Hemiparetic stroke produces asymmetric ventilation with an increase in carbon dioxide sensitivity and a decrease in voluntary on the paretic. Altered respiratory mechanics and efficiency reflect impaired chest wall movement, asymmetry, and the degree of muscle paresis and spasm. The goals of long-term management for the patient with stroke include optimize chest wall excursion and ventilation, lung volumes and capacities.
The purpose of this study was to determine whether feedback breathing exercise might increase the pulmonary function of the patients with stroke.
Thirty-seven patients with stroke were randomly assigned to experimental group (feedback breathing device exercise(n=13), combination of diaphragmatic breathing and pursed-lip breathing exercise(n=10)) and control group(n=14). During four weeks, each group participated thirty minutes for three times per week.
Subjects were assessed using pre-value and post-value measurement chest length(chest length for resting, chest expansion) and pulmonary function(forced vital capacity, forced expiratory volume at one second, FEV1/FVC, peak expiratory flow, vital capacity, tidal volume, expiratory reserve volume, inspiratory reserve volume).
The results of this study were as follows.
1. In comparison of chest length for resting between pre and post value, the chest length for resting was not significant in the experimental and control group(p>.05). In comparison of the difference of the chest length for resting among 3 group, there was not significant in the experimental and control group(p>.05).
2. In comparison of chest expansion between pre and post value, the chest expansion was significant in FB group(p<.05). In comparison of the difference of the chest expansion among 3 group, there was not significant in the experimental and control group(p>.05).
3. In comparison of FVC between pre and post value, the FVC was significant in FB and CB group(p<.05). In comparison of the difference of the FVC among 3 group, there was significant difference between the FB and CB group, CB and control group(p<.05).
4. In comparison of FEV1 between pre and post value, the FEV1 was significant in FB and CB group(p<.05). In comparison of the difference of the FEV1 among 3 group, there was significant difference between the FB and CB group, CB and control group(p<.05).
5. In comparison of FEV1/FVC between pre and post value, the FEV1/FVC was not significant in the experimental and control group(p>.05). In comparison of the difference of FEV1/FVC among 3 group, there was not significant in the experimental and control group(p>.05).
6. In comparison of PEF between pre and post value, the PEF was significant in FB and CB group(p<.05). In comparison of the difference of the PEF among 3 group, there was significant difference between the FB and CB group, CB and control group(p<.05).
7. In comparison of VC between pre and post value, the VC was significant in FB and CB group(p<.05). In comparison of the difference of the VC among 3 group, there was significant difference between the FB and control group, CB and control group(p<.05).
8. In comparison of TV between pre and post value, the TV was not significant in the experimental and control group(p>.05). In comparison of the difference of TV among 3 group, there was not significant in the experimental and control group(p>.05).
9. In comparison of ERV between pre and post value, the ERV was significant in FB and CB group(p<.05). In comparison of the difference of the ERV among 3 group, there was not significant in the experimental and control group(p>.05).
10. In comparison of IRV between pre and post value, the IRV was significant in FB and CB group(p<.05). In comparison of the difference of the IRV among 3 group, there was significant difference between the FB and control group, CB and control group(p<.05).
These findings suggest that FB and CB group can be used to improve chest expansion, pulmonary function in stroke patient. In comparison of FB and CB group, CB group was more improved. but, FB group was significant chest expansion, pulmonary function.
In conclude, FB group helped improving function of pulmonary volume and respiratory muscle, and thus it indicates that the functions will be more improved through the continued respiratory exercise program.
