이 연구는 평범한 학생이 대학교육을 거치면서 형성해나가는 전문직업인으로서의 정체성에 주목하여 한국 의대생이 겪는 의대에서의 학습 경험과 직업정체성 형성 과정을 알아보는 데에 그 목적을 두었다. 이를 위하여 임상 실습에 참여한 경험이 있는 의대 본과 4학년 학생들을 대상으로 심층 인터뷰를 진행하였다.
연구 결과에서는 한국 사회의 특수한 사회문화적 맥락 안에서 의대생은 역동적으로 예비 의사로서의 직업정체성을 형성하고 있는 것으로 나타났다. 연구 참여자들은 의대 입학 초기에는 의사를 동경하는 일반 사람들의 시선을 그대로 답습하고, 의대생을 이미 예정된 의사로 상정하는 사회적 인식으로부터 제공된 ‘주어진 정체성’을 학습하였다.
연구 참여자들은 의대에서 다양한 학습 경험을 통해 직업정체성 형성이 촉진되거나 저해되고 있었다. 직업정체성 형성에 관여하는 의대 공동체는 다른 사람들과의 상호작용이 제한적이고 치열한 경쟁 문화가 조성되어 있는 폐쇄적인 구조를 띠고 있었다. 이러한 의대 공동체 안에서 연구 참여자들은 좋은 성적을 받는 것에 치중하고 성적과 관련 없는 일에는 수동적이었으며, 사회와 사람에게는 무관심한 태도를 보였다. 폐쇄된 의대의 학습 환경 속에서 좋은 의사가 아닌 좋은 학생을 목표로 삼게 된 결과 이들은 예비 의사로서의 가치, 행동, 태도를 위협받고 있었다.
임상 실습은 연구 참여자들이 이전과는 다른 새로운 학습 경험을 통하여 의사를 향한 일반인의 시선을 거두고 의사의 삶을 있는 그대로 받아들이는 기회가 되었다. 병원 안으로 진입한 이들은 사람들과의 상호작용이 가능한 무형식학습을 바탕으로 맥락 안에서의 실제적 지식을 습득하였다. 환자에 대한 책임을 우선시하는 의사의 모습을 보면서 긍정적인 롤모델을 형성하고 환자와의 만남을 통해 의사로서의 자신의 역할과 책임에 대해 자문하였다. 그리고 개인적인 삶의 가치와 의사로서의 소명의식 사이에서 앞으로 어떠한 길을 걷는 의사가 될 것인지에 대한 고민을 거듭하였다. 연구 참여자들은 의대에서의 여러 가지 어려움과 고충 속에서도 이를 의사가 되기 위한 하나의 준비 과정으로 받아들였다. 병원의 실제적 맥락 안에서 상호작용에 바탕을 둔 무형식학습은 이들 스스로 의사가 된 자신의 모습을 그려보며 직업정체성의 주체로 자리매김하는 데에 결정적인 역할을 하고 있었다.
이 연구에서는 의대생의 학습 경험과 직업정체성에 관한 논의를 바탕으로 대학에서의 의학교육에 대해 시사점을 제시하였다. 첫째, 의대생이 사회적 인식이 제공하는 주어진 정체성을 탈피하고 정체성의 주체로 성장하기 위해서는 지식과 기술 암기를 강조하는 교육에서 벗어나 의사에게 필요한 다양한 역량을 학습할 수 있는 교육이 요구된다. 둘째, 개방적이고 유연한 의대 문화와 상호작용이 활발한 무형식학습이 필요하다. 셋째, 의대생이 정체성 갈등을 극복하고 의사라는 직업의 의미와 가치를 스스로 찾아갈 수 있는 학습 환경을 조성해야 한다.
지금까지 이 연구는 지식과 기술 개발에 편재된 의학교육과 비인간적인 의료 실태에 대한 비판적 문제의식에서 출발하여 한국 의과대학생의 학습 경험과 직업정체성 형성 과정을 탐색하였다. 이 연구에서는 한국 사회의 독특한 사회문화적 맥락을 고려하여 의과대학생의 직업정체성과 의과대학에서의 학습 경험에 대한 심층적이고 확장된 이해를 제공하였으며 이를 바탕으로 전문직업인을 양성하는 대학의 역할에 대하여 실천적 시사점을 제시하였다는 점에서 의미가 있다.

The study was conducted to explore the process of professional identity formation of Korean medical students and learning experiences in medical school. For this end, medical students in their fourth year participated in the study. The data were collected by in-depth interviews.
The result of the study revealed that medical students dynamically formed their professional identity as a preliminary doctor in the specific sociocultural context of Korean society. The participants, in their earlier years, accepted 'given identity' provided from the social perception.
The learning environment in medical school had some problems. The exclusiveness of medical school community with competition culture caused limited interaction with other people. In this community, the participants obsessed with getting good grades while unconcerned with people and society, keeping passive attitudes to those unrelated to their grades. Their learning goal was to become a good student not a good doctor. The exclusiveness of medical school threatened values, behaviors, and attitudes of preliminary doctors.
Clinical practice, meanwhile, was a great opportunity to literally understand a doctor’s life. The participants acquired practical knowledge through interaction with people in context-based informal learning environment. Doctors whose top priority is patients became their positive role models. They asked themselves what their role would be as a doctor in relationship with patients. They are in conflict between individual values and calling as a doctor, considering what kind of doctor they are going to be in the future. The difficulties in medical school were accepted as a preparatory process of becoming a doctor by medical students. In the context of hospital, informal learning based on interaction played a crucial role in becoming professional agency.
This study suggests some implications for medical education based on the result. First, in order for medical students to be agency of their professional identity, it is necessary to provide education programs encourage them to learn various competencies required by professional doctors. Second, open, flexible, and supportive culture of medical school should be created. Third, it is needed to create learning environment that allow medical students to find out the meaning and value of a professional doctor by themselves.
The study starts with a critical consciousness in medical education focused on medical knowledge and skills and dehumanization in medicine. It is meaningful that the study provided an expanded understanding of professional identity and learning experiences of medical students, considering specific sociocultural context of Korean society, and suggests practical implications for roles of medical education.
