혈액투석 환자의 체험 : Parse의 인간되어감 연구방법론 적용 = Lived Experience of Hemodialysis Patients : applied Parse's Human Becoming Research Methodology

국문 초록 (Abstract)

인간의 건강은 한 개인이 독특하게 체험하는 “인간되어감(human becoming)'과정이며, 인간은 끊임없이 환경과의 상호작용을 통하여 자유로이 경험을 선택하고 책임을 지는 존재이다. 혈액투석 환자는 이러한 건강 체험의 과정 중에서 독특한 경험을 한다(Parse, 1981).
본 연구의 목적은 Parse의 ‘인간되어감 이론’을 적용하여 혈액투석 환자의 체험을 이해하고 혈액투석 환자의 체험을 이해함으로써 간호 실무에 도움이 되는 이론적 근거를 개발하고 혈액투석 환자의 간호를 위한 교육에 기여하고자 본 연구를 시행하였다.
본 연구의 기간은 2003년 8월부터 2004년 6월까지 실시되었으며, 연구 참여자는 20대 1인, 30대 1인, 40대 1인, 50대 3인, 60대 1인으로 총 7명으로 주치의로부터 만성 신부전 진단을 받고 K대학병원 인공 신장실에서 6개월 이상 혈액투석을 받고 있는 환자이다.
Parse의 인간되어감 연구방법론을 적용하여 자료를 분석하였다.
연구자는 참여자와 ‘나와 너’ 의 대화 과정에 의하여 자료를 수집하였다. 수집된 자료를 참여자의 언어에서 추출한 내용을 연구자의 언어로 종합하여 명제를 만들고, 명제를 모아 구조를 만들고, 구조를 종합한 후에 발견적 해석의 단계를 거쳐 자료를 분석 하였다.
자료의 신뢰성 및 타당성을 높이기 위해 Guba & Lincoln(1985)과 Sandelowski(1986)가 제시한 기준에 따라 신뢰성, 적합성, 감사가능성, 확증성에 대한 평가 절차를 거쳤다.
분석 결과 ‘혈액투석 환자의 체험’의 구조는 다음과 같다.
(1) 삶의 의욕상실로 인한 절망감.
(2) 대인관계 단절로 인한 상실감.
(3) 사회적 활동 위축으로 인한 좌절감.
(4) 의욕상실과 역할 불이행으로 인한 정신적 고통.
(5) 삶의 집착에서 벗어나 자유롭게 살고자 하는 희망.
(6) 절대자와 의미 있는 타인에 대한 의존성.
(7) 투석 생활의 적응과 자립욕구.
(8) 봉사 생활과 대인관계 회복으로 인한 긍정적 생활 변화.
이를 통해 혈액투석 환자의 체험의 구조를 통합하면 다음과 같다.
혈액투석 환자의 체험은 삶의 의욕 상실로 인한 절망감과 대인관계 단절로 인한 상실감을 가지고 있으며, 사회적 활동 위축으로 인한 좌절감과 의욕상실과 역할 불이행으로 인한 정신적 고통이 있으나 삶의 집착에서 벗어나 자유롭게 살고자 하는 희망과 절대자와 의미 있는 타인에 대한 의존성을 가지고 투석 생활에 적응과 경제적인 자립욕구가 있으며 봉사 생활과 대인관계 회복으로 긍정적 생활 변화를 갈망하는 과정이다.
Parse의 인간되어감 이론에 있는 개념을 사용하여 해석은 체험의 구조가 추상성이 더 높아지면서 다음과 같은 이론적 구조로 옮겨진다.
1. 혈액투석 환자의 가능성을 실현한다는 것은 상상하는 것을 연결-분리하는 방식이다.
이것은 과거에 받아들인 생각, 감정, 신념, 언행 등을 통해 한 현상과는 연결되고 다른 현상과는 분리되면서, 인간의 행동과 목적을 구체화하는 속에 새로운 가능성을 추구 하는 것이다.
2. 혈액투석 환자의 독창성을 나타낸다는 것은 가치화의 노출-은폐를 표현하는 것이다.
이것은 소중히 여기는 신념을 확인하고, 드러내거나 감추는 과정을 통하여 고유한 인간이 자신의 독창성과 자율성을 확인하는 것이다.
3. 혈액투석 환자의 변형성은 가능-제한의 언어화를 통해 나타난다.
이것은 가능과 제한이 동시에 공존하는 상황 속에서 개인의 독특한 현실을 언어적-비언어적으로 상징화함으로서, 환경과의 공동창조를 통해 일어나는 변화이다.
이상의 결과를 통해 혈액투석 환자의 체험은 인간과 우주와의 상호작용을 통해 끊임없이 변화하고 그들의 절망적이고 고통스러운 상황에도 의미를 발견하고 극복하며 긍정적인 변화를 위해 노력하는 인간되어감(human becoming)의 과정이다.

다국어 초록 (Multilingual Abstract)

The health of a human being is the process of 'Human Becoming' uniquely experienced by each individual. Human being freely chooses to experience and take responsibilities through constant interactions with the environment(Parse, 1981).
Hemodialysis patients experience unique experience during this health process.
The purpose of this study is to contribute to the development of practical theoretic basis and the education of nursing Hemodialysis patients by understanding the experience process and the actual course on the basis of Parse's ‘Human Becoming Theory’.
The duration of this study persisted from August 2003 to June 2004 and the total of seven patients - one each from age groups, 20s, 30s, 40s, 50s and 60s - who participated in this study had been diagnosed with end-stage renal disease by their attending physicians and have had blood dialysis at Artificial Kidney Room, K University Hospital for over 6 months.
Applying Parse's 'Human Becoming' methodology, the collected data has been analyzed.
The researcher had collected the data through 'I-You' conversation with the participants. The details extracted from the participants' languages of the collected data were generalized by the researcher's language to draw out a proposition; the propositions were collected to form a structure; and the structure was generalized and analyzed through heuristic analysis.
To improve the credibility and validity of the data, Credibility, Fittingness, Auditability and Confirmability Assessments were conducted on the basis of the standards proposed by Guba & Lincoln (1985) and Sandelowski (1986).
The analysis revealed the following structure of ‘Lived Experience of Hemodialysis Patients'.
(1) Grief due to loss of desire to live
(2) Feeling of loss due to social isolation
(3) Frustration due to unsatisfying social activity
(4) Psychological distress due to loss of desire and unability to perform given roles
(5) Hope to live free from the tenacity for life
(6) Dependence to mighty being or significant others
(7) Adaptation to Hemodialysis and desire to independence of economy
(8) Transition to positive lifestyle through volunteer activities and revitalization of social interactions
Therefore, the integration of the life structure of Hemodialysis parients is as follows:
The lived experience of Hemodialysis patients have grief due to loss of desire to live and feeling of loss due to social isolation along with frustration due to unsatisfying social activity and psychological distress due to loss of desire and unability to perform given roles, but is a process of developing hope to live free from the tenacity for life, being dependent to mighty being or significant others, adapting to regular dialysis for the desire to independence of economy and wishing for the transition to positive lifestyle through volunteer activities and revitalization of social interactions.
The interpretation using the concepts of Parse's Human Becoming Theory is as follows:
1. Powering of Hemodialysis Patient is a way connecting-separating imaging.
They is likely to connect and separate one phenomenon from the other through reflection of their thought, emotion, faith and way of life. And they also keep up with new possibility in the substantiation of human behavior and purpose.
2. Originating of Hemodialysis Patient is a manifestation of revealing-concealing valuing.
The aim is that they confirm their invaluable faith, and originality and self-control in the process of revealing and concealing.
3. Transforming of Hemodialysis Patient unfold in the languaging of enabling-limiting.
It is purpose the in the purpose that in the situation of the co-existence of enabling and limiting, they can interact with their environment through the symbolism of verbal-nonverval behavior.
The above results reveals that the 'Lived Experience of Hemodialysis Patients' is a process of 'Human Becoming' trying to change constantly by constantly interacting with men and the universe and to discover meaningfulness, overcome difficulties and change positively in spite of the despair and distress.
