음악치료가 정신분열병환자의 지속적 주의력과 사회기능에 미치는 영향

국문 초록 (Abstract)

이 연구의 목적은 정신분열병 환자들을 대상으로 음악치료 프로그램을 시행하여 환자들의 지속적 주의력과 사회기능에 미치는 영향을 살펴보고자 한 것이다. 이 연구는 2006년 9월부터 2007년 3월까지 7개월 동안 대전 소재의 서구 b센터에서 시행되었으며, DSM-IV에 의거하여 정신분열병으로 진단받은 환자 중 국문 해독이 가능하여 질문지 내용을 이해할 수 있고, 글쓰기에 지장이 없으며, 의사소통 및 노래 부르기가 가능하고, 청각장애를 비롯한 다른 신체적 장애가 없으며, 악기활동에 지장이 없는 자 로 13명을 선정하였다.
연구 설계법은 사전 사후로 사회기능을 PEI(Physical, Emotional, Intellectual Function Rating Scale)척도로 측정하고, 지속적 주의력은 매 회기마다 체크리스트로 정리하였다. 또한 이 연구는 음악치료 기법 중, 오르프 음악치료기법을 사용한 연구이며, 오르프의 음악적 경험인 재창조의 경험, 창작의 경험, 감성적 음악의 경험을 토대로 집중력과 현실감각을 향상시키고 기억을 향상시킬 수 있는 기술을 익히며, 다른 사람과 감정이입, 정체성 등의 기술을 발달시키고, 자신의 생각과 느낌을 다른 사람과 의사소통을 할 수 있게 하며 다른 사람의 생각과 느낌을 수용할 수 있는 능력을 키운다. 또한, 책임감 증진, 기억기술을 발달, 내면 경험으로부터 커뮤니케이션을 발달, 마지막으로 수용력을 향상시켜 이 연구의 목적인 지속적 주의력과 사회기능에 치료효과를 보았다는 데에 의의가 있다.
이 연구는 음악치료가 정신분열병 환자의 지속적 주의력에 유의미한 차이를 보일 것이라는 가설 아래, 부적절한 의사소통을 감소시킬 것이며, 부적절한 신체활동을 감소시킬 것이며, 주의집중결여를 감소시킬 것이며, 자리이탈을 감소시킬 것이며, 지시미수행을 감소시킬 것이다. 라고 세웠다. 또한, 음악치료가 정신분열병 환자의 사회기능에 유의미한 차이를 보일 것이라는 가설 아래, 일상생활기능수준을 향상시킬 것이며, 대인관계기능수준을 향상시킬 것이며, 작업성취기능을 향상시킬 것이다. 라고 세웠다.
연구 결과, 음악치료가 정신분열병 환자의 지속적 주의력과 사회기능에 효과가 있음을 알 수 있었으며. 또한 사회기능의 향상을 위해 주의력 결함의 개선이 필요함을 알 수 있었다.

다국어 초록 (Multilingual Abstract)

The objective of this research is to engage schizophrenic patients with music therapy so as to explore the effects on their sustained attentiveness and social capacity. This research was conducted for a period of 7 months, from September 2006 to March 2007, at b center in Western Daejeon, Korea. Diagnosed as schizophrenic by DSM-IV, the 13 selected patients are able to read, write, and comprehend contents of the provided questionnaire, demonstrate communicative skills and singing abilities, show no signs of physical disabilities including hearing impairments, and are capable of handling a musical instrument.
Our research process includes a before and after measurement of social capacity in PEI, and a periodical checklist for sustained attentiveness. With the application of Orff music therapy, in particular his musical experiences of improvisation, composition, and expressiveness, patients will acquire better concentration gain a grasp of reality; pick up techniques for memory improvement; develop emotion and a sense of identity; and learn how to interact and accept others with different opinions. Also, by allowing the patients to attain an increased sense of responsibility, better memory, experience-based communicative skills, as well as open-mindedness, this research holds meaning in having proven to be an effective form of treatment in the areas of sustained attentiveness and social capacity.
Assuming that music therapy will cause a significant change in schizophrenic patients’ sustained attentiveness, we expect a decrease in inadequate communication, inappropriate physical motion, attention deficit problems, fidgeting and disobedience. Also, assuming that music therapy will cause a significant change in schizophrenic patients’ social capacity, we expect an improvement in everyday functionality, interpersonal skills, and work achievement standards.
Our research results have shown that music therapy has an effect on schizophrenic patients’ sustained attentiveness as well as social capacity. In addition, it was discovered that one’s level of attention should be heightened in order to improve social capacity.
