구급대나 보호자에게 인계하기 전까지 노인을 돌보고 이후에 노인이 돌아가셨다는 것을 간접적으로 알게 되었는데, 실제로 임종을 경험한 사례도 있었다.
둘째, 방문요양보호사는 노인의 죽음을 간접적으로 알게 된 경우에도 정서적 고통과 상실의 아픔을 겪으며, 근무 종료로 인한 스트레스와 작별 인사 없는 죽음에 대한 아쉬움을 경험하였다. 아무런 마음의 준비 없이 노인의 죽음을 겪고 나서 한동안은 죽음을 받아들이기 어려웠음을 토로하였고, 노인과 더 많은 시간을 보내지 못한 것에 대한 후회를 나타냈다.
셋째, 방문요양보호사는 노인의 죽음 경험이 요양보호사로서 당연히 겪어야 하는 과정으로 받아들이거나 노인이 언제든지 돌아가실 수 있다는 마음으로 미리 선을 그어 대응하였다. 또한, 바쁜 생활 및 시간 흐름으로 죽음의 충격이 자연스럽게 무뎌지는 것을 경험한 사례들이 있었는데, 방문요양보호사들이 직접적인 임종을 경험하지 않았고 노인 죽음으로 인해 근무지가 변경되면서 죽음에 대해 떠올릴 기회가 줄어들었기 때문으로 보이며 이는 시설 요양보호사와의 근무환경 차이에서 비롯된 것이라 할 수 있다.
넷째, 방문요양보호사는 노인 죽음 경험으로 본인은 편한 죽음을 맞이하고 싶다고 하였는데, 이는 70대의 방문요양보호사에게서 두드러지게 나타났다. 또한, 다른 노인의 죽음에 대해서 불안감을 표현하는 등의 죽음에 대한 태도의 변화가 나타났다. 노인의 죽음을 경험하고 나서 또 다른 노인의 죽음에 대해서 불안감을 토로하였는데, 노인 죽음 경험은 익숙해지는 것이 아니라 피하고 싶은 경험이라는 것을 드러낸다.
마지막으로 방문요양보호사는 노인 죽음 경험 이후에 임종 교육이 필요하다고 밝혔다. 노인의 죽음에 대한 불안을 해소하는 데에도 임종 교육은 필요하다고 하였는데 노인을 돌볼 때 발생할 수 있는 응급상황이 발생하거나 노인의 임종 징후를 발견하였을 때의 당황스러움과 불안감을 해소하기 위해서이다.
본 연구의 함의는 다음과 같다. 첫째, 방문요양보호사의 노인 돌봄 범위를 돌봄 그 자체에서 노인 죽음 이후로 확대하여, 방문요양보호사의 요양서비스 제공이 사회적으로 가치 있는 일이라는 점에서 전문적인 직업으로 인정받을 수 있는 환경이 조성되어야 함을 밝혔다. 둘째, 노인 죽음 이후 정서적 불안이나 상실감에 대한 전문 상담 창구나 정서적 지지 제도에 대한 필요성을 제기하였다. 셋째, 방문요양보호사에 대한 임종 교육의 필요성을 제시하였다. 넷째, 방문요양보호사의 노인 죽음 경험이라는 직업적 특성을 활용하여 ‘호스피스보조활동인력’으로 전문성을 강화하는 방안에 대한 논의를 제기하였다.

The purpose of this study is to analyze home-visiting care workers' experiences of the elderly's death and thereby, examine their occupational characteristics and thereupon, suggest a direction for the home-visiting care workers as professional occupation. For this purpose, this study sampled 5 home-visiting care workers who had worked for 2 years or longer and had experienced the elderly's death and thereby, conducted in-depth interviews with them to collect the data. The data collected thus were processed for the intra-case and inter-case analyses to determine the upper and lower categories. The results of the analysis can be summarized as follows;
First, the home-visiting care workers experienced the elderly's death indirectly. Most of them would heard about the death from the home-visiting care center manager or from the elderly's guardians after the expiration of their home-visiting care contracts. Most of them asked them because they were concerned with their elderly clients' health. While they were serving their elderly clients as home-visiting care workers, they would witness their elderly clients' critical condition and thus, would hand over the elderly to the emergency medical service, and thereafter, they would heard that their elderly clients had passed away. Some of the subjects experienced their elderly clients' death directly.
Secondly, the subjects would suffer from the emotional pain and distress of loss even if they had known about their elderly clients' death indirectly. Moreover, they experienced the stress due to the end of their care services and the regret because of no farewell saying. Because they experienced their elderly clients' death all of a sudden, they would feel difficult for accepting the reality for a while after their elderly clients' death. They regretted not having spent more time with their elderly clients.
Thirdly, the home-visiting care workers accepted their experience of the elderly's death as the natural destiny or were prepared for the death. Some of the subjects experienced their elderly clients' death placidly over time, because they had not closed their elderly client's eyes, and because they were busy enough to be reminiscent of their elderly clients' death. Such placidity may well have been due to the differences of their working environment.
Fourthly, the subjects would say that they would like to die placidly. Such a wish was outstanding in the subjects aged 70's. In addition, the subjects' attitude towards death came to change. They expressed their fear for death. After experiencing the elderly's death, they expressed apprehension of another elderly's death. Such findings may well suggest that their experience of elderly's death would not become accustomed but make them fear the death.
Lastly, subjects expressed after their experience of the elderly's death, that an education on deathbed would be essential. They said that such education would help to have them relieved of their anxiety due to their experience of the elderly's death. They felt that such an education would also help them not to be embarrassed and apprehensive but calm when they found a symptom of the elderly's death.
The implications of this study can be summed up as follows. First, the home-visiting care workers' scope of services needs to be expanded to cover the services after their elderly's death in order to help their services perceived as socially valuable professional ones. Secondly, this study emphasized the need for a specialized counselling service for the home-visiting care workers or some emotional support mechanism for them. Thirdly, it is necessary to educate the home-visiting care workers to help them relieved of the fear. Fourthly, it is deemed necessary to help the home-visiting care workers enhance their professionalism as the assistants to 'hospice services', taking advantage of their elderly care service experiences.
