노인요양병원 질 평가 도구개발에 관한 연구 = Development Strategy of Quality Assurance Measurement Tools for Geriatric Hospitals

In this study, author tries to develop quality indicators for nursing facilities and discuss the key issues of pitfalls. The current quality indicators show too much inadequateness if it applied to quality evaluation.
Therefore after reviewing Center for Medicare & Medicaid’s comprehensive quality evaluation tools and Japanese nursing home evaluation tools, author renewed and developed the Korean Geriatric Nursing Facility Evaluation Tool (KGNFET).
When KGNFET apply to practice it showed critical discrimination between the Geriatric Hospital and Simple Nursing Care Facility which means quality difference between two groups.

국문 초록 (Abstract)

본 연구에서는 미국 CMS의 노인 요양기관 포괄적 질평가 도구와 일본의 노인 병원 질 평가 도구를 우리나라 실정에 맞게 개선하여 소위 단기 질 평가 도구를 이용할 때 발생할 수 있는 여러 평가 오류를 극복하고자 하였다.
본 연구의 결과는 노인요양병원과 노인요양원의 제공되는 서비스가 아직 전반적으로 충분하지 않으며 특히 노인요양원에서의 의료서비스제공 미흡이 두드러지게 나타난다고 할 수 있다.
아울러 노인요양기관 질 평가 도구는 구조지표와 질 지표 그리 가장 중요한 피할 수 있는 입원을 막기 위한 노력을 노인요양기관들이 얼마나 열심히 하는 가에 대한 질 평가가 반드시 포함되어야하고 특히 향후에는 노인요양원의 질 개선이 절실히 필요하다는 제언을 하고자 한다.
우리나라에서는 노인요양병원 및 단순 요양시설간의 구분이 모호하고 비용 효율적 구조를 가졌다고 보기 어려운 측면이 있다. 따라서 노인의료시스템선상에 노인요양병원의 역할이 보다 더 정확히 정의되어야한다.
