사회경제적 박탈이 건강에 미치는 영향

국문 초록 (Abstract)

본 연구는 사회경제적 박탈이 건강에 미치는 영향을 박탈지수를 이용하여 살펴보고, 건강수준(주관적 건강수준, 만성질환 이환 여부)에 미치는 영향이 큰 박탈요인을 중심으로 보건정책적 함의를 제공하기 위한 연구이다. 분석에서는 2006년도 한국복지패널조사자료를 이용하여 총 7,072가구를 대상으로 해당설문지에서 23개의 박탈항목을 구성하고 이를 점수화하였다. 박탈점수를 합산한 총점과 건강수준과의 연관성을 살펴본 결과 사회경제적 박탈을 많이 경험할수록 주관적 건강수준은 낮아지고 만성질환 이환율이 높아지는 것으로 분석되었다. 구성된 박탈항목들을 바탕으로 요인분석을 실시하여 10개의 요인변수를 추출한 뒤 자료의 특성을 따라 주관적 건강수준의 경우 순서형 회귀분석을, 만성질환 이환 여부는 로지스틱 회귀분석을 실시하였다. 연구 결과 주관적 건강수준과 만성질환 여부에 영향을 미치는 요인들의 특징은 대부분 경제적인 결핍으로 인한 박탈로 “절대적 박탈”과 “직업수입 박탈”이 주요한 영향을 미치고 “주거의 질적 박탈”도 주요한 요인으로 밝혀졌다. 사회적 요인으로는 “가족관계 박탈”과 “사회적지지”가 주관적 건강과 유의한 관계인 것으로 나타났다. 따라서 이들 절대적 박탈을 경험하는 가구를 우선으로 하여 1차 의료의 접근성 향상과 주거의 질적 향상에 비중을 두는 정책과 더불어 사회구성원으로서의 기본적 생활양식을 보장하는 사회적 안전망 보강 등의 보건복지 정책을 수립할 필요가 있다.

다국어 초록 (Multilingual Abstract)

This study aims to examine the effect of socioeconomic deprivations on self-rated health (SRH) and chronic diseases. From Korea Welfare Panel Study data (KOWEPS, 2006) deprivation index is constructed to measure the various dimensions of deprivation. We conducted correlation analysis, factor analysis, ordinal regression for SRH, and logistic regression for chronical diseases. Results show that experiences of socioeconomic deprivations are associated with low self-rated health and more chronic diseases: absolute deprivations, job-income related deprivations and deprivations in residental qualities affect both SRH and chronical diseases. In addition, deprivations in family relationship and social supports are associated with low SRH. To promote population health, health and social policy needs to target those who experience deprivations, strengthen primary care services, and improve the standard of living of those who experience deprivations.
