노년기 자살행동 경험의 의미와 과정에 관한 근거이론 연구 = The Meaning and Pathways of Suicidal Experiences among Older Adults: a Grounded Theory Method

This study aimed to understand experiences of Korean older adults who have attempted suicide through exploring the motives and processes related to suicidal behavior in the rapidly aging Korean society. In this study using grounded theory method, in-depth interviews with 31 elderly participants, field notes, and theoretically related documents were analyzed.
A tentative model exploring pathways to suicidal behavior among older adults is described with six main elements. Seesawing in dynamic of despair and hope was the phenomenon experienced among older suicide attempters. The other elements were as follows: deficiency and loss with ageing, tedious life with no desire as causal conditions; broken connection with families, lack of public pension system, normlessness in later life, and invisible being in the society as context; myths about elderly depression and suicidal behavior, relatively easy access to anti-depressant and other products for self-harm, and lack of social support as intervening conditions; and overdose with sleeping pills or anti-depressants for a band-aid and other self-neglect behaviors as action strategies. Finally, as consequences of the action strategies, these older adults tried to find meaning in life and death while suffering from sequela after conducting intentional self-harm and still considering the option of killing oneself. This study advances the understanding of the underlying, dynamic context of suicide behavior in later life. Based on the results, it addresses the implication for the elderly suicide prevention and geriatric practice.

국문 초록 (Abstract)

본 연구의 목적은 노인 자살행동 경험의 의미는 무엇이며, 자살시도는 어떠한 과정으로 일어나는가를 살펴보는 것이다. 근거이론 방법을 활용하여 자살행동을 한 노인 31명과의 심층 면담과 이론적 메모를 통해 얻은 자료를 분석하였다. 노인 자살행동 경험의 의미와 그 과정을 설명하는 실체이론을 여섯 가지 구성요소를 통해 설명하였다. 노인자살행동 과정의 중심현상은 ‘절망과 실낱같은 희망사이에서 시소타기’로 나타났다. 인과적 조건으로는 ‘삶의 결핍과 상실의 심화로 인한 심리적 위축’, ‘무료하고 막막한 일상’, ‘안정제 또는 수면제에 의지하는 밤’, 그리고 ‘삶에 대한 의욕 없음’이 있었다. 맥락적 조건은 ‘의지할 가족의 부재’, ‘노후대책이 미비함’, ‘노년기 삶에 대한 구체적 좌표의 부재’, ‘현대사회에서 노인의 자리는 없음’으로 나타났다. 중재적 조건으로는 ‘절망을 부추기는 조건들’과 ‘희망을 품게 하는 조건들’이 있다. 작용/상호작용 전략은 ‘자살방법 연구하며 때를 기다리기’, ‘자기방임과 자해’, ‘고비 넘기기’로 도출되었다. 마지막으로 결과는 ‘삶의 의미 찾기’, ‘죽는 것도 복이고 능력이며 운명이라 생각함’, ‘자살시도 실패 후 자살시도에 따른 후유증 겪음’ ‘여전히 자살시도의 유혹을 강하게 느낌’으로 나타났다. 연구 결과를 바탕으로 노인자살예방과 노년기 삶의 증진을 위한 구체적인 네 가지 실천적 방법 및 후속 연구를 제안하였다.
