地方公社 醫療院의 效率的 經營方案 硏究 = (A) Study on An Efficient Management Plan for the Public Medical Center

국문 초록 (Abstract)

이 연구는 지방공사의료원의 경영성과를 분석하여 경영상의 문제점을 찾아내고 이에 대한 적절한 경영개선방안을 제시함으로써 지방공사의료원이 갖는 본래의 기능을 보다 원활하게 수행할 수 있는 데 기여하고자 한다. 이를 위하여 전국 34개 지방공사의료원을 대상으로 그 경영성과를 대학병원 경영성과와 비교분석 하였으며 보다 구체적으로는 의료원 경영의 안정성, 수익성, 활동성, 성장성, 생산성 등 재무 비율분석과 함께 공공성 및 효율성을 실증분석 하였다.
분석결과, 지방공사의료원은 안정성 및 활동성에서 대학병원 보다 우수한 것으로 나타났으며, 수익성, 성장성 및 생산성은 낮게 나타났다. 대학병원에 비해 지방공사 의료원이 안정성 및 활동성에서 더 양호한 것으로 나타난 것은 그 설립취지인 지역경제의 활성화 및 지역주민의 복리증진을 위하여 지방자치단체가 지속적으로 지원한 결과로 보인다.
지방공사의료원이 수익성과 생산성 및 성장성부문에서 경영효율을 보다 향상시키기 위해서는 기업경영의 주요 패러다임인 수익성향상 개념을 지방공사의료원에도 도입하고 끊임없이 변화하는 국내외 의료환경변화에 적절히 대응할 수 있는 경영전략을 수립하여야 할 것이다.
자료포락분석(DEA)을 이용한 효율성분석 결과 대학병원은 기술(TE)의 비효율성이 규모(SE)의 비효율성보다 크게 나타났으나, 지방공사의료원은 규모(SE)의 비효율성이 기술(TE)의 비효율성보다 크게 나타났다. 특히 2003년도 기술의 비효율성을 보면, 지방공사의료원의 비효율성이 대학병원의 비효율성보다 월등히 크게 나타났으며, 효율성비율분석에서는 지방공사의료원이 전체 의료산업평균보다 낮았을 뿐만 아니라 대학병원보다도 효율성 측면에서 저조하였다. 이러한 결과는 지방공사의료원이 대학병원에 비하여 전반적으로 비효율적으로 운영되었음을 보여주는 것이다.
결론적으로 지방공기업 형태로 경영활동이 이루어지고 있는 지방공사의료원은 경영 내적으로는 수익성, 성장성, 생산성의 제고와 효율성증진을 통한 경영개선 노력이 필요하며, 경영외적으로는 관리운영체계 및 운영방식에 대한 재검토를 통하여 효율적인 경영방안을 재정립해야 한다. 이 연구에서는 주요개선방안으로 의료시설 등 장비의 현대화를 통해 고객의 신뢰를 확보하고, 양적·질적으로 우수한 민간의료기관에 경영을 위탁하여 지역주민에게 우수한 의료서비스를 공급하며, 지역적 특성에 맞는 전문 의료기관화로 수익성, 공공성, 효율성을 제고하는 방안을 제시하였다.

다국어 초록 (Multilingual Abstract)

This study intends to enable the Public Medical Center to perform its original functions more efficiently by finding out any weak side of its business performance and suggesting measures to improve its management performance through analysizing its business management.
For this, the management performance of 34 Public Medical Centers around the country is compared with that of the specialized general hospital. For more detail, this study analyzes positively the publicity and efficiency including financial analysis related to stability, profitability, activity, growth, and productivity of the Center's business management.
The results shows that the Public Medical Center is better than the specialized general hospital in sides of ability and activity. It, however, shows poor result in sides of profitability, productivity, and growth. In general, it seems that the continuous financial support of local governments for development of citizens' welfare and its economy affects favorable results of the Center in stability and activity.
To enhance management efficiency in terms of profitability, productivity, and growth, the Center should introduce the idea, enhancement of profitability, which is a major paradigm of business management. And, it should establish some business strategies which are fit for dealing with internal and external environmental changes in the sector of health and medicare.
The efficiency analysis based on the analysis of efficiency using Data Envelopment Analysis shows that in the specialized general hospital, inefficiency of Technical Efficiency(TE) was much bigger than that of Scale Efficiency(SE). However, in the case of the Center, inefficiency of SE was bigger than that of TE. Especially, in terms of the size of inefficiency in 2003, the inefficiency size of the Center is by far bigger than that of the specialized general hospital. Furthermore, efficiency ratio based on the financial analysis shows that the ratio of the Center is lower than the average of the whole health care industry, and also lower than that of the specialized general hospital. That result shows that the Center is operated much inefficiently than the specialized general hospital on the whole.
In conclusion, the Public Medical Center doing its business as a quasi-governmental organization needs to enhance its profitability, productivity, and productivity ratio and needs to improve its business performance through increasement of management efficiency. In addition, it should make some efficient business strategy with reviewing its operational structure and performing system of business management. This study suggests that the Center should get trust from the consumer with modernization of medical instruments, provide quality service for citizens through entrusting business management to private medical institutions, and improve profitability, public good, and efficiency through changing itself to a specialized medical institution for the community.
