The purpose of this study was to be helpful for the endoscopic examination task, which is most burdensome for a task at Health Examination center, and which the examination patients complain of anxiety and discomfort about the examination, targeting the nurses who work at health examination center of a general hospital where is located in Gyeonggi Province, and the examination patients who visited in order to have a medical checkup. It was carried out in order to reduce a work burden of the endoscopic unit nurses by offering training after developing the nursing practice standard and guideline before gastroscopy in line with the endoscopic unit situation, and to offer high-quality information before gastroscopy to examination patients.
The simulation-based nursing training program before gastroscopy using the standardized patient was developed with 5 stages in analysis, design, development, implementation and evaluation based on ADDIE model in the instructional system design. To implement the operation of simulation following the stages of analysis and design and to verify effect of training at the stage of evaluation, the nurses' competency & confidence and the examination patients' satisfaction before and after the participation in training were evaluated
The research subjects were 21 nurses who agreed to cooperation with understanding this study through a face-to-face interview with a researcher, and 110 examination patients, among the nurses who are working at health examination center and the examination patients who visited in order to take gastroscopy from February to April in 2015.
As a research tool, the nursing competency was evaluated by which the researcher directly observed the nursing matters before gastroscopy as for examination patients, before and after participating in the simulation-based education training by using checklist that was directly developed by the researcher. The nursing confidence allowed nurses to have self-evaluation on own confidence level after nursing performance before gastroscopy as for examination patients by using the questionnaire of the competency evaluation tool. The examinee satisfaction was allowed to be measured questionnaire targeting the examination patients who got nursing before gastroscopy.
Data analysis was made by using error, percentage, the mean, standard deviation, statistical analysis, paired t-test, Wilcoxon signed-rank test, and ANCOVA method with the application of SPSS 21.0 program(SPSS Inc, Chicago, IL).
The findings are as follows.
Hypothesis 1 as saying that ‘the endoscopic unit nurses' competency will be higher after participation than before participation in a simulation-based training program’ had statistically significant difference, thereby having been supported (t=-8.121, p<.001).
Hypothesis 2 as saying that ‘the endoscopic unit nurses' confidence will be higher after participation than before participation in a simulation-based training program’ had statistically significant difference, thereby having been supported (t=4.98, p<.001).
Hypothesis 3 as saying that ‘the satisfaction in examination patients with having gotten nursing before gastroscopy performed by the endoscopic unit nurses after participation will be higher than before participation in a simulation-based training program’ had statistically significant difference, thereby having been supported(F=4.661, p=.033).
Through this study, a new training method was suggested by carrying out the nurse training on nursing before gastroscopy through operating a scenario using the standardized patient by developing the practice standard and guideline on nursing prior to gastroscopy. The effect was verified by carrying out the examinee satisfaction in order to confirm the effect of the simulation-based nursing training. A follow-up research is expected to be need to be actively arranged that strives to apply it to new employee training as well as health examination center employees and that verifies an effect of simulation-based nursing training, by developing a simulation training program on diverse examinations in the future.
