뇌졸중 환자의 우울 및 불안 특성과 환측 상지기능과의 관련성 = The Relationships between Depression, Anxiety, and Hemiplegic Upper Extremity Function in Stroke Patients

The purpose of the study was to know the characteristic of depression and anxiety to investigate the relationship between depression, anxiety, and hemiplegic upper extremity function in stroke patients. Participants of this study were 119 stroke patients who were registered in 4 rehabilitation center based in Daegu·Gyeongbuk enrolled in the study. And data were collected during the period from January 2012 to february 2012. Manual function test(MFT) was used to evaluation hemiplegic upper extremity function. Korean version of the Beck Depression Inventory(K-BDI) was used to evaluation depression. State Trait Anxiety Inventory-Korean YZ(STAI-KYZ) was used to evaluation anxiety. Data were analyzed by SPSS 12.0 program using independent t-test, one way ANOVA, and pearson correlation coefficient. As a result, 26.9% of patients in STAI-KYZ analysis showed to have State anxiety. And 21% of patients in STAI-KYZ analysis showed to have Trait anxiety. 58.5% of male and 59.5% of female patients in K-BDI analysis showed to have depression. The anxiety of stroke patients was significantly different depending on premorbid job, monthly income. The depression of stroke patients was also significantly different depending on marital status, premorbid job, monthly income. There was no correlation depression, anxiety, hemiplegic upper extremity function. Therefore, It was found that severity of depression and anxiety in stroke patients. It is necessary to develop the various program for psychologic support for stroke patients and to develop strategies to social·economic support.

국문 초록 (Abstract)

본 연구는 뇌졸중 환자의 우울과 불안의 특성을 파악하고 환측 상지기능과 우울 및 불안의 관련성을 알아보고자 실시하였다. 대상자는 대구·경북에 소재하고 있는 재활병원 4 곳에서 입원 하여 치료받고 있는 뇌졸중 환자 119 명을 대상으로 하였으며, 자료수집 기간은 2012 년 1 월부터 2 월까지 2 개월 동안 실시되었다. 연구 도구는 환측 상지기능 평가를 위해 뇌졸중상지기능평가, 우울을 측정하기 위해 한국판 Beck 우울증 척도, 불안을 측정하기 위해 상태 특성 불안검사 KYZ 형이 사용되었다. 대상자의 일반적 특성에 따른 우울, 불안, 환측 상지기능의 차이는 t-검정, 일원 분산분석 하였고 우울, 불안, 환측 상지기능과의 상관관계는 피어슨 상관계수로 분석하였다. 연구 결과 대상자의 26.9% 가 상태불안을 겪고 있고, 21% 가 특성불안을 겪고 있었으며 남자는 58.5%, 여자는 59.5% 가 우울을 겪고 있었다. 발병 전 직업, 수입에 따라 불안의 차이를 보였으며 결혼상태, 발병 전 직업, 수입에 따라 우울의 차이를 보였다. 환측 상지기능은 우울 및 불안과 상관관계가 없었다. 따라서 뇌졸중 환자에서 우울 및 불안의 심각성을 알 수 있었으며 환자의 심리적 지지를 위한 다양한 프로그램의 개발과 사회·경제적 지지를 위한 대책이 필요함을 알 수 있었다.
