한국 노인의 구강건강 실태조사 : 예산군 65세 이상 노인을 중심으로 = (A) Survey on Oral Health of the Aged Koreans : Surveyed on the aged people older than 65 years in Yesan-Gun

With extended average span of human life in line with the development of industrial and medical technology, currently Korea is becoming an aging society with increasing aged people. In case of agricultural villages, especially, young people goes to the big and medium-to-small cities while only aged people remains there, which accelerates our society to the aging one day by day. Along with this, the problem of aged people is serious, among which, problem of health care is much serious. Above all, oral health is really serious since it makes a great role in getting nourishment. The purpose of this study is to make survey on oral health status for the aged people older than 65 years living in the agricultural villages, find out the problems, and prepare the basic materials necessary to establish a plan to promote oral health of the aged people older than 65 years.
As for research method, the situation was studied and also its improvement method was investigated through a survey carried by a direct interview and oral examination of the aged people older than 65 years in Yesan-Gun. SPSPP10.0 / PC program was used for data process and analysis, and to customize this as a model case is not recommended since the target was limited to the aged people older than 65 years in Yesan-Gun. The survey result was turned out as follows.
88.7% aged people does not receive a regular oral examination, and it was surveyed that they go to the dentist only when they feel pain. And 93.3% was found to have received dental treatment. As for oral health education, it was investigated that 73.0% of them did not receive, and it was found out that they brush their teeth twice a day in average.
The aged people older than 80 years appear to have the highest dental prosthesis. And as for the required dental prosthesis, the necessity of partial denture was found a lot in the age of seventies; the full denture was investigated the highest in age of eighties; and it was investigated that prosthesis is required more by people whose medical care should be supported than people who subscribed medical health insurance.
As for production of prosthesis, 60% was made by non-authorized person whereas 40% by dentists, which means bad prosthesis is widely spread. The cost for it is found to be paid by themselves in the age of 65 to 69; in the age of eighties or more, it is frequently found that they are supported by their children and neighborhood.
In most cases, due to financial problem, they receive medical treatment by non-authorized person or they do not treat it even though they need that operation. The people in 65 to 69 replied that they are using well their prosthesis whereas it was investigated as the lowest in the age of eighties or more, and in most cases, they use their prosthesis for 1 to 10 years.
As for oral condition, all DTrate, Mtrate and DMFTindex were found the highest in the age of eighties or more. Remaining number of teeth and Ftrate was investigated the highest in the age of 65 to 69. It was investigated that more than 80% of the aged people has periodontal disease since it was turned out that CPITN mostly occupies No. 2 and 3 in almost ages.
As a promoting method of teeth health for the aged people focusing on the status of their oral health as investigated above, firstly, free movable oral examination system should be practiced. This helps them - who only used to go to the dentist only they have problem - to find their oral disease early and treat it through free movable oral examination system, and minimize the deterioration of oral disease. And this movable oral examination system should be practiced considering the regional characteristic as agricultural villages and aged people more than 65 years, also it should consider mental, physical, economical problem together with transportation and the distance between medical facilities.
Secondly, a systematic oral health education has to be prepared. More than 73% does not know how to maintain it since they did not receive oral health education, and they do not recognize the importance of oral health. There is even a proverb saying that People can leave with gum if they dont have teeth.
Thirdly, free prosthesis campaign should be widely practiced. Its because that it was turned out 52% of the subscribers of medical health insurance, and 63% of people for medical insurance need it but cannot manage it due to economical reason.
Fourth, the study how to stop prosthesis operation of non-authorized person. It was investigated that it is in serious condition since 40% of person received their treatment by non-authorized person. Now it is urgent to stop to produce bad prosthesis considering it is a major reason of dental caries and periodontal disease.
