요양보호사의 임종돌봄 경험에 관한 연구 = A Research on Generic Care Workers' Experiences of End-of-Life Care

국문 초록 (Abstract)

본 연구의 목적은 노인요양시설에서 근무하고 있는 요양보호사들의 임종경험을 파악하여 내부자적 관점에서 지금까지 밝혀지지 않았던 요양시설에서 근무하고 있는 요양보호사의 임종경험에 대한 의미를 밝히는데 있다.
본 연구의 참여자는 노인요양시설에서 근무하고 있는 1년 이상의 요양보호사 9명으로 구성되었으며 자료 수집은 2013년 11월 20일부터 2014년 2월 20일까지 심층면접을 진행하였다. 수집된 자료는 현상학적 지오르기(Giorgi)의 연구 방법을 선택하여 노인요양시설 요양보호사의 임종돌봄에 대한 경험을 분석하였다.
수집된 자료로부터 도출한 의미 있는 진술은 총 452개였고, 이를 통한 의미 단위는 94개, 하위주제는 21개, 주제 4개로 분류하였다.
분석한 결과, 요양보호사들의 임종돌봄을 ‘아름다운 동행’으로 정리하였다. 노인요양시설에서 근무하는 요양보호사들이 경험한 임종돌봄을 통해 알 수 있었던 것은 ‘타인의 죽음에 대한 정서’, ‘요양보호사 역할’, ‘요양보호사 자세’, ‘죽음에 대한 인식변화’의 네 가지 주제로 정리할 수 있었다.
첫 번째 ‘타인의 죽음에 대한 정서’ 주제는 ‘대처의 미성숙성’, ‘정서적 회피성’, ‘의료적 비전문성’, ‘준비된 임종의 안정성’, ‘직무 책임성’, ‘업무적 자존감’ 하위주제를 포함한다. 두 번째 ‘요양보호사 역할’ 주제는 ‘신체적 돌봄’, ‘정서적 돌봄’, ‘영적 돌봄’, ‘가족 돌봄’의 하위주제를 포함한다. 세 번째 ‘요양보호사 자세’ 주제는 ‘긍정적 응대’, ‘세밀한 관찰’, ‘임종순간 옆에서 지켜주기’, ‘눈 감겨주기’, ‘마음열기’, ‘보호자 대행’의 하위주제를 포함한다. 네 번째 ‘죽음에 대한 인식 변화’ 주제는 ‘삶의 과정으로서 죽음 인정’, ‘삶에 대한 성찰의 시간 가짐’, ‘본인의 죽음을 생각하게 됨’, ‘임종돌봄 체험을 통해 숙련가가 됨’, ‘전문지식에 대한 필요성을 느낌’의 하위주제를 포함한다.
이러한 연구결과는 노인요양시설에서 그 동안 알려지지 않았던 요양보호사의 임종돌봄 경험에 대한 총체적인 이해를 제공하고 쓰일 수 있다. 그리고 노인요양시설 임종돌봄을 경험한 요양보호사를 대상으로 임종경험 사전․사후 서비스를 위한 프로그램 개발에도 기여 할 수 있다.

다국어 초록 (Multilingual Abstract)

The purpose of this study is to investigate the experiences of End-of-life care in geriatric care workers at nursing homes and to understand the meanings of their experiences of End-of-life care from the internal perspective.
Nine geriatric care workers who have worked for more than a year at nursing homes participated in this study; in-depth interviews were performed to collect data from November 20, 2013 to February 20, 2014. Giorgi's phenomenological method was used to analyze the experiences of End-of-life care in geriatric care workers at nursing homes.
A total of 452 meaningful statements were drawn from the collected data and were categorized into 94 meaningful units, 21 sub-areas, and 4 subjects.
As a result, geriatric care workers' End-of-life care was conceptualized as beautiful going-together. The experiences of End-of-life care in geriatric care workers at nursing homes could be categorized into four themes: emotions for others' death, geriatric care workers' roles, geriatric care workers' positions, and shift in awareness of death.
The first theme of emotions for others' death includes such sub-areas as immaturity in coping, emotional avoidance, medical unprofessionalism, stability in prepared dying, job liability, and occupational self-esteem. The second theme of geriatric care workers' roles includes such sub-areas as physical care, emotional care, mental care, and family care. The third theme of geriatric care workers' positions includes such sub-areas as positive service, careful observation, being together at the moment of dying, closing the eyes, opening the mind, and acting for a caregiver. The fourth theme of shift in awareness of death includes such sub-areas as acceptance of death as a part of life, having time to reflect on life, thinking about one's own death, becoming skilled through the experiences of End-of-life care, and feeling the need of expertise.
On the basis of the results of this study, it is possible to take the initiative in providing and using the general understanding of End-of-life care experienced by geriatric care workers at nursing homes. It is also possible to contribute to development of programs for pre- and post-service in End-of-life care that target geriatric care workers who have experienced End-of-life care at nursing homes.
